Larger volumes of sicker patients presenting with much later progression of their
diseases are characteristic features of tropical surgical illness, in comparison with
similar illnesses seen in temperate, developed areas of the world. This difference
in disease presentation is often based on poverty, ignorance, and biological fea¬
tures. Also, it is often mismatched against scarce medical care with few skilled
manpower resources, and a lack of medical supplies and high technology equip¬
ment. The challenges for developing skill in the management of tropical surgical
illnesses are less in the recognition of exotic diseases than in the adaptation and
ingenuity of improvising therapeutic approaches to this higher volume of later
stage diseases.
Population Dynamics of Surgical Tropical Diseases 10

The largest human populations with the highest growth rates and the youngest
population pyramids live in the tropical areas of the world. These facts have im¬
mediate implications for the incidence of disease and the needs for medical and
surgical services. Because of the skewed age curve of these populations (many
tropical countries have the median population age represented by teenagers) rel¬
ative to temperate populations, which have a proportionally larger burden of older
people and may even have zero population growth, disease incidence is remark¬
ably different. For example, the latter populations have a larger burden of the de¬
generative and malignant diseases of cardiovascular, central nervous system, and
gastrointestinal origin as well as musculoskeletal abnormalities that are acquired
later in life, whereas the tropical populations have a higher proportion of congen¬
ital, communicable, and traumatic illnesses largely on the basis of the age distri¬
bution of the respective populations. Venereal diseases would not be expected to
be a major concern in a geriatric population, whereas the teeming populations of
urban youth in many tropical port cities are a virtual hothouse for such epidemics.

This article summarizes clinical and laboratory aspects of some tropical diseases
and provides evidence needed to make a differential diagnosis. The importance of
preoperative nutritional evaluation is discussed. Nutritional support may dimin¬
ish surgical complications in the treatment of tropical diseases.

Chagas disease is an endemic, clinical entity caused by Trypanosoma cruzi, a par¬
asite transmitted to humans by a triatomid bug. It affects several million people
in Latin America, mostly in Brazil, Argentina, Chile, Paraguay, and Bolivia. This
disease causes several motility disorders in the digestive tract, as well as myo¬
carditis. The authors reviewed some clinical and surgical aspects of this disease
highlighting esophageal and colonic presentations.
Schistosomiasis: Upper Gastrointestinal Bleeding and Its Management 37

This article intends to analyze some of the characteristics regarding schistosomi¬
asis, an endemic disease in the northeast region of Brazil. The author focuses on
the most common clinical features of this disease, the upper gastrointestinal
bleeding, its treatment, and outcome.

Amebiasis is an infectious disease caused by Entamoeba histolytica, a unicellu¬
lar organism, which belongs to the pseudopod forming protozoan superclass Rhi
zopoda. Its pathogenesis is expressed most often as inflammatory or ulcerative
colonic lesions, which may account for a broad spectrum of intestinal symptoms.
Occasionally, amebae may gain access to extraintestinal sites, most commonly the
liver, where significant tissue destruction may occur.
Primary Intrahepatic Lithiasis 51

Intrahepatic lithiasis is defined as the presence of stones located above the junc¬
tion between the left and right hepatic biliary ducts. The presence of stones in the
intrahepatic biliary ducts is a consequence of three different circumstances. The
most frequent situation is the migration of gallbladder stones to the hepatic duct
and retrogradely into the intrahepatic ducts. Stones can also originate inside the
intrahepatic ducts as a result of biliary stasis secondary to iatrogenic bile duct
stricture, choledochal cysts, or biliary stenosis due to benign or malignant dis¬
eases. Primary intrahepatic stones, also known as hepatolithiasis or intrahepatic
lithiasis, a disease with unknown etiology, rare in the western world but very fre¬
quent in eastern Asia are the third circumstance. In the first two situations, intra¬
hepatic stones are considered secondary. In this article, we will focus our atten¬
tion on primary intrahepatic stones.

The hydatid liver cyst is an infection caused by the larval cyst Taenia Echinococ
cus. Human beings are intermediate hosts for Echinococcus granulosus. Indirect
systemic contamination occurs when the eggs reach a person s digestive tract.
More than half of the time, the embryo fixes in the liver, which causes problems.
Surgical solution is determined according to the patient s condition and the expe¬
rience of the surgical group.

Typhoid fever is a public health problem in developing countries and a major
problem for travelers to endemic areas. The broad spectrum of clinical manifes¬
tations may be an impediment in physicians establishing an early diagnosis. Ty¬
phoid fever can lead to a prolonged disease, and in some cases, a threatening con¬
dition—intestinal perforation.

The incidence of tuberculosis has increased in the past decade, mainly in patients
who are HIV positive. Each year, more and more cases are being diagnosed in de¬
veloping countries. Physicians must be aware of primary tuberculosis and sec¬
ondary organic compromise.
Lymphatic Filariasis 79

Lymphatic filariasis is one of the most prevalent tropical diseases and may lead
to severe disfiguration and disability. It is a major social and economic burden in
developing countries, Africa, Asia, and America. Recent advances in diagnosis,
monitoring, treatment, and especially prevention, constitute the basis for the elim¬
ination of filariasis as an endemic problem.

Tropical pyomyositis is an acute infection of skeletal muscle. Penicillin resistant
Staphylococcus aureus is the main etiologic agent. Muscles of the lower extrem¬
ities and trunk are commonly involved. Initial overlying skin and subcutaneous
tissue preservation can mask the diagnosis. Antibiotic therapy and prompt
drainage of abscesses leads to functional recovery.

Infectious arteritis is an inflammatory process caused by septic emboli or im
munologic phenomena. Recently, the increasing incidence has been attributed to
the statistically rising number of drug abusers and immunosuppressed patients.
Clinical signs of the disease are described, as well as etiologic agents. Currently
available management and procedures are also described.

The importance of ascariasis in children lies in its morbidity rate. As in adults, most
complications are due to the natural migratory instinct of the worm, penetrating
various natural orifices of the body, mainly in the gastrointestinal tract, and its ca¬
pability to interlace and form huge boluses that can lead to intestinal obstruction.

Abdominal angiostrongyliasis is a rare parasitosis caused by the nematode
Angiostrongylus costaricensis that affects children almost exclusively. Clinical

presentation is similar to appendicitis, with diagnosis being made on histologic
examination of surgical specimens in most cases. High grade eosinophilia in a
child with suspected appendicitis should prompt this differential diagnosis.
Amebic Toxic Colitis 103

Toxic colitis is a rare complication of intestinal amebiasis in children that, even
today, carries a high mortality rate. It is characterized by toxemia, fever, bloody
diarrhea, and extreme dilatation of the entire colon or a colonic segment. Early di¬
agnosis and treatment are critical to maximize the chance of survival.
