Defining high quality care and cost effectiveness is not an easy
task. In this article, the authors describe several strategies that
have practical applicability for the clinician treating patients with
sports related injuries. Integration of some of these strategies
may allow the more quality based, cost efficient use of available
resources in sports rehabilitation.
Shoulder Instability in the Athlete 729

This article reviews current concepts of shoulder instability, which
are influenced by increasing knowledge of shoulder anatomy,
biomechanics, and function. An overview of treatment options,
including arthroscopic techniques, is discussed. The difference
between shoulder laxity and instability and a review of arthro¬
scopic thermal capsulorrhaphy are highlighted.
Shoulder Injury in the Throwing Athlete 745

Successful nonoperative treatment of shoulder injury in the
throwing athlete requires a team approach. The physician, physi¬
cal therapist, and athletic trainer work together with the athlete
to determine a diagnosis and design a treatment protocol. Injury
recovery requires restoration of muscle strength and balance fol¬
lowed by a careful return to a throwing program.

Scapular Instability: The Scapulothoracic Joint 755

Instability of the scapula is increasingly recognized as a cause of
shoulder and arm pain. There are several distinct causes for this
malady, each of which requires appropriate work up. Treatment
is focused to correct the cause of instability. Nonoperative mea¬
sures are the mainstay of most treatment regimens. This article
describes possible causes and treatments of scapular instability
and outlines an appropriate rehabilitation plan.
Burners and Stingers 771

The burner or stinger syndrome is one of the most common
injuries seen in sports medicine. It has been reported to affect
more than 65% of college football players. Athletes describe a
burning or stinging pain that radiates down one upper limb.
Symptoms are usually temporary, lasting only a few minutes, but
can be recurrent and lead to permanent neurologic deficits. Burn¬
ers are primarily believed to be either an upper cervical root (C5
or C6), or upper brachial plexus trunk injury, but an injured
peripheral nerve can cause similar symptoms. Early recognition,
proper diagnosis, and appropriate management can help ensure
a quick recovery and prevent a more catastrophic event.
Spondylolysis 785

Spondylolysis is a relatively common radiographic finding and,
although frequently asymptomatic, can cause significant low back
pain. Isthmic spondylolysis likely represents a fatigue fracture in
the region of the pars interarticularis, relative to repetitive stress
of this region with spinal motion and weight bearing. Symptom¬
atic pars lesions are a clinical problem, particularly for adoles¬
cents, and show increased frequency of occurrence with a variety
of sports activities. Epidemiology, pathophysiology, clinical pre¬
sentation, imaging, and treatment of this disorder are reviewed
based on the available medical literature.
Pelvis and Sacral Dysfunction in Sports and Exercise 805

Pelvic and sacral dysfunctions resulting from sports related injur¬
ies are unusual. A simple groin pull, however, can prohibit an
athlete from participating in sports activities for an extended
period of time. Pelvic floor dysfunctions can be particularly im¬
pairing for women athletes, and are rarely discussed in sports
literature. This article reviews the anatomy, biomechanics, com¬
mon injury patterns, and treatment options for athletic injuries
related to the pelvis.
Internal Disc Disruption and Axial Back Pain in the
Athlete 837

Axial low back pain is a common complaint in the athlete. This
article focuses on a commonly overlooked source of axial back
pain; internal disc disruption and its inherent diagnostic and
therapeutic challenges. The authors examine the biochemical and
biomechanic theories of the pathogenesis of internal disc disrup¬
tion as they relate to the athletic population. The clinical presenta¬
tion and diagnostic tests for acute and chronic internal disc dis¬
ruption are discussed. Treatment options, including minimally
invasive procedures, are reviewed, and a goal directed rehabilita¬
tion program tailored to the competitive athlete is outlined.
Meniscal Tears in the Athlete: Operative and
Nonoperative Management 867

The meniscus has a dynamic role and contributes in many ways
to the healthy condition of the human knee. An active individual
with a tear in the meniscus may have meniscal dysfunction that
can negatively alter their sport participation and performance.
There are numerous factors to consider when treating an athlete
with a suspected or known meniscal tear. These factors include
chronicity of the tear, size, location, age of athlete, concurrent
intra articular injuries, and desired timing of return to competi¬
tion. Some athletes can recover with nonoperative treatment,
while others need operative intervention to recover and return
to play.
The Athlete with Early Knee Arthritis 881

Athletes participating in contact sports, or activities such as exces¬
sive running, are at higher risk for the development of knee
osteoarthritis. Newer advances in treatments, such as minimally
invasive management, are helpful in getting people active earlier
as compared with older, conventional methods.
Anterior Cruciate Ligament Injuries: Operative and
Rehabilitative Options 895

Though not life threatening, rupture of the anterior cruciate liga¬
ment (ACL) can be viewed as a debilitating injury. Instability of
the knee can result in a diminished quality of life for the athletic
individual, and changes in lifestyle for activities of daily living in
others. Surgical and conservative treatment options are available
with restoration to and improvement on premorbid status being
the goal of treatment. Over 500 ACL reconstructions are per¬
formed annually at the authors' institution and a strong emphasis
is placed on rehabilitation techniques and strategies.
Stress Injury to the Bone Among Women Athletes 929

Megan Staton
Stress fractures in women athletes may be the presentation of
more serious underlying medical conditions, such as the female
athlete triad. This article presents a general overview of stress
injury to the bone among women athletes. The history of women
in sports, basic bone biology, the female athlete triad, and risk
factors for stress fractures are discussed. Stress fractures can occur
at any anatomic site in women; however, they tend to occur
more frequently in the pelvis, sacrum, and femoral neck. Stress
fractures at these sites, therefore, are discussed in detail. Surgical
indications are also reviewed.
Performance Enhancing Supplements 949

The use of nutritional supplements commonly marketed as ergo
genic agents has skyrocketed in the last ten years. Two of the
most popular agents are creatine and androstenedione. This arti¬
cle explores the theoretic and biomechanic bases for the use of
these agents, and data that support or refute their ergogenic
claims. Safety issues associated with the use of these agents are
also discussed.
Pediatric Sports Emergencies 961

This article reviews the major categories of sports related medical
emergencies in young athletes. Sports participation is a healthy
and generally safe activity for child and adolescent athletes, and
should be encouraged by health care providers. Thankfully, medi¬
cal emergencies are rare events during youth sports participation.
When these events occur, however, prompt recognition and ap¬
propriate treatment are of tremendous importance.
