At its core, epidemiology is concerned
with changes in health and disease.
The discipline requires counts and measures:
of births, health disorders, and deaths, and
in order to make sense of these counts it
requires a population base defined by place
and time. Epidemiology relies on closely
defined concepts of cause
—
experimental or
observational
—
of the physical or social envi¬
ronment, or in the laboratory. Epidemiologists
are guided by these concepts and have often
contributed to their development. Because the
disciplinary focus is on health and disease in
populations, epidemiology has always been an
integral driver of public health, the vehicle that
societies have evolved to combat and contain
the scourges of mass diseases.
In this book, the authors trace the evolu¬
tion of epidemiological ideas from earliest
times to the present. Beginning with the
early concepts of magic and the humors of
Hippocrates, it moves forward through the
dawn of observational methods, the system¬
atic counts of deaths initiated in iSth-century
London by John Craunt and William Petty,
the late i8th<entury Enlightenment and the
French Revolution, which established the
philosophical argument for health as a human
right, the national public health system begun
in
îçth-century
Britain, up to the develop¬
ment of eco-epidemiology, which attempts
to re-integrate the fragmented fields as they
currently exist. By examining the evolution of
epidemiology as it follows the evolution of
human societies, this book provides insight
into our shared intellectual history and shows
a way forward for future study.
Dr. Mervyn Susser began his career in
community and primary health care in Alexandra,
South Africa, and with his wife
Zena
Stein was
appointed to joint charge of the Alexandra Clinic
and University Health Centre. He later spent a
year as a chest physician, qualified as a member
of the Royal College of Physicians (Edinburgh),
was recruited to the Department of Social and
Preventive Medicine at Manchester University, and
served as medical officer for Mental Health for the
City of Salford. He served as professor and head
of Epidemiology, Columbia University Faculty of
Health Sciences, founded the Sergievsky Center
and was Sergievsky Professor of Epidemiology until
his retirement in
1990.
From
1990
through
1996,
Dr. Susser was editor of the American Journal of
Public Health. With
Zena
Stein in
1999,
he served
as the joint director of the newly founded Africa
Centre for Population and Reproductive Health
Research in Kwazulu/Natal, South Africa. In
2005,
he received the
Rema LaPouse
Award from the
American Public Health Association, as well as the
Lifetime Achievement Award from the National
Maternal Child Health Conference.
Dr.
Zena
Stein received her medical degree
in
1950,
from the University of Witwatersrand in
Johannesburg, South Africa. She began her career
in community health and primary health care in
Alexandra, South Africa, then followed with nearly
a decade at Manchester University, working on
epidemiological and family and cultural studies
of mental retardation, child development, and
psychiatric disorders. Since coming to New York
in the
mid-1960s,
she has occupied her present
academic and research positions at Columbia
University. She is widely recognized as a pioneer
in the movement to provide women with methods
of protection that they might use in heterosexual
relationships, including microbicides, physical
barriers (especially the female condom), and
behavioral strategies for women that confer
empowerment in sexual encounters. She is the
recipient of many prestigious awards, dedications,
and symposiums in her honor.
