Thrombosis is a common complication of Cancer, occurring in up to 15% of
patients. This article reviews the diagnosis and management of the most
common cancer-related thrombotic problems; deep venous thrombosis,
pulmonary embolism, and catheter-related thrombosis. Rarer entities, such
as cerebral vein thrombosis and Budd-Chiari Syndrome, are also reviewed.

The epidemiology, presentation, and diagnostic evaluation of patients

Neurologie Symptoms commonly oeeur in oncology patients, and in some
cases they may be the presenting symptom of malignancy. Cancer-related
neurologic Syndromes are rarely pathognomonic and must be differenti-
ated from other benign or serious conditions. This article reviews common
neuro-oncologic Syndromes that may lead to urgent evaluation in the
emergency department, including cerebral edema, altered mental Status,
seizures, acute stroke, leptomeningeal metastases, and paraneoplastic
neurologic Syndromes.
Optimal Management of Malignant Epidural Spinal Cord Compression 537

Malignant epidural spinal cord compression (MESCC) is a common neuro-
logic complication of cancer. MESCC is a medical emergency that needs
rapid diagnosis and treatment to prevent undergo emergent evaluation in-
cluding magnetic resonance imaging of the entire spine. If MESCC is diag-
nosed, corticosteroids should be administered. Simultaneously, spine
surgery and oncology teams should be immediately consulted. If indi-
cated, patients should undergo maximal tumor resection and stabilization,

followed by postoperative radiotherapy. Emerging treatment options such
as stereotactic radiosurgery and vertebroplasty may be able to provide
some symptomatic relief for patients who are not surgical candidates.
Electrolyte Complications of Malignancy 553

A thorough working knowledge of the diagnosis and treatment of life-
threatening electrolyte abnormalities in cancer patients, especially hypo-
natremia, hypoglycemia, and hypercalcemia, is essential to the successful
practice of emergency medicine. Although most minor abnormalities have
no specific treatment, severe clinical manifestations of several notable
electrolytes occur with significant frequency in the setting of malignancy.
The treatment of life-threatening electrolyte abnormalities is reviewed
here. Promising future treatments directed at the underlying physiology
are also introduced.
Renal Complications in Oncologic Patients 567

Acute renal failure (ARF) can be one of the many complications associated
with malignancy and, unfortunately, often harbors a worse prognosis for
the afflicted patient. Insult to the kidneys can occur for a variety of reasons
in the oncologic patient. This article focuses on several of these etiologies,
such as tumor lysis Syndrome (TLS) and thrombotic microangiopathy
(TMA), which are unique threats faced by the oncologic patient.

Neutropenie enterocolitis, also known as typhlitis or ileocecal Syndrome,
is a rare, but important, complication of neutropenia associated with ma-
lignancy. It oecurs as a result of chemotherapeutic damage to the intes-
tinal mueosa in the context of an absolute neutropenia and can rapidly
progress to intestinal Perforation, multisystem organ failure, and sepsis.
Presenting signs and Symptoms may include fever, abdominal pain, nau-
sea, vomiting, and diarrhea. Rapid identification and timely, aggressive
medical and/or surgical Intervention are the comerstones of survival for
these patients.
Myeloproliferative Disorders and the Hyperviscosity Syndrome 585

Myeloproliferative disorders and the serum hyperviscosity Syndrome can
rapidly manifest with emergent presentations. Hyperviscosity oecurs
from pathologic elevations of either the cellular or acellular (protein) frac-
tions of the circulating blood. Classic hyperviscosity Syndrome presents
with the triad of bleeding diathesis, Visual disturbances, and focal neuro-
logic signs. Emergency medicine providers should be aware of these con-
ditions and be prepared to rapidly initiate supportive and eaiiy definitive
management, including plasma exchange and apharesis. Early consulta-
tion with a hematologist is essential to managing these complex patients.

Management of Acquired Bleeding Problems in Cancer Patients 603

Cancer patients can have acquired bleeding probiems for many reasons. In
this review, an approach to the evaluation and management of the bleeding
patient is discussed. Specific issues including coagulation defects, throm-
bocytopenia, platelet dysfunction, and bleeding complications of specific
hematological malignancies due to anticoagulation, are discussed.

Patients and families struggling with cancer fear pain more than any other
physical symptom. There are also significant barriers to optimal pain man-
agement in the emergency setting, including lack of knowledge, inexperi-
enced clinicians, myths about addiction, and fears of complications after
discharge. In this article, we review the assessment and management op-
tions for cancer-related pain based on the World Health Organization
(WHO) 3-step approach.
