Postoperative respiratory complications account for a preponderance of morbid¬
ity and mortality in the postoperative period. These complications constitute a
wide range of disorders; accordingly, preoperative identification of risk factors
for respiratory complications allows preoperative intensification of outpatient
treatment regimens in the higher risk patient groups.
Postoperative Cardiovascular Complications 242

Myocardial infarction, cardiac dysrhythmias, and venous thromboembolism are
common during the postoperative period. In this chapter we review the inci¬
dence, pathophysiology, and current management of these postoperative compli¬
cations.
Problems With Analgesia in the Postanesthesia Care Unit 254

Inadequate analgesia and postoperative pain remain common problems in the
postanesthesia care unit. Better understanding of the pharmacokinetics of anal¬
gesic drugs and newer pain modalities may improve patient satisfaction and out¬
come. Preemptive analgesia is potentially the next frontier to be conquered in
this arena.
Postoperative Bleeding 265

The best preoperative screen to predict perioperative bleeding is a carefully con¬
ducted medical history. Routinely obtained prothrombin time, activated partial
thromboplastim time, and bleeding time may indicate an appropriate therapeutic
course but must be interpreted with caution. Mechanisms leading to, and tools
assessing bleeding dysfunction are discussed. Prevention of the changes in
platelet function and coagulation factors that occur with even mild hypothermia
represents a means of reducing perioperative blood loss. Patients who receive
massive transfusions or develop signs of disseminated intravascular coagulopa
thy require an aggressive investigation of the underlying cause of bleeding to
ensure the most effective treatment.
Postoperative Nausea and Vomiting 274

The past decade has yielded important progress in our ability to prevent or treat
postoperative nausea and vomiting. With forethought and an appropriate appre¬
ciation of those patients at high risk for postoperative nausea and vomiting, it is
possible to design a cost effective, multimodal anesthetic plan that can attenuate,
if not entirely eliminate, this extremely unpleasant problem.
