ANNOTATION Perekhodko N. M. Medical Service in the Volhynian Voivodship (19211939). Monograph. The comprehensive health care research in the Volhynian Voivodship in the internar period was carried out in the academic paper. Organization of medical assistance of the population of Volhynia and health protection work of the state, selfgovemment authorities and the public during the Second Polish-Lithuanian Commonwealth was reconstructed. The state of scientific study of the subject and the source base is analyzed. On the basis of the historiographic works of Ukrainian and foreign scholars, it became clear that there was a study of individual health care problems in the Western Ukrainian lands in historical scientific thought, while a comprehensive study on health care in the Volhynian Voivodship was not conducted. In the scientific works on the history of medicine in the Western Ukrainian lands, the development of health care in the interwar period is analyzed mainly in Galicia. Unpublished documents found in archival repositories of Ukraine and the Republic of Poland, as well as published documents, print media materials and memoirs have made the source base. The use of general scientific and special research methods facilitated the optimal use of sources and scientific works for the disclosure of the problem and implementation of the reasoned and wellconsidered conclusions. The organizational structure and mechanism of the health care system functioning in Poland in the years since its statehood establishment has been investigated. The medical sector was controlled by the
Ministry of Public Health, and after its reorganization, public health care control functions were distributed among various state governmental institutions. The health care management system in the Volhynian Voivodship was organized between the Department of Health at the Voivodship Administration and the county public health departments. The county doctor was the main representative of the state health service at the county level. For the medical provision of the population and its control, the voivodship territory was divided into sanitary districts. The activities of doctors were regulated by the President s Order on Medical Practice. Professional activity of doctors of all specialties was coordinated by the self-governing institution - the Chamber of Doctors. The Volhynian doctors were under the jurisdiction of the District Lublin Chamber of Doctors, which in its turn was subordinated to the central seat in Warsaw. Since 1938, there was a separate Chamber of Dentists in Poland. In health care human resourcing in the Volhynian Voivodship, the shortage of medical workers against the backdrop of 3
population growth was a tangible problem observed throughout the period under investigation. Along ethnic lines, Poles and Jews dominated in the medical sector. Activities of healthcare institutions in the Volhynian Voivodship were restored. Medical attendance was provided to the population by inpatient facilities and outpatient settings of different types. The most numerous were medical institutions financed by the local self-government bodies, and functioning of the rest of the medical institutions was covered by the state, private individuals and the public. The insufficient number of hospitals, high prices for medical services and the lack of qualified medical personnel adversely affected the provision of wide access to proper medical care. The Jewish population used the services of national medical institutions. The medical care of rural residents remained unsatisfactory. Pharmaceutical institutions were mostly held by private owners, and only a small number of them belonged to the municipal self-government bodies. This caused high cost of medicines, despite the state regulation of pricing policies. The work of the sanitary and epidemiological service is traced. Unsatisfactory sanitary condition in settlements, low level of culture in everyday life and lack of knowledge on sanitation and hygiene contributed to the growth of infectious diseases, which sometimes exceeded the epidemiologic cutoff and threatened the state security. Over the interwar period, the Polish authorities have developed the principles of infectious diseases control. Systemic antiepidemic actions
partially stopped outbreaks of infections and reduced mortality of the population from infectious diseases. Reducing the number of tuberculosis patients was one of the important tasks at the domestic level. Implementation of the anti-disease measures in Poland was under the patronage of the president. A set of actions aimed at the protection of maternity and childhood has been identified. Consultation units worked in each county center - mother and child custody stations, which were maintained at the expense of the municipal authorities and public organizations. Medical workers conducted educational work, promoted the raising of the cultural and educational level on maternity and childhood, overcoming the practice of poorly qualified obstetrics. By their work, these communities covered a small number of women, mostly urban. The situation with the opening of specialized maternity institutions and the presence of qualified medical personnel remained unsatisfactory in the Volhynian Voivodship. There existed traditional practice of maternity patients care by people who made use of folk medicine knowledge, especially in rural areas. The responsibility for health care of school-aged children was partially laid on a doctor who worked in an educational institution. The maintenance of a doctor 4
and the provision of medical services depended on the funding and availability of medical staff. It has been established that sanatory and health care facilities in the Volhynian Voivodship were at the stage of creation, mainly those were establishments for the improvement of tuberculosis patients in the forest zone. One of the balneological establishments was opened in the 1930s in GutaStepanska of the Kostopil district. A small number of inhabitants of the Volhynian Voivodship improved health in well- known sanatoriums in Poland, but the high cost of services made it impossible for the majority of the population to receive such a kind of treatment. It has been shown that in order to improve medical care in Poland, public health insurance was introduced through the creation of Sickness Funds. The mechanism of their operation consisted in providing compulsory sickness insurance of workers and employees and paying a certain monetaiy contribution. These institutions were not subject to state administration, were public associations and had a status close to local self-government. The main centers of Sickness Funds functioned in the two largest cities of the Voivodship - Lutsk and Rivne and covered the population of the counties according to the territorial principle. Insured persons and members of their families, mostly urban population, used the services of insurance medicine. In 1933, Sickness Funds were reorganized into insurance companies. Insurance medicine, supported by the Polish authorities, did not become compulsory at the state level. It was established that in
order to protect professional interests and to exchange professional experience, medical workers from the Volhynian Voivodship joined the Polish associations, as well as established their regional organizations. Doctors participated in the activities of Polish public associations - the Union of Doctors of the Polish State, the Polish Union of Dentists and the only regional - the Scientific and Medical Society in Rivne. Jews joined the National Society for the Protection of the Jewish Population and took care of the medical provision of the Jewish community: they operated Jewish hospitals, set up ambulatory clinics and carried out educational and preventive activities among the population. Volhynian doctors of the Ukrainian origin did not create their own national community due to limited quantitative composition, but they participated in the work of the Galician Ukrainian Medical Society, which was operating in Lviv. In conditions of fierce competition with Polish and Jewish doctors, the UMS facilitated the consolidation of doctors by nationality and increase of their professional level. It is proved that the Volhynian department of the public institution of the Red Cross played an important role in the health care of the Volhynians. 5
Various forms of work of the society consisted in the creation of hospitals, ambulatory clinics, medical aid stations, training of rescue and sanitary wards, and conducting training courses for junior medical personnel and sanitaryeducational activities. Medical care was provided to socially vulnerable groups of the population who needed free care. 6
