Rhytidectomy remains a challenging surgical procedure for even the most experi¬
enced aesthetic plastic surgeons. The challenges are compounded by com¬
plications that are inherent to this procedure and place added pressure on the
doctor-patient relationship. Expectations for both parties are high and the margin
for error nil. This article presents a personal approach to the avoidance and man¬
agement of complications associated with facelift surgery. It presents the author s
personal approach as a plastic surgeon in the practice of aesthetic plastic surgery
over the past

years. Clinical pearls are provided to obtain optimum results in
rhytidectomy and limit associated sequelae.

Although office-based anesthesia for facial cosmetic surgery remains remarkably
safe, no anesthesia or sedation performed outside the operating room should be
considered minor. Proper organization, preparation, and patient selection, close
collaboration with the surgeon, and expert and effective anesthesia care will in¬
crease patient safety and improve pertoperative outcomes and patient satisfaction.
This article presents a comprehensive overview of anesthesia in terms of facial
plastic surgery procedures, beginning with a broad review of essentials and pitfalls
of anesthesia, followed by details of specific anesthetic agents, their administration,
mechanism of action, and complications.
Complications of Patient Selection: Recognizing the Difficult Patient

This article reviews some basic principles of patient selection for facial plastic
surgery. There are patients who are not good candidates, independent of the defor¬
mity and the ability of the surgeon. Reasons include subtle and not so subtle psychi¬
atric disorders, unrealistic expectations, lack of communication despite multiple
visits, and litigious patients. Complications or
suboptimal
results are not well
handled in these patients and often produce an uncomfortable experience for the
surgeon and staff in the postoperative period. These patients are best avoided or
should be provided a much longer evaluation period prior to any surgery.

Adverse reactions and unintended effects can occasionally occur with toxins for
cosmetic use, even although they generally have an outstanding safety profile. As
the use of fillers becomes increasingly more common, adverse events can be ex¬
pected to increase as well. This article discusses complication avoidance, address¬
ing appropriate training and proper injection techniques, along with patient selection
and patient considerations. In addition to complications, avoidance or amelioration
of common adverse events is discussed.
Complications in Hair Restoration

Hair restoration requires a high level of specialized skill on the part of both the
surgeon and the assistant team. Recipient-site problems can manifest from either
surgeon or assistant error. The surgeon can create an unnatural hairline due to
lack of knowledge of natural hair-loss patterns or badly executed recipient sites.
He must also be cognizant of how hairs naturally are angled on the scalp to re-create
a pattern that appears natural when making recipient sites. Assistants can also
greatly contribute to the success or failure of surgery in their task of graft dissection
and graft placement.
