Beschreibung:Twenty-five years ago on March 17, 1943, in the small, but beautiful medieval town of Kampen in the Netherlands, a young woman with contracted kidneys and chronic uremia was treated with an artificial kidney. Yet, it was believed for many years that the artificial kidney had nothing to offer in terminal chronic renal disease. Partly this was true before the permanent cannulation of blood vessels was practical. But, Scribner introduced his A. V. shunt cannulas more than eight years ago and presently only one and one-half percent of the patients, doomed to die in chronic renal failure, receive the benefit of either dialysis or kidney transplantation. I will not discuss the excuses, selection committees, and other avenues of escape that have been created to condone and give an appearance of legality to this situation, but I will define three words that are often con­ fused. Triage. Under conditions of sudden emergency such as an explosion, a massive few physicians quickly review the railroad accident, an earthquake, or war, a victims and try to determine how the best use can be made of medical facilities and personnel to save the largest number. Evaluation. The physician or physicians collect meaningful data concerning the patient. They resort to expert help wherever possible, including reports from social workers, laboratory data, medical specialists, and perhaps psychiatrists
