The first-time appearance of a murmur in an adolescent can create a sub-
stantial amount of anxiety in the parents and the teenager. The appropriate
evaluation and diagnosis is very important in decision making regarding
sports participation in this population. Accurate identification of the inno-
cent murmurs can obviate the need for echocardiography. Identification
of a pathologic murmur may reduce morbidity and, possibly, mortality in
critical lesions such as hypertrophic cardiomyopathy. This article dis-
cusses the physiology and characteristics of different murmurs and out-
lines an approach to cardiac murmurs in adolescents.
A Teen with Chest Pain 17

Chest pain is a frequent symptom and complaint in the teenage population.
There are many common, benign causes of chest pain. However, it can cre-
ate tremendous anxiety in the mind of the patient and family members and
sometimes it can be perplexing even for the primary providers especially if
there is associated family history of premature coronary artery disease.
This article focuses on the evaluation of chest pain and how to differentiate
between noncardiac and cardiac causes of chest pain in teenagers.
A Teenage Fainter (Dizziness, Syncope, Postural Orthostatic Tachycardia Syndrome) 29

This article informs the general pediatrician about the diagnosis, evalua-
tion, and treatment of teenage patients with presyncope and loss of
consciousness. The focus is on distinguishing noncardiac fainting from
life-threatening syncope. Current treatment strategies of vasovagal syn-
cope and postural orthostatic tachycardia syndrome are also outlined.
The Asymptomatic Teenager with an Abnormal Electrocardiogram 45

Use of medications for attention-deficit hyperkinetic disorder and preparti-
cipation sports physical examination has led to an increase in number of
electrocardiograms (ECG) performed during adolescence. Interpreting
ECGs in children and young adults must take into account the evolutionary
changes with age and the benign variants, which are usually not associated
with heart disease. It is crucial for primary-care providers to recognize the

changes on ECG associated with heart disease and risk of sudden death. In
this article, the significance, sensitivity, specificity, and the diagnostic
workup of these findings in the asymptomatic teenager are discussed.
The Teenager with Palpitations
Farshad Sedaghat-Yazdi and Peter R. Koenig
Palpitations can result from cardiac awareness (increased conscious per-
ception of the heart beating) or from a fast or irregular cardiac rhythm.
Most causes for palpitations in the teenager can be diagnosed with mini-
mal testing. Patients with an abnormal ECG, non-sinus tachycardia,
abnormal cardiac examination, concerning family history, or palpitations
associated with activity or syncope should be referred to a pediatric cardi-
ologist. This article discusses the evaluation, testing, and management of
teenagers with palpitations. It also provides a general guideline for referral
for subspecialty evaluation.
Management of a Hyperactive Teen and Cardiac Safety

Since the earliest descriptions of attention deficit hyperactivity disorder,
controversy has prevailed as to the existence and appropriate manage-
ment of this condition. Often diagnosed in childhood, symptoms of atten-
tion deficit hyperactivity disorder can continue into adolescence and
adulthood, requiring lifelong therapy. Effective therapeutic interventions in-
clude stimulant medications with all their respective potential side effects,
including the cardiovascular system. However, although initial studies
raised concerns for an increase in serious adverse cardiovascular effects
among children receiving these drugs, more recent and extensive reports
have failed to substantiate those findings among young patients.
Sports Participation During Teenage Years
James M. Galas
Ensuring the safety of young athletes is a priority among health care pro-
viders. Controversy remains as to the best method of preparticipation
screening. The American Academy of Pediatrics and the American Heart
Association currently recommend screening with history and physical
examination, without routine electrocardiogram or echocardiography.
Meticulous conduction of a cardiac focused history and exam during the
preparticipation evaluation can help identify those who may be at risk for
sudden cardiac arrest. Understanding presenting signs of the most com-
mon cardiac diseases is useful in recognizing those in need of a directed
cardiac evaluation before sports participation.
Cardiac Trauma During Teenage Years
Peep Talving and Demetrios Demetriades
The spectrum of blunt cardiac trauma ranges from asymptomatic myocar-
dial contusion to fatal cardiac arrhythmias and/or cardiac rupture. Although
cardiac rupture is common in fatal traffic accidents, these patients rarely
reach hospital care. Insignificant blunt cardiac injury during sports may
cause fatal arrhythmia in teens. Penetrating cardiac trauma is fairly

common in the United States, encountered frequently in major urban
centers. Most patients are dead at the scene and never reach hospital.
The incidence of cardiac sequelae in survivors is high, and these patients
should be evaluated with early and late echocardiography to detect
anatomic or functional cardiac involvement.
Hypertension in the Teenager 131

Over the last two decades, essential hypertension has become common
in adolescents, yet remains under-diagnosed in absence of symptoms.
Diagnosis is based on normative percentiles that factor in age, sex and
height. Evaluation is more similar to adult essential hypertension than child-
hood secondary hypertension. Modifiable risk factors such as obesity,
sodium consumption and low exercise should be addressed first. Many
anti-hypertensive medications now have specific regulatory approval for
children. Sports participation need not be limited in mild or well-controlled
cases. Primary care physicians play an important role in reduction of car-
diovascular mortality by early detection and referral when needed.

Obesity in the child and adolescent population is a growing problem. This
article discusses the identification of obesity and overweight youth using
body mass index and anthropometry and reviews literature to show the re-
lationship between obesity and premature cardiovascular disease.
Cardiomyopathies Encountered Commonly in the Teenage Years and Their
Presentation 173

Cardiomyopathy encompasses a broad range of inherited and acquired
abnormalities affecting the myocardium. This heterogeneous group of dis-
orders is an important cause of morbidity and mortality in the adolescent
patient as a result of the presence of systolic or diastolic dysfunction, as
well as the risk of coexisting arrhythmias and sudden cardiac death. It is
important for the primary care physician involved in the care of adoles-
cents to understand the different causes of cardiomyopathy and their typ-
ical clinical presentations. The cause, pathogenesis, clinical presentation,
and diagnostic evaluation for adolescents with cardiomyopathy are re-
viewed. An overview of treatment modalities is also presented.
A Pediatric Approach to Family History of Cardiovascular Disease: Diagnosis, Risk

The medical family history is a comprehensive and dynamic record of ill-
nesses and other pertinent health information among family members.
Family history is used to facilitate diagnosis, to identify family members
at risk for developing a particular disease, and increasingly to manage dis-
ease. This article reviews the application of family history to pediatric

cardiovascular disease. As more is learned about the genetic basis of car-
diovascular disease, the family history will play an increasingly central role
in management. Improved understanding of the causes of pediatric car-
diovascular disease promises the opportunity to develop new diagnostic
and therapeutic strategies.
Caring for a Teen with Congenital Heart Disease 207

Adolescents with congenital heart disease (CHD) are a rapidly growing
population with complex medical needs and psychosocial challenges.
Identity formation is an important developmental task accomplished dur-
ing the teenage years. This article reviews different aspects of ongoing
care that pertain to teenagers with CHD, with a particular focus on primary
care issues and a summary of recommendations from various scientific
societies. A successful smooth transition to the adult health care setting
should achieve two important goals: to prevent loss of follow-up and to
foster and encourage self-care behaviors.
