Intraosseous Ameloblastoma 309

Ameloblastoma is the most common epithelial derived odontogenic neoplasm. Its inci¬
dence, combined with its clinical behavior, makes ameloblastoma the most significant
odontogenic neoplasm of concern to oral and maxillofacial surgeons. The demographic
data, etiologic factors, and clinical and radiographic features of the neoplasm are
reviewed. The core histopathologic attributes common to most ameloblastomas and spe¬
cific features of the various histopathologic subtypes are documented. The two growth
patterns of the neoplasm as they impact treatment decisions are discussed. Areas of con¬
troversy in determining the growth pattern and formulating a surgical treatment plan
are presented.
Calcifying Epithelial Odontogenic Tumor 323

The calcifying epithelial odontogenic tumor (CEOT) was first introduced into the scientific
literature almost 50 years ago by the late Dr. Jens J. Pindborg. In deference to this interna¬
tionally renowned oral and maxillofacial pathologist, CEOTs are now commonly referred to
as Pindborg tumors, which are classified as uncommon, benign, odontogenic neoplasms
that are exclusively epithelial in origin. There are differences of opinion within the oral and
maxillofacial pathology community as to the exact degree of differentiation to the odonto¬
genic epithelium that forms the basis for tumor pathogenesis. At the time of this writing,
a confirmed etiology remains elusive.
Adenomatoid Odontogenic Tumor 333

The adenomatoid odontogenic tumor (AOT) is one of the most unique and thoroughly
profiled odontogenic tumors; however, oral and maxillofacial surgeons must be aware of
its less typical variants to provide optimal patient care. A rarely encountered potential
diagnostic pitfall at the light microscopic level is delineated for the first time. The associa¬
tion of AOT with other established and recently proposed odontogenic tumors is reviewed
and analyzed and reportedly recurrent AOTs are evaluated case by case. Because of its
consistently benign behavior, in selected cases it may be possible to preserve the tooth or
teeth associated with the tumor provided that on going follow up is assured.

Squamous Odontogenic Tumor 355

Squamous odontogenic tumor is a rare, benign epithelial odontogenic tumor. Only a few
dozen cases have been documented in the literature. Squamous odontogenic tumor has
occurred over a wide age range, and the reported incidence peaks in the third decade of
life. There is a slight male preponderance, and the mandible commonly is involved.
Although squamous odontogenic tumor exhibits a unique microscopic picture, it may be
confused with other conditions, including ameloblastoma and squamous cell carcinoma.
This article references only studies in which the supporting photomicrographs demon¬
strate typical squamous odontogenic tumor.

The histogenesis, nomenclature, diagnostic criteria, and biologic behavior of odontogenic
mesenchymal neoplasms have remained controversial over the years. The recent literature
more clearly has defined the clinical, radiographic, and histologic parameters of odonto¬
genic myxoma, central odontogenic fibroma, and central granular cell odontogenic tumor.
This thorough review provides an update for oral and maxillofacial surgeons on these
issues and the characteristic and unique clinical and histopathologic features of these neo¬
plasms. This discussion includes their histopathologic similarities with the normal com¬
ponents of odontogenesis and the microscopic misinterpretations that can result.
Ameloblastic Fibroma, Ameloblastic Fibro Odontoma, and Odontoma 375

This article addresses the histogenesis, clinical and radiographic characteristics, histo
pathology, and treatments of mixed odontogenic tumors (ameloblastic fibroma, ameloblastic
fibro odontoma, odontomas, and their variants). These lesions probably arise as either
hamartomas or true tumors. Odontomas are regarded best as hamartomas (complex)
or malformations (compound). Some treatment innovations have been developed.
Ameloblastic fibroma and fibro odontomas may represent early stages in the development
of odontomas. These painless, slow growing, expansile neoplasms arise primarily in
children. They appear radiolucent to radiopaque and are treated by procedures ranging
from conservative enucleation to modified resection. Rare malignant transformations have
been known to occur.

In the clinical setting, most peripheral odontogenic tumors resemble benign reactive lesions.
All benign appearing hyperplastic lesions of the gingiva should be examined histopatho
logically to ascertain their true diagnosis, to reassure the patient, and to provide the practi¬
tioner with information about the recurrence potential and the type of follow up treatment
that is necessary.
Odontogenic Malignancies 409

Odontogenic malignancies are rare neoplasms. They are derived from tooth forming
epithelium; result from malignant transformation of a pre existing benign odontogenic
tumor, typically an ameloblastoma or ameloblastic fibroma; or arise as de novo malig¬
nancies that resemble an ameloblastoma or ameloblastic fibroma histologically. Most are
carcinomas which have potential for regional or distant metastasis. They can result in the
patient s death, often from uncontrolled local growth. The best chance for cure is assured
surgical excision, often with postoperative radiation therapy.
