Cerebral Blood Flow and Vascular Physiology 247

The cerebral circulation is tightly regulated to meet the brain s
metabolic demands. Although the mechanism is not fully under¬
stood, the major physiologic influences on cerebral blood flow
have been well documented. In this chapter the basic vascular
anatomy, and physiologic control of the cerebral circulation are
reviewed. Clinical implications are emphasized.
Anesthetics and the Brain 265
Tod B. Sloan
A variety of effects of anesthetics on cerebral physiology have
implications for their choice and use in patients undergoing neuro
surgical procedures. This chapter reviews the studies that suggest
an important target for anesthetic effect is synaptic ion channel
receptors. The known effects of anesthetic agents on cerebral phys¬
iology (cerebral blood flow and metabolism, cerebral spinal fluid
formation and drainage) follow. The chapter concludes with a
discussion of the implications for anesthesia in patients with raised
ICP, where brain protection is desired, or where neurophysiological
monitoring is desired.

Anesthesiologists and intensive care physicians have long sought
a monitor that would accurately assess the state of cerebral oxy¬
genation, particularly in the unconsciousness patient. Current
monitors of cerebral oxygenation include jugular bulb venous

saturation catheters, cerebral oximeters, and oxygen tension
probes placed within brain parenchyma. Discussed in this chapter
are the technical aspects, experimental basis, clinical use, and lim¬
itations of each of these monitors.
Brain Protection During Neurosurgery 315

The initial concept of brain protection during neurosurgery is
based on research done in the 1970s 1980 which established the
concept that by decreasing cerebral metabolic rate the brain could
survive longer periods of ischemia. The first section of this chapter
reviews some of this initial research that promoted the use of
barbiturates for cerebral protection. The second section reviews
current anesthetic drugs and their potential for cerebral protection
in addition to the benefits of blood pressure, temperature and glu¬
cose control. The final section discusses the new mechanisms of
cerebral protection and the role old and new drugs may play in
the future for brain protection during neurosurgery.

tion on brain pathophysiology. Those factors that influence water
movement into the brain are examined, in order to provide
reasonable recommendations for peri operative fluid management
in the patients with brain pathology.

Interventional neuroradiologic procedure represents treatment of
central nervous system disease by endovascular access for the pur¬
pose of delivering therapeutic agents, including both drugs and
devices. For optimal anesthetic management, anesthesiologists
should be familiar with specific radiological procedures and their
potential complications. This article provides a brief overview of
special considerations in conducting general anesthesia, sedation,
and cerebral hemodynamic monitoring for patients undergoing
interventional neuroradiologic procedures.

Minimally invasive surgical techniques allow the surgeon to per¬
form a procedure through a small incision using state of the art
surgical tools and imaging systems. These advances benefit
patients and health care delivery; decreasing blood loss, postoper

ative pain and hospital stays. Neurosurgeons remain at the
forefront of these breakthroughs and neuroanesthesiologists must
continue to evolve their practice to meet surgical needs. This
chapter describes many of the currently changing procedures and
the anesthetic implications.

Patients with intracranial vascular disease are at high risk for
morbidity and mortality throughout hospitalization. The neuro
anesthesiologist has become an integral part of the multidis
ciplinary team required to care for these patients. Increasingly, the
neuroanesthesiologist must facilitate care during neuroimaging
and interventional procedures as well as provide skillful manage¬
ment during and after surgery. This article will review the risks of
intracranial vascular surgery, potential complications of the
disease, treatment options, and the special anesthetic considera¬
tions required for these patients.

agement of neurosurgical lesions in infants and children present

approach to the pediatric neurosurgical patient.

Current operative approaches to the spine range from band aid
lumbar discectomy and foraminotomy to extensive anterior/poste¬
rior resections for spinal tumors. In this chapter, perioperative and
intraoperative pathophysiologic conditions and procedures
relevant to the anesthesiologist are discussed including rheuma¬
toid arthritis, management of metastatic spinal tumors, posterior
lumbar inter body fusion procedures and laparoscopic approaches
to the spine. Considerations and new findings related to periopera¬
tive visual loss following spine surgery are also highlighted.

The current concepts in neurocritical care including advancement
in therapeutic interventions and monitoring modalities are cov¬
ered for four entities: stroke, subarachnoid hemorrhage, traumatic
brain injury and spinal cord injury. Although therapies were main¬
ly supportive in the past, acute ischemic stroke may now be treat¬
ed with tissue plasminogen activator if inclusion and exclusion
criteria are met. The management of subarachnoid hemorrhage
including cerebral vasospasm is discussed in detail. Traumatic
brain injury and spinal cord injury with prevention of secondary
injury to limit further sequelae are also covered. Medical compli¬
cations which increase morbidity and mortality are also presented.
