The incidence of skin cancer has been increasing throughout the United States and much
of the world. Although the mortality rate of nonmelanoma skin cancer is low, that type
of skin cancer accounts for considerable morbidity, including cosmetic and functional
impairment. Melanoma, although less common, is a life threatening malignancy if not
detected and treated early. Skin cancer also significantly contributes to the rising costs of
health care in the United States. Prevention, early diagnosis, and treatment are critical in
helping to reduce the incidence, morbidity, and mortality associated with skin cancer.
This article provides an introduction to skin cancer, including the changing incidence,
clinical presentation, and summary of treatment and prognosis.

It is increasingly important for clinicians to be proficient at performing skin biopsies
because of the prevalence of skin cancers and other skin diseases in the population. With
the appropriate knowledge and training, the three major types of biopsies—shave biopsy,
punch biopsy, and excisional/incisional biopsy—can be performed. The choice of biopsy
type depends on the suspected disease pathology and lesion location.

Whenever possible, surgical excision of skin cancers should be the gold standard of treat¬
ment. There are many considerations when choosing one treatment modality over another.
These include the lesion s location, the surgeon s experience and comfort level, the patient s
health status and their potential compliance, access to available technology, and economic
considerations for the patient and the provider. Regardless of the type of therapy, all forms
of treatment share the following goals: complete disease cure, preservation of normal tissue,
preservation of function, and optimal cosmesis.

Mohs micrographic surgery is a surgical technique developed for removal of mucocu¬
taneous malignancies. The surgeon integrates the responsibilities of surgery, specimen
orientation and preparation, and microscopic interpretation to determine tumor free
margins. It has the lowest recurrence rate of all surgical techniques while conserving the
most tumor free skin. In selected cases it is the preferred method for treating carcinomas. i

Dermatologic secondary procedures involve careful preoperative planning and patient :
preparation, skillful execution of the appropriate procedure, and thorough postoperative !
wound care. Many modalities of treatment are used, including skin preparation through
elimination of inflammatory conditions, resurfacing of skin, and improvement of patient
health. Proper selection of incisional design, local or regional flaps, and grafting tech¬
niques is key to successful revisional surgery. Care of the revised lesion or wound
through medications, dressings and resurfacing techniques will optimize the end result.

Surgical management of cutaneous malignant melanoma of the head and neck requires
careful assessment of the primary lesion, use of appropriate biopsy and excision technique,
and assessment of the regional lymphatics in all but the thinnest of lesions. Sustained
disease free survival can be achieved with appropriate initial intervention and serial
follow up. The major significant differences between current and past management are
the advent and acceptance (or not) of sentinel node biopsy technique for intermediate
thickness lesions without evidence of lymphadenopathy and an improved, validated,
staging system. Early diagnosis and management are crucial to improving patient sur¬
vival. They are best achieved through education of patients and clinicians.

Surgical excision is the gold standard for management of cutaneous basal cell carcinoma
or squamous cell carcinoma. Surgical management of nonmelanotic facial skin cancer
requires preoperative planning and an in depth understanding of reconstructive tech¬
niques, including primary closure, skin grafting, and local tissue flaps. The decision
regarding the method of treatment of nonmelanotic skin cancer is highly individualized
and depends on patient age, cancer size, histologic subtype, and site. No single therapy
or technique can be used in every situation. The goals of treatment include complete
removal of the lesion with preservation of normal tissue, function, and cosmesis. In this
article, we discuss the principles and techniques of surgical excision and reconstruction
of site specific facial skin cancers.
