ABSTRACT
There are various data indicating existence of close connection between the total
amount of alcohol consumption per capita and the level of different damages in
physical, mental and social spheres. This damage is considered to be called alcohol-
related problems. The term itself emphasizes the causal relationship between the
problem development and alcohol consumption. These may be serious problems
arising from single intake of large alcohol doses: acute alcohol poisoning, industrial
and transport injuries and homicides. As well, chronic problems may result from a
long-term alcohol abuse. Firstly alcohol dependence and alcoholic psychoses which
are the direct psychobiological consequence of alcohol abuse are to the point. Be¬
sides, chronic somatic disease are related to the problem too (cirrhosis of the liver,
pancreatitis, cancer of the upper digestive tract) alcohol playing the main part in
their etiology. Many nonalcoholic related diagnoses are often associated with alco¬
hol abuse: diseases of the gastrointestinal and respiratory systems, heart and vascu¬
lar diseases, nutritional deficiencies, tuberculosis, endocrine and immune systems
disorders.
The most significant health trend in Europe is the growing disparity in life ex¬
pectancy and mortality between the former socialist countries of Central and East¬
ern Europe and Western part of the region. In the CCEE life expectancy has de¬
clined, while in the most west European countries it increased. Several researchers
explain the deterioration of demographic situation in the republic of the former So¬
viet Union by the growth of alcohol-related problems. In recent decades, when many
countries of Western Europe were facing decrease in the level of alcohol consump¬
tion per capita, contrary tendency was observed in CCEE.
As experience shows, the level of alcohol-related problems is subject to control
by means of political measures. Quite a number of cases are known, when restricted
alcohol availability in various countries was accompanied by rapid reduction of
alcohol consumption, which leads to decrease of alcohol-related problems. On the
other hand, in cases when alcohol availability increased, both the level of alcohol
consumption and the level of alcohol-related problems grew correspondingly.
The national anti-alcohol campaign organized in the former USSR in the
1985-
88
was aimed at restriction of vodka and wines consumption. In Belarus rapid,

per cent reduction of the alcohol consumption level was accompanied by
8
per cent
decline of the total mortality level. The level of mortality due to accidents and in¬
jures decreased by
30
per cent. Mortality due to alcoholism, alcoholic psychosis and
poisoning by alcohol was decreased by
46
per cent.

general, the results of the
campaign can be interpreted controversially. Thus, alongside the positive results
mentioned, the campaign had some negative ones as well. Among them there are
growth of homemade alcoholic beverages consumption and increasing poisoning by
various toxic substances. The outcomes of the anti-alcohol campaign show that se¬
vere restrictive alcohol policy aimed at lowering the total level of alcohol consump¬
tion cannot be considered optimal. It should be pointed out that sharp decline in
alcohol consumption was achieved mainly due to the reduction of wine sales. De¬
creasing levels of wine production, both during the campaign and afterwards, added
to its unavailability. This, in its turn leads to alteration of the alcohol consumption
structure
i.e.
to prevalence of strong spirit in it.
1990s
were associated with growth
of alcohol consumption per capita and with rapid growth of different types of mor¬
tality in former Soviet Republic of the Eastern Europe.
The association between alcohol consumption and different type mortality is
well documented. Considerably less is known about the beverage-specific effect of
alcohol on mortality rate. The role of drinking pattern in the alcohol-mortality asso¬
ciation is very important issue today. Trends in different types of mortality rate from
1970 - 1999
in Belarus were analyzed in relation to trends in the level of different
types of alcoholic beverage consumption per capita applying time series, factor and
cluster analyses. The results of this study demonstrated a positive and statistically
significant effect of changes in strong spirits per capita consumption on mortality
rate. At the same time, the relationship between the total level of alcohol consump¬
tion and different types of mortality rates is far below statistical significance. The
analysis suggest that

increase in spirits consumption per capita would result in a

increase in total mortality rate,

increase in accidents and injuries mor¬
tality rate, in a
0,5%
increase in suicide rate, in a
1,1%
increase in homicide rate, in
a
1%
increase in fatal traffic accidents
rate, a
1,2%
increase in alcohol poisoning
mortality rate, in a
0,5%
increase in liver cirrhosis mortality rate, in a
2,4%
increase
in a alcoholism and alcoholic psychoses mortality rate, in a
0,45%
in a myocardial
infarction mortality rate, in a
0,4%
in a hypertension mortality rate, in a
0,45%
increase in a breast cancer mortality rate and in a
0,66%
in esophagus cancer mortal¬
ity rate. The results of this study support the idea that different types of mortality
rates tends to be more responsive to changes in strong spirits consumption per capita
than in total level of alcohol consumption, as well as the level of wine and beer
consumption per capita. The outcome of this study also suggest that alcohol related
mortality prevention programs schoud put more focus on addressing alcohol prob¬
lems. |
