The circumareolar mastopexy with augmentation is an excellent procedure for the right
patient. Correct preoperative assessment of the patient s skin, as well as their desires and
expectations, are as critical as the surgeon s experience and acquaintance with his own
circumareolar technique. If all these factors can be brought together, however, successful
operations can be performed, happy patients will ensue, and less scars will be placed.

The increasing demand for reduced scars has led to the development of numerous
minimal incision procedures, which constitute the most recent and exciting advances in
breast surgery. Numerous different periareolar techniques are described in an attempt to
eliminate scars on the breast surface by restricting them to the interface between the
areola and the skin. These conclusions show a clear evolution of the technique as well as
an improvement in the obtained aesthetic results. The author has developed new tech¬
niques for periareolar mammaplasty based on repositioning the breast parenchyma and
maintaining this new architecture with a stronger and more durable supporting system.
The development of new materials, technical refinements in the technique, and increased
experience will undoubtedly lead to even better results in the future.

During the past few decades, periareolar and vertical scar mammaplasties were intro¬
duced as a novel way of approaching breast reductions. Many surgeons worldwide are
still reluctant to apply them as a standard. The areolar vertical approach (AVA) mam¬
maplasty, derived from the Vertical Mammaplasty described by Lejour, offers us the
opportunity to achieve good cosmetic results in breast reduction even in larger hyper¬
trophies and makes it available to all patients. Long term results, symmetries, aesthetic
scars, and patient satisfaction encourage me to continue with this procedure.

The goal in breast reduction surgery is to reduce volume, but at the same time to main¬
tain circulation, sensation, and breast feeding potential. A dermoglandular or central
pedicle is the most likely to achieve all three. Any pedicle can be used to achieve a good
breast reduction. Different situations will determine which is preferable. The author per¬
formed a standard inferior pedicle Wise pattern technique for the first 11 years of her
practice and has now used variations of the vertical technique for the last 8 years. She
has personally found that the medial pedicle gives the best breast reduction results in her
hands. On the other hand, a lateral pedicle is used for mastopexies so that the inferior
and lateral tissue can be rotated up under the areola. When faced with a re reduction of
a previous inferior pedicle, the vertical technique can still be used and an adaptation of
the inferior pedicle can be very acceptable. The superior pedicle is reserved mainly for
very small reductions or mastopexies, but the author still finds the medial pedicle allows
better lateral resection even in small reductions, and the lateral pedicle with recruitment
of tissue allows for some auto augmentation in mastopexies.

The main indication of the circumvertical technique is the removal of 400 1000 mg of
breast tissue. It is also an intermediate method and an alternative between the periareo
lar and the vertical techniques. The advantages of the circumvertical technique is that it
is simple and fast to accomplish, only the inferior half of the gland is operated, there is
no transection of the lactiferous ducts, at the end of the surgery there is an harmonious
redistribution of the pleats, the vertical scar never crosses the submammary fold, and at
the end of the surgery, an acceptable result is almost always observed. The posterior skin
retraction will improve this initial result even more. In brief, this technique is ideal for
young women because it neither distorts the remnant anatomy nor alters the future
lactation, being a good alternative to moderate and large hypertrophies.

Recent developments in breast reduction technique have focused on reducing the
amount of cutaneous scar. Most of these techniques utilize a superior or superior
medial pedicle to maintain the blood supply to the nipple and areola. For reductions of
more than 200 to 300 grams, management of the excessive skin envelope results in an
early distorted appearance to the breast, which is then expected to settle into the proper
shape over time. Issues related to less than desirable scar appearance and persistent
shape distortion in the vertical scar was also a sharp drawback. It was for these reasons
that the short scar periareolar inferior pedicle reduction or SPAIR mammaplasty was

developed. This procedure provides a versatile alternative to existing short scar tech¬
niques for both mastopexy and reduction that may be more easily incorporated into any
given surgeons practice due to the use of the more familiar inferior pedicle as well as the
direct control over shape which is afforded.

Procedures that minimize the resultant scars while achieving comparable results are
preferred by patients and greatly enhance their overall satisfaction. Suction mamma¬
plasty—and indeed all short scar procedures that are performed on the breast, face, and
body—rely to variable degrees on harnessing the under appreciated capability of the .skin
to shrink or retract. Patients with breast hypertrophy represent a wide spectrum of defor¬
mities based on NAC size, shape, and location, skin quality and tone, and breast volume.
No one breast reduction technique is ideal for reducing all breasts. Suction mammaplasty
represents one of a number of methods of reduction mammaplasty that can be selected
according to variations in anatomy, patient desires, and surgeon preference.
