Forehead Reconstruction 1

The article focuses on forehead reconstruction of surgical wounds following skin cancer
resection. Reconstruction options are presented for three forehead subunits. Preserving
motor and, when possible, sensory nerve function, maintaining eyebrow position, and
camouflaging incisions are priorities for successful reconstruction. Most forehead defects
that cannot be closed primarily are usually reconstructed with laterally based advance¬
ment flaps.
Scalp Reconstruction 13

The article focuses on scalp reconstruction of surgical wounds following skin cancer
resection. In contrast to other cosmetic units, scalp wounds are closed under significant
tension, because the galea restricts overlying tissue movement. For defects that cannot be
closed primarily, rotation flaps developed within the subgaleal space are often the best
reconstruction option.

Reconstruction strategies for the ear should address the major aesthetic goals of main¬
taining the helical curvature and the symmetric frontal profile. When the auditory canal
is involved, maintaining its patency is a major functional goal. The anatomic location of
the defect, by virtue of its topographic features and adjacent donor pools, will dictate the
appropriate reconstructive options. Familiarity with these anatomically specific options
will enable the surgeon to more simply and effectively reconstruct the ear.

Defects of the lip and perioral area present a significant challenge for reconstructive sur¬
geons. It is important to obtain results that are a success from the functional and aesthetic
standpoint. In planning the reconstruction, the surgeon should consider the age and
general state of health of the patient and dental status. The surgeon should be well
acquainted with the essential principles in planning reconstructive procedures. By
becoming familiar with advantages and shortcomings of different cheiloplasty proce¬
dures, the surgeon will be able to implement the functionally and cosmetically appropri¬
ate technique in each specific case.

units and functional considerations. This article discusses single and two staged melo¬
labial flap repairs for nasal reconstruction following removal of skin cancers.
The Dorsal Nasal Flap 73

As dermatologists and plastic surgeons have focused more effort on nasal repair, the dorsal
nasal flap has been revitalized as an elegant repair for distal nasal defects. Small modifica¬
tions of the flap provide for flexibility in execution. The viability of the dorsal nasal flap is
unparalleled. When properly designed and executed, the dorsal nasal flap can reproducibly
succeed in the repair of many challenging distal nasal wounds.
Interpolation Flaps 87

Interpolation flaps provide an excellent method for reconstruction of large or deep defects
where adjacent local tissue cannot supply sufficient donor tissue for repair. These flaps use
tissue imported from nonadjacent sites with an inherent blood supply (vascular pedicle) to
support the flap while attached to the recipient defect until neovascularization has been
ensured between the flap and recipient bed. Hence, they can supply the thickness or bulk
needed for large or deep defects and can survive on exposed bone or cartilage. The main
disadvantage of these flaps is that they require two stages to complete because the delayed
second stage releases the pedicle after inosculation and neovascularization render the tis¬
sue independent of the pedicle. This article describes three major interpolation flaps use¬
ful in repair of the nose and ear. The forehead, melolabial, and postauricular interpolation
flaps are excellent methods that will add to the dermatologic surgeon s reconstructive
armamentarium for the repair of large or deep defects.
Island Pedicle Flap 113

The island pedicle flap uses an island of skin that is detached from its epidermal and der¬
mal attachments while retaining its vascular supply from an underlying pedicle to repair
a cutaneous defect. Proper design of this flap requires familiarity with the anatomic and
vascular basis of the flap and with technical aspects of flap mobility. Transposition, tun¬
neling, interpolation, myocutaneous, and transcartilage variants of the island pedicle
flap are discussed. This flap is most useful for defects on the upper cutaneous lip and
eyebrow, but its use on the nose, ears, and periorbital area are indispensable for the
repair of select defects.
Composite and Free Cartilage Grafting 129

The nasal ala and helical rim of the ear are common sites for the development of skin
neoplasms. Removal of skin cancers from these aesthetically delicate areas may leave
defects that free skin grafts alone may not adequately repair. The contractural forces of
scarring within and around the skin graft during healing may produce elevation or
notching of the rim and may lead to functional compromise. The use of a composite graft
or the inclusion of a free cartilage graft in conjunction with a skin graft or flap repair can
provide structural support, which may help to prevent undesirable outcomes. This arti¬
cle focuses on the use of composite or free cartilage grafts harvested from the ear to
repair nasal alar or helical rim defects within a single surgical session. Preoperative con¬
siderations, surgical technique, and postsurgical complications are discussed.

Imiquimod for the Treatment of Skin Cancer 151

Imiquimod is a topically applied immunomodulator. Although initially approved for
genital and perianal warts, dermatologists have prescribed this compound for many
neoplastic and nonneoplastic skin conditions. The US Food and Drug Administration
recently approved imiquimod for the treatment of actinic keratoses. This article reviews
the literature on imiquimod in the treatment of cutaneous malignancies.
