Posterior glenohumeral subluxation is more common than once believed. Defects of the
posterior capsular restraint can occur adjacent to the glenoid, midsubstance, or laterally
as an avulsion from the humeral head (RHAGL). Repair of the posterior capsular
restraints, reduction of the rotator capsular interval, and repair of additional lesions
superiorly and anteriorly can return most patients to physical activity.

The advent of arthroscopy and arthroscopic repair techniques has opened new frontiers
in the diagnosis and repair of torn rotator cuff tendons. Until recently arthroscopic rotator
cuff repairs have been limited to the supraspinatiis and infraspinatus tendons. Now
arthroscopic subscapularis repair has advanced to the forefront. Despite being the largest
rotator cuff tendon of the shoulder, the function and clinical relevance of subscapularis
pathology has been largely ignored in the literature. Most recent studies have focused on
open subscapularis repairs, failing to describe the technique of arthroscopic repair and
the subsequent outcome of arthroscopic repairs. In this article, the surgical rationale,
technique, and preliminary results of arthroscopic subscapularis repair are reviewed.
Arthroscopic Biceps Tenodesis 499

Disorders of the proximal biceps are common. Treatment with biceps tenodesis generally
is recommended for younger patients. All arthroscopic biceps tenodesis techniques can
be performed successfully with the use of interference screw fixation that optimizes
fixation strength while easing the technical demands and minimizing neurovascuiar
injury risk.

Arthroscopy has emerged as the gold standard for the evaluation of the rotator cuff, the
subacromial space, and the glenohumeral joint. The past decade has seen advances in
arthroscopic skills such that many shoulder surgeons repair all rotator cuff tears with
arthroscopic techniques, and many orthopaedic surgeons now treat simple rotator cuff
tears with minimally invasive procedures. The gold standard for the treatment of rotator
cuff pathology will likely become arthroscopic repair. Early reports of all arthroscopic
repairs are encouraging. This article describes the many uses of the arthroscope in the
diagnosis and classification of rotator cuff pathology and discusses the surgical remedies
for these problems that are afforded by the skillful use of arthroscopy in the shoulder.

With the advent of shoulder magnetic resonance imaging (MRI), ganglion cysts adjacent
to the superior labrum have been increasingly recognized. As a result, the potential of
these cysts to cause suprascapular nerve compression at the spinoglenoid notch has
gained more attention. Many investigators have written on proposed etiologies and
recommended treatments to decompress the nerve and eradicate the cysts. More recently
the association between superior labral pathology (SLAP) and ganglion cysts has led to
potential arthroscopic techniques to decompress the cyst and repair the labrum. This
article reviews the pathogenesis, anatomy, and diagnosis of ganglion cysts, and describes
an arthroscopic technique for cyst decompression and fixation of an associated SLAP tear.
Arthroscopic Repair of Anterior Glenohumeral Instability and Rotator

Traumatic anterior instability is the most common shoulder instability pattern seen in
orthopaedic practices. Historically, open procedures have produced excellent results and
are the most commonly accepted surgical treatment for this problem. Recently there has
been a trend toward arthroscopic management of this condition. As our understanding
of the pathology of instability improves and corresponding advancements are made in
surgical techniques and instrumentation, the results from arthroscopic Bankart recon¬
structions are fast approaching those seen with open procedures. Arthroscopic repair of
such lesions are now well described and have become an integral part of instability sur¬
gery. As more surgeons become familiar with these techniques, the authors believe that
arthroscopic Bankart reconstructions with rotator interval repair will be considered the
preferred method in the future.

The concept of internal impingement has become an increasingly mainstream explana¬
tion for the spectrum of injury seen in the glenohumeral joint of the overhand athlete.
Disruption of the posterior and superior labrum or intra articular tearing of the superior
and posterosuperior fibers of the rotator cuff are the most common findings. Treatment
has advanced from isolated debridement of these tears to selective repair of partial thick¬
ness rotator cuff tears and repair of labral detachments. Only time will tell whether the
refined techniques of arthroscopic management will make a difference in the long term
outcome of treatment of this selective patient population.

Patients with multidirectional glenohumeral instability without a Bankart lesion represent
a challenging group. Arthroscopic pancapsular plication affords the surgeon flexibility to
correct the degree and direction of laxity in the patient with multidirectional instability
(MDI). This technique can be combined with suture anchors if needed to correct capsu
lolabral defects in patients with more advanced lesions. The mainstay of treatment for
MDI remains conservative. In resistant cases, however, arthroscopic management can be
performed safely and efficaciously, now with documented extended success in an active
patient population.

When nonoperative measures fail in the treatment of glenohumeral arthritis,
arthroscopy has emerged as a viable therapeutic option. It is valuable as a diagnostic
and therapeutic intervention and has a low morbidity and low complication rate.
Although the role of arthroscopy in the treatment of glenohumeral arthritis is evolving, in
certain situations and patients it is a viable option and another tool in the armamentarium
of the orthopedic surgeon

Calcific tendinitis of the shoulder is a process involving calcium deposition commonly in
the rotator cuff tendons. It is a cell mediated process that is often chronic in nature, but
it is usually self limiting with regard to its acute pain states. Conservative management
is still the treatment of choice and is successful in up to 90% of patients. When conservative
measures fail, a needling technique or surgical removal may be indicated; the trend is
toward arthroscopic management.
