The Systemic Inflammatory Response to
Cardiopulmonary Bypass 453

Cardiac surgery necessitating cardiopulmonary bypass results in a
systemic inflammatory response characterized by activation of the
contact, coagulation, fibrinolytic, and cytokine cascades. Factors
contributing to the initiation of the systemic inflammatory
response include: (1) contact of the cellular and humoral blood
components with the synthetic surfaces of the extracorporeal cir¬
cuit, (2) leukocyte and endothelial activation following reperfusion
of ischemic tissues, (3) endotoxin release from gut flora, and (4)
operative trauma. The pathophysiology and therapeutic manage¬
ment of the systemic inflammatory response to CPB are reviewed.
Perioperative Myocardial Ischemia Reperfusion Injury 465

The restoration of blood flow following sustained myocardial
ischemia results in a phenomenon known as myocardial ischemia
reperfusion (I R) injury, in which tissue injury is augmented in
excess of that produced by ischemia alone. Perioperative myocar¬
dial I R injury contributes to adverse cardiovascular outcomes fol¬
lowing cardiac surgery. The pathogenesis of I R injury is complex,

involving the activation, coordination, and amplification of several
systemic and local pro inflammatory pathways. Treatment and
prevention of perioperative myocardial I R injury involves the
use of preoperative risk stratification, novel surgical techniques,
and pharmacologic agents aimed at reducing the generation and
pathophysiologic consequences of pro inflammatory mediators.

Monitoring Anticoagulation and Hemostasis in
Cardiac Surgery 511

Cardiovascular patients have many acquired and inherited hemo¬
stasis abnormalities. When these patients undergo cardiopulmonary
bypass for cardiac surgery, careful management of anticoagulation
for surgery must be coupled with the precise measurement of coag¬
ulation and platelet function after surgery, in order to ensure the
return to normal hemostasis. This complex management system
could not be performed without specific monitoring devices. This
article details these physiologic abnormalities and the monitors used
to measure them.
Current Status of Antifibrinolytics in Cardiopulmonary
Bypass and Elective Deep Hypothermic Circulatory Arrest 527

Cardiopulmonary bypass (CPB) results in many physiologic
derangements, including activation of the hemostatic and fibri
nolytic pathways. Deep hypothermic circulatory arrest (DHCA)
adds a further insult to the coagulation systems because it involves
more extreme hypothermia and organ ischemia related to blood
stasis. The abnormalities induced by CPB disrupt the checks and
balances in the hemostatic and fibrinolytic systems, resulting in a
pathologic state that leads to excessive bleeding and other periop¬
erative complications. Prophylactic antifibrinolytic therapy can
attenuate the response to this insult by restoring the delicate bal¬
ance within these systems, potentially reducing the complication
rate and improving patient outcomes.

Clinical Potential of Blood Substitutes or Oxygen
Therapeutics During Cardiac Surgery 553

The search for a blood substitute began more than 70 years ago,
when the United States War Department sought a blood replace¬
ment solution that could be stored at room temperature and
administered on the battlefield without the need for crossmatching.
With time, the focus shifted from the search for a blood substitute
to the development of oxygen therapeutics. Currently, there are
three major types of oxygen therapeutic agents: hemoglobin
based solutions, perfluorochemical solutions, and allosteric modi¬
fiers of hemoglobin (drugs that enhance the amount of oxygen
released to tissues). This article reviews the basic characteristics of
each solution, with an emphasis on the mechanism of oxygen
transport, and reviews the available data from clinical trials.
New Treatments for Perioperative Cardiac Arrhythmias 569

In patients undergoing major surgery with general anesthesia, car¬
diac arrhythmias are a significant cause of morbidity and mortality
during the perioperative period. Whereas the literature about the
use of antiarrhythmic agents in postoperative and nonsurgical
intensive care settings is expanding, few studies have been pub¬
lished about the use of these agents in the operating room. In par¬
ticular, drug selection and therapeutic strategies for treating
arrhythmias during pediatric surgery tend to be based on empiric
methods. Nonetheless, as our understanding of the relevant molec¬
ular targets for manipulating cardiac excitability increases, the
range of options for treating arrhythmias during surgery expands.
In this article, these molecular targets are used as a scaffold for dis¬
cussing the management of supraventricular and ventricular
arrhythmias in adults and children during operations involving
general anesthesia.
What Does Transesophageal Echocardiography Add to
Valvular Heart Surgery? 587

Intraoperative transesophageal echocardiography (TEE) has become
an essential diagnostic and monitoring tool for patients undergoing
valvular heart surgery. The unique esophageal window and real¬
time imaging it provides offer a wealth of anatomic and physiologic
information. TEE is a pre procedure guide and a postprocedure
safety check for valvular heart surgery. Intraoperative TEE serves as
a diagnostic and management tool in the operating room, allowing
immediate visualization of surgical results and detection of compli¬
cations related to the surgical technique. Standardized approaches

to all types of valvular pathology enable the surgeon and echocar
diographer to optimize each procedure to achieve the ultimate goal
of improving patient outcomes.
Off Pump Coronary Surgery: How Do the Anesthetic
Considerations Differ? 613

In recent years, there has been much interest in performing
coronary artery bypass graft (CABG) surgery without the aid of
cardiopulmonary bypass (CPB). Initial efforts focused on mini¬
mally invasive direct coronary artery bypass, wherein the left
anterior descending artery is bypassed with an in situ left internal
mammary artery graft through a small left anterior thoracotomy.
A more widely adopted approach however, is off pump CABG
(OPCAB), in which CABG surgery is performed on one or more
vessels through the usual median sternotomy approach without the
aid of CPB. This article reviews the differences in the anesthetic con¬
siderations of OPCAB compared to conventional CABG using CPB.

article provides insight into various anesthetic concerns regarding
patient management in this remote setting.

Because of continued improvement in the outcomes of reparative
or palliative surgery for congenital heart disease (CHD), the num¬
ber of adults with CHD now equals the number of children with

such disease. This article reviews the pathophysiologic complica¬
tions often encountered in adults with CHD, discusses the most
common congenital heart lesions seen in these patients, and pro¬
vides specific recommendations for their anesthetic care.
Gender and Cardiac Surgery 675

The increased operative mortality and morbidity of women com¬
pared with men undergoing coronary artery bypass grafting sur¬
gery results from multiple differences in presentation, preoperative
risk profile, and surgical factors. Investigators have found con¬
sistently that women present with a different preoperative risk pro¬
file than do men. As further knowledge accumulates, it is hoped
that gender specific risk factors can be mitigated and protective
factors exploited, thereby improving the outcomes for all cardiac
surgery patients.
