adam_text | CONTENTS Foreword: Find the Differences by Amos Oz 11 Author s Note 17 Introduction: What is Gender Medicine? 21 Over six million years of evolution men and women assumed different tasks and duties in order to perpetuate the species. This specialization of men as hunters and warriors and. women as gatherers and caretakers led, among other reasons, to hardwired genetic changes and variations in the way the bodily systems function in both sexes. 1 : Sex, Gender, and Personalized Medicine 31 Definitions of maleness, masculinity, femaleness, femininity, sex and gender are discussed, as is how our genetic makeup and the environment in which we live determine health and disease. 2: Life in the Womb—Part One 45 Fetal development is when the stage is set for future health and disease. What happens during this time is termed fetal programming, and this process will be explored specifically from the gender angle. Genetics, epigenetics, and intrauterine hormonal environment will be introduced and their various impacts on later life will be shown. 3: Life in the Womb—Part Two 69 Gender aspects of fetal programming will be discussed as they relate to the development of future disease. 4: Emotional Stress During Pregnancy 85 We will discuss the impact of stress on pregnant women and her fetus. Maternal stress which may irreversibly disturb stress-coping mechanisms in her fetus, is associated with preterm deliveries and may also cause mental disturbances in offspring.
Gender Medicine 5: Gender Aspects of Heart Disease 95 This chapter deals with gender aspects of the human heart and gender differences in diagnosis and treatment of heart disease. 6: The Digestive Tract—Gender Aspects 115 In this chapter, the functional intricacies of the gastrointestinal system and the gender-related differences in the function of the various components of the gastrointestinal tract will be addressed. 7: Our Intestines—The Second Brain and the Life within the Gut 125 Two major aspects of the gastrointestinal tract will be discussed at greater length: The first aspect relates to the microbioma, the huge population of bacteria that inhabits our intestines and with whom we live as a functional and symbiotic unit. The second is what has been termed “our second brain, ” namely the nervous system, which resides within the walls of our intestines and functions largely with little control by our brain. Gender aspects of these systems will be emphasized. 8: Gender Aspects of the Reproductive System 141 In this chapter, the differences in the reproductive systems will be discussed from the angle of divergent reproductive strategies, courtship, and the impact of differential reproductive processes. Finally, different types of disease of the reproductive organs in men and women will be discussed. 9: Gender Aspects of Infertility 153 This chapter complements the previous chapter. Here we will discuss the differential therapeutic approaches related to men and women and also how men and women cope differently with infertility and infertility treatment. 10: Gender Aspects
of Pain 167 The basic concepts and the different types of pain will be discussed both in general terms and from the gender angle. Women perceive pain differently than men and react to certain pain medications differently than men.
Contents 11 : Too Hot, Too Cold—Gender Aspects of Temperature Regulation 185 This chapter deals with the question of why men and women experience hot and cold differently and why their bodies react differently to temperature changes. Being cold and feeling cold have different causes and they differ between the sexes. 12: Men—The Weaker Sex 193 In this chapter we will examine gender medicine from the angle of the male. It will be demonstrated that throughout the male life cycle, the male is actually the weaker sex. There is a substantially higher loss rate of male fetuses during pregnancy, a higher perinatal mortality rate in boys and a shorter life span in men. 13: Are Men an Endangered Species? 211 We will discuss the theory that man is actually an “endangered species and that the continuous deterioration of the male Y chromosome may lead eventually to the disappearance of men within a few hundred thousand years. 14: Gender Aspects of Doctor-Patient Relationships 219 In this chapter we will discuss various aspects of interpersonal communication, verbal, and nonverbal channels with emphasis on the encounter between physicians and patients. 15: Gender Medicine—Future Directions 239 Exponential developments in research related to gender and sex-specific medicine during the last decade clearly indicate that gender differences can no longer be neglected by the medical community. Moreover, in this chapter we will discuss why, even with the best of intentions, it is hard to introduce changes in attitudes and how gender medicine can be promoted. Acknowledgments 249 Endnotes 251

YOU DON’T KNOW THE HALF OF IT O ver millions of years, male and female bodies have developed crucial physiological differences to improve the chances for human survival. In Gender Medicine, Professor Marek Glezerman, one of the world s leading experts on this new area of medicine, reveals countless differences between the sexes that are frequently overlooked. Often culturally obsolete with the overturning of traditional gender roles, they are nevertheless very real, and go well beyond the obvious sexual and reproductive variances. For instance, women are more resistant to infectious diseases than men, but are more likely to suffer from autoimmune diseases; female smokers are at greater risk of developing colon cancer than male smokers; Alzheimer s is expressed differently in men and women; the pain threshold is not the same for men and women; drugs to treat nausea work less well for women, and women are more sensitive to antihistamines; aspirin is more effective in preventing strokes in women, but more effective in preventing heart attacks in men... and there is more. However, the medical establishment still largely treats men and women as though their needs were identical - and medical research is done predominantly on men, with the results then applied to the treatment of women. It is therefore time for a paradigm change - and such a change is the purpose of Gender Medicine.
