The definition of a risk factor is discussed, and risk factors for primary
open angle glaucoma (intraocular pressure, age, race, family history, genes,
myopia, diabetes, systemic hypertension, migraine and vasospasm, and
gender) and primary angle closure glaucoma (race, age, gender, refractive
error, family history, and environmental factors) are reviewed. Both issues
and guidelines for screening for glaucoma are presented.

The ability of perimetry and visual field testing to provide useful clinical
information has been responsible for its long term use as a diagnostic proce¬
dure. This article concentrates on describing the pyschophysical principles
underlying achromatic perimetry and providing information on clinical use
of visual field testing in the evaluation of glaucoma. Particular emphasis is
placed on both identification of glaucomatous visual field defects and detec¬
tion of visual field defect progression. Two recently developed visual field
testing procedures that have demonstrated clinical utility are introduced:
short wave automated perimetry and frequency doubling technology pe¬
rimetry.

This article gives a brief, yet comprehensive look at the technology and
literature available to image the optic nerve head and the nerve fiber layer
in glaucoma. Many new methods, such as retinal thickness mapping, multi
focal electroretinography (ERG), and pattern ERG are trying to prove them¬
selves, whereas the existing devices are constantly being updated with new
knowledge and attempts to increase utility.

Not long ago, glaucoma was considered to be caused by pressure related
damage to the optic nerve. Intraocular pressure (IOP) is now considered a
risk factor for glaucomatous optic neuropathy. Current therapies for glau¬
coma are aimed at lowering IOP. Establishing a target IOP is an important
part of glaucoma therapy. This article reviews the rationale for establishing
target IOP and outlines a step wise approach.

With the advent of multiple new medications for the treatment of glaucoma
and the utility of early argon laser trabeculoplasty as an adjunctive therapy
early in the progression of glaucoma, the clinician has the tools necessary
to control the visual field loss and optic nerve changes characteristic of
glaucoma. The article discusses the most effective and appropriate treat¬
ments for patients at significant risk for developing glaucoma.

Rapidly increasing understanding of the origin of disease states on a molecu¬
lar, generic, and biochemical level is opening new doors for treatment op¬
tions, if not eventual cures. This article provides an overview of tailoring
treatment to the underlying mechanism in angle closure glaucoma and
describes present and future approaches toward understanding and dealing
with the two most commonly identifiable open angle glaucomas, pigment
dispersion syndrome and exfoliation syndrome.

The angle closure glaucomas are a diffuse group of disorders characterized
by the final common endpoint of iridotrabecular contact and elevated intra¬
ocular pressure. Understanding the anatomic and pathophysiologic mecha¬
nisms involved in the etiology of the various forms of angle closure is critical
for both diagnosis and management. This review discusses the classification,
etiology, and therapy of the various types of angle closure glaucoma and
the role that ultrasound biomicroscopy has played in helping understand
this heterogeneous disease.

Normal tension glaucoma (NTG) is defined as an optic neuropathy charac¬
terized by optic disc excavation and visual field loss in the setting of open
anterior chamber angles and statistically normal intraocular pressure (IOP).
In this article, the clinical findings of NTG are reviewed, some of the theories
of the pathogenesis of the disease are considered, and the evidence support¬
ing the use of treatment strategies that lower IOP are described. Promising
novel approaches to the treatment of this sight threatening disease are dis¬
cussed also.

Physiologic and cellular mechanisms of retinal ganglion cell death in glau¬
coma are discussed in a two stage paradigm. The primary insult in glaucoma
involves elevated intraocular pressure, which is implicated in structural
changes in the lamina cribrosa and anterior optic nerve ischemia. The sec¬
ondary degenerative process involves extracellular and intracellular signals
that ultimately result in the ganglion cell death. Based on experimental
and clinical evidence, the rationale for various neuroprotective strategies
is described.

The recent advances of the human genome project have made genetic analy¬
sis of many human disorders possible including hereditary forms of glau¬
coma. This article reviews the genkics of congenital glaucoma, the develop¬
mental glaucomas including Rieger anomaly, pigmentary glaucoma,
pseudoexfoliation glaucoma and adult and juvenile primary open angle
glaucoma. The future clinical use of genes associated with glaucoma for
DNA based diagnosis and gene based therapeutics is discussed.
Molecular Genetics in the Clinical Practice of Glaucoma 489

This article discusses recent advances in molecular genetics of glaucoma,
as they pertain to the clinical practice. Over a dozen loci have been associated
with various subtypes of glaucoma, ranging from congential to pigmentary
glaucoma. Six additional loci have been associated with primary open angle
glaucoma. Although these advances are significant and meaningful, genetic
screening has not yet penetrated clinical practice.

This article reviews the background, rationale, and surgical indications for
use of the antiprofliferative agents 5 fluorouracil and mitomycin as adjuncts
to glaucoma surgery. Although both agents seem to enhance surgical success
after glaucoma filtering surgery, mitomycin seems to be more effective in
high risk eyes. Appropriate operative and perioperati ve techniques for these
agents are described. In addition, methods to avoid and manage the compli¬
cations associated with the use of these agents are discussed.
The Failing Filtering Bleb 517

Blebs fail when the conditions necessary for their continued existence cease
to exist. The investigation of failing blebs entails taking a relevant history
and performing a step wise evaluation at the slit lamp, with the aid of
gonioscopy and often a Ritch or Hoskins lens to view the interior structures
of blebs, to determine the site of obstruction. This article discusses causes
of bleb failure and different modalities of treatment.

Late Complications of Glaucoma Filtration Surgery 531

The advent of antifibrosis therapy has increased the success rate of glaucoma
filtration surgery in high risk eyes. There has been a concomitant increase,
however, in the rate of postoperative surgical complications. Early postoper¬
ative adverse events (in the first postoperative month) include hypotony,
shallow or flat anterior chamber, wound leakage, and serous or hemorrhagic
ciliochoroidal effusion. This article reviews late complications of glaucoma
filtration surgery with emphasis on the incidence, pathogenesis, clinical
recognition, and management of the commonly observed adverse events.
Surgical Management of Coexisting Glaucoma and Cataract 545

Visual recovery is slower, intraocular pressure is higher, bleb survival is
lower, and the need for antiglaucoma medications is greater when cataract
surgery is combined with trabeculectomy. Sequential (staged) surgery for
eyes with coexisting open angle glaucoma and cataract is a safe and useful
option for the surgeon. Cataract extraction is performed using a temporal
clear corneal phacoemulsification approach, either prior to or after an antifi
brotic trabeculectomy. Staging maximizes the benefits of each surgical com¬
ponent, while minimizing complications.
