The last decade has seen considerable improvement in the under¬
standing and treatment of neonatal surgical disorders. This article
reviews areas of neonatal surgery in which there has been signifi¬
cant change, either in the surgical treatment strategy or technique,
including the role of prenatal diagnosis, application of vid
eoscopic and minimally invasive techniques, improvement in pre¬
natal imaging, and management of specific neonatal gastrointesti¬
nal disorders.
Newborn Intestinal Circulation: Physiology and
Pathophysiology 23

One of the most important differences between the circulatory
physiology of the newborn intestine and adult intestine is that the
basal vascular resistance across newborn intestine significantly
decreases in the first several days after birth, secondary to a
marked increase in the constitutive and stimulated production of
nitric oxide by the vascular endothelium. This resistance reduc¬
tion causes the nominal rate of intestinal blood flow, and hence
oxygen delivery, to be increased during early newborn life. The
importance of endothelium derived nitric oxide in regulation of

the newborn intestinal circulation renders it susceptible to ische¬
mia during periods of endothelial cell dysfunction.

The gastrointestinal tract presents the largest surface area of the
body that interfaces with the internal milieu. Some of the intricate
defense mechanisms that permit entry of nutrients while pre¬
venting invasion by harmful microbes and other potentially dam¬
aging agents are reviewed. This article provides a brief review of
several immunonutrients and other functional foods containing
probiotics and prebiotics that may contribute to the integrity of
the innate gastrointestinal defense system.

Gastrointestinal function has a significant impact on the nutri¬
tional support and clinical management of the neonatal infant.
Nutrition has a major influence on neonatal gastrointestinal
growth and function, particularly with respect to the modality,
quantity, and composition of nutrition. This article provides a
critical review of the nutritional and hormonal factors that affect
gut growth and development with an emphasis on their physio¬
logic relevance and therapeutic action in neonates.
Development of the Enteric Nervous System 97

The enteric nervous system (ENS) is required for normal gastroin¬
testinal motility and can function independently from the central
nervous system, and alterations in its development result in sig¬
nificant human disorders. Insights into the development of the
ENS have been obtained by the application of new approaches in
animal models. Such insights have been translated into under¬
standing of the pathophysiology and genetics of congenital disor¬
ders of ENS function in humans. This article highlights these
recent findings and their application to human disease.

Many complex changes occur in the gastrointestinal tract of the
developing infant that may alter the pharmacokinetics and phar
macodynamics of orally administered drugs. Although the avail¬
able knowledge of the pharmacokinetics of many drugs used for
the treatment of gastrointestinal disease in the perinatal period is

increasing, many knowledge gaps remain, particularly for the
preterm infant. In addition, well controlled efficacy trials for
many of the commonly used drugs are needed.
Gastroesophageal Reflux in the Neonate 135

Gastroesophageal reflux is a common problem in the neonate,
with varied clinical presentation. Much of the information on
gastroesophageal reflux in neonates is largely from older infants
and children. The symptoms resulting from gastroesophageal re¬
flux disease in neonates are not very clear with regards to cause
and association. This article summarizes the current literature
regarding physiologic and pathologic neonatal gastroesophageal
reflux to a larger extent, and discusses clinical presentation and
available treatment options of a disease whose pathophysiology
is not very clear in neonates.

This article gives an overview of the causes of neonatal cholesta¬
sis, defined as a marked impairment in bile flow. A brief overview
of the determinants of bile flow assists in the analysis of clinical
and laboratory parameters of cholestasis. It is only by having a
firm grasp of the various causes that a practitioner can optimize
his or her diagnostic approach and therapeutic management of
cholestatic infants.

Management of patients with short bowel syndrome represents a
formidable challenge. Aggressh e treatment including nutritional
care and anticipation of potential complications and rapid treat¬
ment of complications enhance outcome. New therapies offer
the promise of significantly improving morbidity and mortality.
Intestinal transplant is appropriate for infants who would other¬
wise die from liver disease, recurrent sepsis, or lack of venous ac¬
cess.
