Preterm premature rupture of the membranes affects 3% to 4.5%
of all deliveries and is associated with increased rates of perinatal
morbidity and mortality. Multiple predisposing risk factors for
preterm premature rupture of the membranes have been investi¬
gated and remain of considerable debate in the literature. This
article discusses many of these risk factors and the evidence that
supports them.

Intrauterine infection diagnosed in conjuction with prelabor rup¬
ture of the membranes (PROM) has traditionally been called
chorioamnionitis. Clinical chorioamnionitis has the potential to
cause serious complications in both mother and fetus. A discus¬
sion of the mechanisms of infection, the pathophysiologic rela¬
tionships between infection and PROM, the options for detecting
infection, and the management of patients with PROM once clini¬
cal chorioamnionitis is diagnosed are all critical to a complete
understanding of PROM.

Preterm premature rupture of the fetal membranes complicated
by oligohydramnios may have significant impact and sequelae
on pregnancy outcome. In this article the role of amniotic fluid
in fetal development, especially lung development, is reviewed;
complications resulting from oligohydramnios are outlined; and
the evaluated therapeutics and management schemes are deline¬
ated.

Midtrimester premature rupture of membranes is an infrequent,
but serious pregnancy complication that puts the fetus at risk
for preterm delivery and associated neonatal problems. Unique
neonatal complications may occur with prolonged latency. Ex¬
tended fetal exposure to oligohydramnios beginning early in
pregnancy may result in disturbed lung growth, resulting in
severe respiratory distress in the newborn, often associated with
skeletal deformations. Guidance for early neonatal management
is presented, based on pathophysiologic considerations.

Tocolytic therapy with preterm premature rupture of membranes
continues to be a controversial issue. Articles focusing on poten¬
tial benefits and adverse outcomes associated with tocolysis in
this disease process are reviewed. Although there may be some
short term neonatal benefit, further investigation of this issue is
warranted.

Antenatal corticosteroid administration in women with preterm
premature rupture of the membranes results in well documented
neonatal benefits. The improved survival and decreased morbid¬
ity categorically outweigh any persistent safety concerns. Unre¬
solved issues that require further research revolve around the
optimal exposure interval, the optimal timing of treatment, and
the optimal dosage schedule.

Preterm premature rupture of membranes remains an important
cause of preterm birth and neonatal mortality. Although the un¬
derlying pathophysiology remains largely undefined, subclinical
infection has been implicated both in the mechanism of mem¬
brane rupture and the resultant neonatal morbidity. The use of
maternal systemic antibiotics reduces both neonatal and maternal
morbidity in the expectant management of preterm premature
rupture of membranes. Although concern persists over the devel¬
opment of resistant strains of organisms involved with neonatal
sepsis, current data support the use of antibiotics in this setting.

The presence or absence of fetal pulmonary maturity as assessed
by amniotic fluid analysis and the role of fetal maturity tests in
the management of premature rupture of the membranes are
addressed. The hazards of the high falsely immature test are
carefully explored. A management scheme based on the results
of amniotic fluid analysis is also described.

The management of patients with premature rupture of mem¬
branes remains controversial. Generally, when patients are in
labor, have infection, or there is irreversible fetal distress there
are few options other than delivery. For those not in labor the
complexities of the many combinations of decisions to be made
make studying and solving the problem of the best option for
management difficult at best. These problems are somewhat unre¬
solved and several reasonable options often exist and are likely
to remain so for some time to come.

Preterm premature rupture of the membranes before viability is
an uncommon, yet devastating complication of pregnancy. Al¬
though the cause is not well understood, the perinatal morbidity
and mortality have been carefully documented, including the
substantial risk of pulmonary hypoplasia. During the past 5 years,
improved outcome has been reported, perhaps because of the use

of prenatal corticosteroids, antibiotics, neonatal surfactant, and
improved mechanical ventilation. The long term prognosis of ba¬
bies born with this complication, however, remains guarded.

This article reviews efforts to understand and develop methods
to treat iatrogenic and spontaneous previable premature rupture
of membranes (PPROM). The current success rate with the amnio
patch suggests that iatrogenic PPROM should no longer be
viewed as a devastating complication of pregnancy, but rather as
a readily treatable condition. In a select group of patients with
spontaneous PPROM there is the potential for developing a surgi¬
cal technique that may successfully graft the defect and allow the
integrity of the amniotic membrane to be restored.

The management of preterm premature rupture of membranes is
one of the many controversial areas of medicine. Many of the
protocols used to manage preterm premature rupture of mem¬
branes are not based on solid data. Other situations are rarely
encountered, making it very difficult to arrive at a management
scheme on which all can agree. This article presents five such
areas along with available literature and discusses treatment op¬
tions invloving these controversial or special topics.
