The scalp provides a relatively limited amount of excess tissue that can be used in
reconstructing significant scalp defects, which arise most often from oncologic re¬
section or traumatic loss. Scalp reconstruction encompasses a broad spectrum of
flaps, grafts, and techniques that should be readily available to the facial plastic sur¬
geon treating this patient population. Meticulous attention to detail, particularly in the
planning and early postoperative periods, is associated with gratifying results in the
majority of patients. This article presents defect analysis and discussion of recon¬
struction options, with figures of successful reconstructive surgeries.

The structures of the nose are arguably the most complex within the face to recon¬
stitute when absent. Total nasal reconstruction has evolved to encompass ad¬
vanced surgical techniques in an effort to achieve increasingly satisfactory
cosmetic results while restoring nasal function that mimics the function of a patient s
natural nose. In this article, the history of total nasal defects and their reconstruction,
relevant nasal anatomy, etiologies of the defect, and the surgical approaches to re¬
constructing each of the three-layered structure of the nose (ie, nasal skin, cartilage/
bone, and lining mucosa) are explored.

crosurgeons. This article reviews the goals of lip reconstruction after major ablative
or traumatic lip losses and outlines the free flap options available to achieve these

The maxilla and midface have an important aesthetic and functional role. Their re¬
construction after surgical ablation is a complex problem. The historical develop¬
ment of reconstruction of the midface, classification systems of maxillary defects,
and descriptions of reconstructive options are presented.

Extensive skin, paranasal, and skull base malignancies often invade the orbit neces¬
sitating the removal of the orbital contents to achieve local tumor control. Defects of
this region often create a communication between the intracranial and extracranial
contents that involves a large volume of skin, soft tissue, and bone. Local and re¬
gional tissue alone is rarely adequate to repair these defects. In contrast, free tissue
transfer can provide vascularized skin, soft tissue, and bone that can reliably sepa¬
rate the extracranial and intracranial compartments and restore the bony and soft
tissue architecture of the orbitomaxillary complex. This article summarizes the ap¬
proach to orbitomaxillary reconstruction and discusses the use of free tissue transfer
in the rehabilitation of this complex region.

Maxillofacial trauma caused by severe ballistic or avulsion injuries to the face creates
complex composite defects. The essential difference between most blunt facial in¬
juries and ballistic or avulsive facial injuries is the severity of the soft and hard tissue
loss and damage. Application of modern principles of fracture management and
wound care, however, allows restoration of the facial framework until a healthy tissue
bed is achieved for definitive repair. Free tissue transfer has made it possible to bring
well-vascularized bone and soft tissue into a hostile wound environment with excel¬
lent survival of the flap tissues. This has allowed for simultaneous reconstruction of
the bony facial skeleton and the overlying soft tissue soon after injury.
Reconstruction of Periauricular and Temporal Bone Defects 253

Large periauricular and temporal bone defects most commonly follow resection of
advanced nonmelanoma skin cancers. Reconstruction aims to cover the cutaneous
defect and adjacent vital structures with the ability to heal in an irradiated field and
withstand further treatment. Preferred reconstructions are class I, cervicofacial rota¬
tion or radial forearm free flap; class II, anterolateral thigh; and class II, rectus abo-
minis free flap. Ancillary procedures, especially for associated facial paralysis, often
are required. Although free flap reconstruction provides rapid wound healing, local
and regional flaps are alternatives for patients unable to tolerate prolonged anesthe¬
sia and for use after recurrence or complications.

Parotidectomy is a widely performed procedure for various indications, including be¬
nign and malignant conditions. For malignant neoplasms of the parotid gland or met-
astatic disease, it may be performed in conjunction with cheek or temple skin
resection, facial nerve sacrifice, or composite resection of a portion of the mandible.
There are various options for reconstruction depending on the extent of resection.
This article discusses various reconstructive options following parotidectomy and
other ablative procedures that are often performed in conjunction. Reconstruction
of parotidectomy defects associated with lateral temporal bone resection is dis¬
cussed elsewhere in this issue.

Facial reconstruction poses a unique surgical challenge—restoring the aesthetic
form and function of the face. Established techniques for reconstruction include
skin grafts, local cutaneous tissue flaps, and free flap autografts. The anatomic com¬
plexity of the face renders it challenging, however, to obtain a successful cosmetic
and functional result. The success of recent hand, knee joint, and larynx allotrans-
plantation and advances in immunosuppressive regimens have pushed the techni¬
cal frontiers of composite tissue transfer to include partial facial transplantation.
This article reviews current techniques for reconstruction of facial defects, with
a focus on the microsurgical, immunologic, and ethical considerations of facial
allotransplantation.
Free Tissue Transfer in the Reconstruction of Massive Skin Cancer 279

Skin cancer arising in the head and neck is a common occurrence. Although the vast
majority of these cancers can be treated with simple excision and local reconstruc¬
tion there is a subset of patients who have massive tumors that require composite
tissue resection. These patients are best reconstructed with free tissue transfer.
Acceptable functional and cosmetic results can be expected. Long-term survival
is excellent in patients who have negative margins.
