Diabetic ketoacidosis (DKA) remains the most common reason
for hospitalization and diabetes related morbidity and mortality
among individuals with diabetes in the pediatric age range. This
article discusses the underlying pathophysiology of DKA and
relates this to the successful management approach used to treat
DKA in children and adolescents. Particular attention is devoted
to important issues related to fluid management and prevention
and recognition of cerebral edema.
Diabetic Ketoacidosis and Hyperglycemic Hyperosmolar

Diabetic ketoacidosis (DKA) and hyperglycemic hyperosmolar
nonketotic syndrome (HHNS) are life threatening acute metabolic
complications of diabetes mellitus. Although there are some im¬
portant differences, the pathophysiology, the presenting clinical
challenge, and the treatment of these metabolic derangements are
similar. Each of these complications can be seen in type 1 or type
2 diabetes, although DKA is usually seen in patients with type 1
diabetes and HHNS in patients with type 2 disease. The clinical
management of these syndromes involves careful evaluation and
correction of the metabolic and volume status of the patient,
identification and treatment of precipitating and comorbid condi¬
tions, a smooth transition to a long term treatment regimen, and
a plan to prevent recurrence.

Sick day Management in Type 1 Diabetes 707

This article reviews the importance of sick day management in
persons with type 1 diabetes. Topics include current sick day
protocols and new technologies that may improve home manage¬
ment of uncontrolled diabetes in an attempt to avoid full blown
metabolic decompensation and the development of diabetic keto
acidosis (DKA). Prevention of DKA with adequate sick day man¬
agement will reduce the human and economic toll of this compli¬
cation. Increased awareness of this therapy, which involves
frequent monitoring to detect hyperglycemia and ketosis and sup¬
plemental insulin administration, is the key to its successful use
at home.
Hypoglycemia: Pathophysiology and Treatment 725

Hypoglycemia is a common limiting factor that prevents patients
with diabetes from achieving the most optimal level of glycemic
control. Redundant hormonal mechanisms, in great part regulated
by brain glucose sensing, operate to limit the glucose nadir and
permit recovery from hypoglycemia. Hypoglycemia unawareness
occurs following repeated bouts of hypoglycemia associated with
intensified diabetes management in patients with type 1 diabetes.
Patients with type 2 diabetes are more prone to hypoglycemia
associated with long acting sulfonylureas. This article discusses
the mechanisms underlying the differences in rates of hypoglyce¬
mia in the two forms of the disease and differences in specific
treatments to resolve this metabolic emergency.
Hospital Management of Diabetes 745

An estimated six million annual hospitalizations are accompanied
by hyperglycemia in the United States. There is a growing body
of literature that strongly supports the concept that optimizing
glycemic control in the hospital setting would result in improved
outcomes. This article reviews the rationale and evidence for tight
glycemic control, particularly among patients hospitalized for
cardiac disease, stroke, infections, and surgical procedures. The
authors give an overview of goals for control and the use of
insulin in the hospital setting, and among inpatients with hyper¬
glycemia in special circumstances. These circumstances include
the settings of stroke, myocardial infarction, and surgical proce¬
dures, including coronary artery bypass, enteral and parenteral
nutritional therapy, and glucocorticoid therapy.
Medical Emergencies in the Patient with Diabetes

Pregnancy in a woman with diabetes becomes a medical emer¬
gency when the diabetes is not strictly controlled before, during,
and after pregnancy. These complications may be life threatening
for the mother and unborn child. Other emergencies increase
morbidity for the mother and include retinopathy and nephropa
thy. All complications can be minimized when the attention is
directed towards achieving and maintaining normoglycemia.
Dangerous and Common Drug Interactions in Patients
with Diabetes Mellitus 789

The choice of medications for the patient with diabetes presents
the provider with a plethora of special problems not encountered
in the nondiabetic patient. In addition to the potential for adverse
interactions caused by prescription medications, the practitioner
must also be cognizant of the potential for interactions with over
the counter medications. This article provides a brief discussion
concerning the importance of and pharmacologic basis of drug
interactions, and also reviews some of the more common and
potentially dangerous interactions encountered in the manage¬
ment of patients with diabetes.
Nonhypoglycemic Drug Reactions of Agents Used to
Treat Diabetes 803

Although hypoglycemia is the most common side effect of the
pharmacologic agents used to treat diabetes, there are other im¬
portant adverse events that need to be considered. Furthermore,
hypoglycemia becomes less of an issue as insulin sensitizers are
used as monotherapy. The purpose of this article is to summarize
the nonhypoglycemic adverse events caused by the agents used
to treat diabetes.
Ophthalmalogic Emergencies in the Patient with
Diabetes 813

This discussion focuses on rapidly progressive eye symptoms
and emergent ophthalmologic problems in patients with diabetes
mellitus. Blindness is a feared complication of diabetes, and may
result from a variety of conditions, most commonly diabetic reti¬
nopathy. Urgent laser treatments have been shown to protect
vision in patients with proliferative diabetic retinopathy and clini¬
cally significant macular edema. Other emergent conditions dis¬
cussed are vitreous hemorrhage, retinal vascular occlusions, ocu¬
lar motor nerve palsies, and severe eye infections.
Managing Myocardial Infarction in the Diabetic Patient 831

Myocardial infarction (MI) is the leading cause of death among
persons with diabetes. Recent advances in the understanding and
treatment of cardiovascular disease in diabetes have made it
increasingly important to tailor therapy when treating this high
risk population. Because patients with diabetes are at a signifi¬
cantly higher risk of complications and death from MI, these
patients are most likely to benefit from early and aggressive
therapeutic intervention.
