Minor oral surgical procedures make up a significant part of the daily prac¬
tice of dentistry. With the increased sophistication of medical technology
and medications there is increased likelihood of performing surgery on pa¬
tients who are being treated for conditions that require some type of anti¬
coagulant therapy. These patients are at an increased risk for
perioperative
bleeding or thrombotic complications if
anticoagulation
is discontinued or
the dosage is adjusted. Therefore, a fine balance needs to be obtained and
adequate preparation of these patients is the key to establishing this bal¬
ance. This article reviews suggested approaches to the management of
such patients.

Extraction of impacted teeth is one of the most common surgical proce¬
dures performed by oral and maxillofacial surgeons. Every surgical proce¬
dure results in some degree of postoperative bleeding and inflammation,
typically manifesting as pain and edema. Although the complex physiology
of the human body is beyond the scope of this article, the educated clini¬
cian should have an understanding of the time line associated with these
processes so as to determine whether a patient s complaint of postoper¬
ative bleeding, pain, or swelling represents a normal response to surgical
trauma or an aberrant reaction.

This article is devoted to risk-management strategies regarding oral surgi¬
cal procedures in the general dental office. Lawsuits are more likely to be
filed following poor outcomes related to oral surgical procedures rather
than after operative or prosthetic dental procedures. The article is not
meant to discourage practitioners from performing oral surgical proce¬
dures if they have the experience, training, and appropriate skill set to
complete the planned procedure. Rather, it advises clinicians as to the
steps one can take to limit the chances of litigation from occurring, and
avoid the emotionally and painful time-consuming process associated
with a malpractice lawsuit.

Conventional endodontic therapy is successful approximately
80-85%
of
the time. Many of these failures will occur after one year. The presence of
continued pain, drainage, mobility or an increasing size of a radiolucent
area are some of the indications to treat the case surgically. Since many
of these cases may have had final restorations placed by the dentist, the
salvage of these cases is of importance to the patient. Advances in

periapical
surgery have included the use of ultrasonic root end prepara¬
tion. With the use of these piezoelectric devices, a more controlled apical
preparation can be achieved. Additionally, isthmus areas between canals
can be appropriately prepared and sealed. The precision afforded with
these devices reduces the chances for a malpositioned fill and a more suc¬
cessful outcome.

dental procedures. It also gives an overview of the mechanism of action

local anesthetics with the relevant anatomy are explained. An overview
is given of the possible complications that can occur because of local

disorders (TMDs) and temporomandibular joint pain and dysfunction

clinical findings, and institute a therapeutic regimen that will provide

years the maxillary sinus augmentation graft has been
a mainstay of implant-directed maxillary reconstruction. The purpose of
this article is to review the fundamentals of maxillary sinus reconstruction
including anatomy and physiology of the sinus, indications for surgery,
preoperative
evaluation, surgical techniques, and management of com¬
plications. While there are some relative contraindications for the proce¬
dure, there are almost no absolute contraindications. With preparation,
education, and experience, the maxillary sinus augmentation/elevation
graft is a procedure that greatly benefits the patient, with a predictable
outcome.

Antibiotic prophylaxis to prevent infective endocarditis has been contro¬
versial through the years, with various changes made to recommenda¬
tions provided to treating physicians and dentists. The dentist must
always use his or her best judgment when applying any guideline. How¬
ever, it is important to remember that the guidelines may be cited in any
malpractice litigation as evidence of the standard of care. Early diagno¬
sis with prompt treatment with effective antimicrobial therapy is the best
way to lower the mortality and morbidity. When prescribing antibiotics,
the clinician must realize that the overprescription of antibiotics has
led to resistance to antibiotic regimens and the rise of antibiotic-resistant
bacteria.

niques to make simple and complex exodontia more predictable and effi¬

periotome that has been developed to aid in the atraumatic extraction of
teeth and another new device, the piezosurgery, increasingly used for out¬
patient oral surgery procedures are included. Physics forceps, a new type
of exodontia forceps, is also discussed in this article.

Mucogingival conditions are deviations from the normal anatomic relation¬
ship between the
gingival
margin and the mucogingival junction. Mucogin¬
gival surgery is plastic surgery designed to correct defects in the
gingiva
surrounding the teeth. Common mucogingival conditions are recession,
absence, or reduction of
keratinized
tissue, and probing depths extending
beyond the mucogingival junction. Surgical techniques used to augment
cosmetic mucogingival defects include the free
gingival
autografi,
the sub-
epithelial connective tissue graft, rotational flaps, lateral sliding flaps, coro-
nally repositloned flaps, and the use of acellular dermal matrix grafts.

materials and their tissue reactivity, and it reviews various suturing tech¬
niques used in contemporary dentoalveolar surgery. Familiarity with the
concepts presented in this article, and continuous practice of the surgical
skills presented, enhances surgical acumen and allows for improved heal¬
ing, increased postoperative comfort, and successful surgery.
