Considering the wide variety of options for the reconstruction of posttraumatic and
postablative intraoral defects, the palatal flap holds many advantages over other forms
of reconstruction. It has been used successfully with minimal morbidity for recon¬
struction of defects within the reach of the flap. The cases presented herein demonstrate
the versatility of this flap, which should be part of the armamentarium of the recon¬
structive surgeon.
Tongue Flaps in Maxillofacial Surgery 475

The tongue provides well vascularized tissue that can be used predictably in maxillo¬
facial reconstruction. Pertinent surgical anatomy and the different designs and clinical

The Nasolabial Flap 487

The nasolabial flap is a straightforward and time tested reconstructive option suitable
for a variety of facial and oral defects. The superiorly and inferiorly based variants
are well described in the literature and offer a rapid and reliable alternative to time
consuming microvascular free flaps and less esthetic skin grafts. Despite newer and
more complex alternatives, the nasolabial flap maintains its prominent position in the
reconstructive armamentarium of the oral and maxillofacial surgeon.

Lower lip reconstruction is ideally achieved with innervated flaps that supply mucosa,
functional muscle, and skin. These flaps should be capable of being used when vermilion
ectomy is necessary. The size and shape of the defect determine the optimal flap. This
article illustrates the flaps used by the authors for particular reconstructive problems.

The continued use of the temporalis muscle flap in maxillofacial surgery for over a cen¬
tury serves as a testament to its versatility and reliability. The technical ease of its pro¬
curement and the amount and quality of tissue available make this flap a valuable tool
in the armamentarium of the reconstructive surgeon.
The Temporoparietal Galea Flap 537

The temporoparietal galea flap has been rediscovered as a useful tissue transfer tech¬
nique. It is the only single layered fasdal flap that can be transposed into the craniofacial
and head and neck region on its vascular pedicle. In the 1990s, it was used extensively in
the surgical reconstruction of a wide variety of defects in the craniomaxillofacial area,
ranging from scalp and auricle defects to nasal and maxillo orbital repair to all types of
intraoral and even mandibular and pharyngeal reconstructions.

The cervicofacial flap, with or without a pectoral extension, is an excellent source of pli¬
able skin with perfect color match and a similar texture to the surrounding tissues for
reconstructing large skin defects of the face.
The Plastysma Myocutaneous Flap 559

Reconstructing extirpative defects of the oral cavity or face frequently presents a sig¬
nificant challenge to the surgeon. This article discusses two versions of the platysma
myocutaneous flap that have proved to be reliable and versatile methods for recon¬
structing defects of the oral cavity, lower face, and ear. The advantages of these regional
axial pattern flaps include good color match, appropriate thickness, minimal donor site
morbidity, easy harvesting, and an adjacent donor site in the same operative field.
A thorough understanding of the pertinent anatomy, particularly with respect to the
location where the vascular supply enters the flap pedicle, is critical to the success of
this flap.
Pectoralis Major Myocutaneous Flap 565

The pectoralis major myocutaneous flap is a versatile and predictable transfer of soft
tissue for the reconstruction of avulsive and ablative defects of the oral and maxillofacial
region. This article reviews preoperative considerations, surgical anatomy, technique,
and possible complications related to the development of this flap.

The unique attributes of the radial forearm flap for head and neck reconstruction make
it a primary choice from the reconstructive ladder. As more surgeons become familiar

and comfortable with microvascular techniques and recognize the ease of its harvest, the
popularity of this flap will increase. The radial forearm free flap should not be used for
every head and neck defect. Flap choice should be dictated by the needs of the patient
and those of the site to be reconstructed.
