This review discusses the preoperative evaluation of patients preparing for
elective colorectal resection, touching on several specific categories of
morbidity, including cardiac, pulmonary, hepatic, renal, and surgical site
complications. For each of these, the evidence for practices that optimize
patient function and minimize risk is reviewed. Finally, authors discuss how
to counsel high-risk surgical patients, including those for whom elective
surgery is not recommended.
Fast-Track Pathways in Colorectal Surgery 21

Enhanced recovery after surgery or fast-track pathways are a multimodal
approach to the perioperative management of patients undergoing colo-
rectal surgery designed to improve the overall quality of care. These path-
ways use existing evidence to streamline and standardize the
perioperative management of patients to improve pain management,
speed intestinal recovery, and ultimately facilitate a more rapid hospital
discharge, thus minimizing complications, decreasing the use of hospital
resources and health care costs, and improving overall patient care and
satisfaction. Fast-track protocols are safe for patients and offer improve-
ment in intestinal recovery and hospital discharge.

a primary anastomosis is to be considered. This article reviews the risk
factors, management, and outcomes associated with anastomotic
complications.
Outcomes Following Proctectomy

Rectal resection is the most common treatment of rectal cancer and
inflammatory bowel disease. The surgical techniques for removing and
reconstructing the rectum have evolved significantly over the past
50 years. Technological advances including retractors, stapling devices,
energy delivery systems, and minimally invasive approaches, as well as
the nerve-sparing total mesorectal excision, have revolutionized the surgi-
cal treatment. Surgical exposure and precise technique affect the ability to
preserve the pelvic autonomic nerves, directly influencing postoperative
urinary and sexual function. The complex interplay between all these fac-
tors demands attention because of the associated short-term and long-
term impact on patient quality of life.
Considerations and Complications in Patients Undergoing Ileal Pouch Anal
Anastomosis

Total proctocolectomy with ileal pouch anal anastomosis (IPAA) preserves
fecal continence as an alternative to permanent end ileostomy in select
patients with ulcerative colitis and familial adenomatous polyposis. The
procedure is technically demanding, and surgical complications may arise.
This article outlines both the early and late complications that can occur
after IPAA, as well as the workup and management of these potentially
morbid conditions.
Management and Complications of Stomas

Stomas are created for a wide range of indications such as temporary pro-
tection of a high-risk anastomosis, diversion of sepsis, or permanent relief
of obstructed defecation or incontinence. Yet this seemingly benign proce-
dure is associated with an overall complication rate of up to 70%. There-
fore, surgeons caring for patients with gastrointestinal diseases must be

proficient not only with stoma creation but also with managing postopera-
tive stoma-related complications. This article reviews the common compli-
cations associated with ostomy creation and strategies for their
management.
Surgical Management of Crohn s Disease 167

Although medical management can control symptoms in a recurring incur-
able disease, such as Crohn s disease, surgical management is reserved
for disease complications or those problems refractory to medical man-
agement. In this article, we cover general principles for the surgical man-
agement of Crohn s disease, ranging from skin tags, abscesses, fistulae,
and stenoses to small bowel and extraintestinal disease.
Rectal Prolapse 187

Optimal management of rectal prolapse requires multiple clinical consider-
ations with respect to treatment options, particularly for surgeons who
must counsel and give realistic expectations to rectal prolapse patients.
Rectal prolapse outcomes are good with respect to recurrence. Although
posterior rectopexy remains most popular in the United States, increas-
ingly surgeons perform ventral rectopexy to repair rectal prolapse. Func-
tional outcomes vary and are fair after rectal prolapse repair. Although
incarceration with rectal prolapse is rare, it is potentially life threatening
and requires immediate and effective measures to adequately address
in the acute setting.

this complex topic in patients with both benign and malignant disease.
Perspectives regarding a safe approach to patients who may require reo-

malady is limited. There are controversies in several areas of care. This
article addresses some of the gray areas of care for the patient with enter-
ocutaneous fistula. There is particular attention directed toward the phe-
nomenon of enteroatmospheric fistula, as well as prevention and
abdominal wall reconstruction, which is often required in these individuals.

Colorectal disease in pediatric patients includes a spectrum of diseases,
many of which have a significant impact on quality of life and warrant
long-term follow-up and treatment into adulthood. Although many dis-
eases, such as inflammatory bowel disease and colon cancer, are man-
aged similar to adults, other disease processes are more common to
pediatric patients and are the focus of this article.

Robotic approaches in all surgical realms have seen tremendous growth
over the previous few years. Taking advantage of 3-dimensional visualiza-
tion, improved articulation, and the opportunity for an enhanced ability to
suture/operate in the deep pelvis all provide theoretical and real advan-
tages in colorectal surgery. This article reviews the potential advantages
and disadvantages, current indications, future directions, and lessons
learned for robotic approaches in colorectal surgery.
