This article reviews the basic design of flow cytometers, as well as
many of the essential concepts for use of flow cytometry. The goal
of this article is to provide the reader with a fundamental under¬
standing of flow cytometry which will permit appreciation of
applications described elsewhere in this issue. A brief discussion
of cytometry safety and quality control/standardization also is
presented to introduce the reader to these concepts.
Immunophenotypic Analysis of Acute Lymphocytic

Acute lymphoblastic leukemia (ALL) is one of the most common
hematologic malignancies. Flow cytometry is an integral part of
ALL diagnosis and also provides significant patient prognostic
information. This article is a practical review of the basic principles
of the flow cytometric evaluation of acute leukemias, the inter¬
pretation of flow cytometric data, and the management of practical
problems such as aberrant antigen, hematogones, bone marrow
regeneration, and minimal residual disease.

Row cytometry has become one of the primary tools for increasing
our knowledge and understanding of leukemias, myelodysplastic
syndromes, and the myelodysplastic/myeloproliferative diseases.

Although the data generated by flow cytometry is not compre¬
hensive enough to completely subtype leukemias or myelodys
plastic syndromes into their myriad divisions, this wealth of
information does give phenotype, reproducible enumeration of
blasts, certain prognostic information, and can reveal the pres¬
ence of cell populations with aberrant antigen profiles. With the
evolution of classifications of leukemia and related conditions,
flow cytometry continues to provide critical information reliably
and quickly.
B Cell Lymphoproliferative Disorders 321

B cell lymphoproliferative disorders are a varied group of dis¬
orders, and include entities such as chronic lymphocytic leukemia,
mantle cell lymphoma, and hairy cell leukemia. Although morpho¬
logic and cytologic features are suggestive of the correct
diagnosis, it is often a signature immunophenotype that yields
the correct diagnosis. A combination of clinical history, morpho¬
logic appearance, and the immunologic profile will, in the
majority of cases, lead to the correct diagnosis. This article will
cover the immunologic features of the B cell lymphoproliferative
disorders.
Use of Flow and Laser Scanning Cytometry to Study
Mechanisms Regulating Cell Cycle and Controlling Cell Death 339

This article presents major applications of flow and laser scanning
cytometry in studies of cell cycle and cell neurobiology. Described
are representative examples of multivariate analysis of different
attributes of the cell that reveal correlations between cellular DNA
content; DNA replication; and expression of cyclins Dl 3, E, A, and
Bl. Also described are major applications of cytometry in studies
of apoptosis, specifically in measurement of Bax translocation to
mitochondria and nuclear factor kappa B to the nucleus, and in
detection of DNA fragmentation.
Monitoring of Cellular Resistance to Cancer Chemotherapy 357

Cellular resistance to a broad spectrum of natural products used as
antitumor drugs is believed to be a major cause for the failure of
chemotherapy. Flow cytometry has been used for monitoring the
expression of drug resistance markers, determining accumulation
of fluorescent drugs, and for screening of drugs that enhance
chemosensitivity by blocking efflux and enhancing drug retention.
This article reviews recent developments in our understanding of
the multiple drug resistance phenotype and the use of flow cyto¬
metry for the study of drug efflux and its modulation in human
tumor cells.

Reticulocyte Analysis by Flow Cytometry and Other Techniques 373

Enumeration of peripheral blood reticulocytes is an essential part of
the diagnosis and management of anemic patients, since the number
of reticulocytes in the peripheral blood reflects the erythrocytic
activity of the bone marrow. Reticulocyte enumeration using flow
cytometric methodology is rapidly replacing the inaccurate, imprecise
manual counting technique used in the past. This article explores
the pathophysiology of the reticulocyte, the various means of
counting reticulocytes, and the diverse clinical applications of
reticulocyte data.
Flow Cytometry of Platelets for Clinical Analysis 421

Row cytometry has proven quite effective in clinical examinations of
platelet function and pathology, especially with demonstration of
subpopulations of platelets with differing attributes. Detection of acti¬
vated platelets in vivo in thrombotic patients is a particularly critical
area of study, but it is made more difficult by ex vivo activation of
platelets in sample collection and preparation; however, use of
multiple markers and correlation with therapeutic interventions
strengthens the results and their prognostic value. While there are few
flow cytometry procedures well enough established with platelets to
be used in routine hematology laboratories, assays of receptor occu¬
pancy by flow will be of tremendous benefit in monitoring the new
platelet directed antithrombotic drugs being developed. Clinical
application of flow cytometry with platelets is still on a growth curve.
Hematopoietic Cellular Therapy: Implications for the Flow
Cytometry Laboratory 455

Cell therapy replaces, restores, or enhances the biological function
of a damaged tissue or system through the use of autologous or
allogeneic cells. These cells may be isolated, modified, cultured, or
changed to increase their therapeutic potential. Emerging research
in this area promises to open a new armamentarium of immuno
logic therapy of cancer through biologic and pharmaceutical
approaches. This article reviews major trends in hematopoietic
cell therapy, particularly with respect to the current and emerging
applications of flow cytometry.

The authors view of the future of flow cytometry is based on a
belief that the single most important aspect flow cytometry offers to

the investigator is high speed interrogation of correlated measure¬
ments on a cell by cell basis. Over the next several years, an enor¬
mous increase in the capabilities of cytometry in general, and flow
cytometry in particular, is likely to occur. A brief description of
some of those capabilities is the subject of this article.
