The preparticipation examination (PPE) is a widely practiced
method of screening athletes for medical limitations before sports
participation. Although the PPE is widely implemented, there is
little consistency regarding content or method in providing this
important service. This lack of uniformity undermines the overall
effectiveness of the PPE. The history efficiency, and nature of
athletic screening in adolescents are discussed in this article, and
specific suggestions are offered on how the examination might be
improved in the future. As sports participation among teens
continues to change—in number of participants and in athlete
demographics—and as the field of sports medicine expands
equally rapidly, the adolescent PPE, the commonest interaction
between adolescent patients and the medical system, should be
updated.
Strength Training for Children and Adolescents 593

Despite traditional concerns associated with youth strength train¬
ing, a compelling body of evidence now suggests that strength
training can be a safe and effective method of conditioning for
children and adolescents provided that appropriate training
guidelines are followed. In addition to increasing muscular
strength and improving selected motor performance skills, regu¬
lar participation in a strength training program may better pre

pare young athletes for sports participation and may reduce the
incidence and severity of sports related injuries. When designing
strength training programs for children and adolescents, it is
important to be aware of the potential benefits and concerns
associated with this type of training and interindividual differ¬
ences in stress tolerance. This article explores the effects of
strength training on children and adolescents and highlights re¬
lated benefits, concerns, and program design considerations.

Skeletally immature athletes do get major knee injuries. Hemar
throsis is associated with peripheral meniscal tears, anterior cruci¬
ate ligament ruptures, tibial tubercle avulsion injuries, and patel
lar/femoral osteochondral fractures and cannot be ignored. The
primary diagnostic tool for patients with a knee injury is a clinical
examination by a physician well trained in knee evaluation. MR
imaging has significant limitations in this age group. The algo¬
rithm for anterior cruciate ligament injury treatment must take
into account the patient s physiologic maturity, not chronological
age
Lower Limb Injuries in Children in Sports 637

This article presents an overview of sports related injuries of the
lower limb in children, with emphasis on management. The spe¬
cial injury related conditions of childhood, epidemiology, and the
particular pattern of injuries are discussed. Because the increased
participation of children in sports will continue—and permanent
damage will continue to be a risk—the authors stress preventive
measures, such as the performance of stretching exercises with
warm muscles and the gradual increase in activities, and the
responsibility of parents, coaches, and healthcare professionals
for safe training and minimizing the potential for injuries.

Back injuries in young athletes affect growth tissues. Different
sports produce distinct injury patterns. Acute and chronic injuries
may produce debilitating problems. Immediate and long term
interventions are important to limit further injury. An accurate
diagnosis directs the appropriate care.

Shoulder dysfunction in the young athlete usually is manifested
differently than in the adult counterpart. The physiology and
biomechanics of a growing child and adolescent result in different
injury patterns that require different and thoughtful approaches
to diagnosis and treatment modalities. Judicious use of surgical
interventions, however, can improve patient outcome signifi¬
cantly and return them to their sport of interest. Postoperative
rehabilitation and proper training techniques are essential to en¬
sure continued participation of the athlete.
Head and Neck Injuries in Young Athletes 693

Although injuries to the head and cervical spine are considered
relatively uncommon in sports medicine, they can have a major
impact on the individual athlete and the sport. These injuries
account for some of the most catastrophic injuries seen in all
levels of athletic participation. Concussion in sports is have a
major impact on professional and college levels and is reaching
epidemic proportions. The effects of paralyzing neck injuries in
sports are devastating and are followed closely by our national
media. In this article, the epidemiology, pathophysiology, and
types of injury are reviewed, with in depth discussion of cranial
and spinal injuries.

This article reviews the injuries most commonly seen in young
dancers. Special emphasis is on the biomechanics that (1) precipi¬
tate these injuries and (2) need to be addressed in preventing and
rehabilitating them. Most dance injuries are microtraumatic in
nature, mainly resulting from errors in technique or repetitive
application of techniques to bodies that are anatomically ill pre¬
pared to perform them.

As a weightbearing or high impact activity, figure skating is one
of the best sports for increasing bone mineral density and perhaps
for preventing osteoporosis. At least half of all competitive figure
skating injuries seem preventable. Boots should be as flexible as
the skater can control and should be carefully fitted. Normal
flexibility of lower extremity muscles, especially in the growing
athlete, markedly decreases the incidence of overuse symptoms

of the knee. Core body strength is critical for controlling high
impact jump landings and decreasing injuries to the spine, pelvis,
and hip girdle. Adequate shoulder strength is required for pair
skaters and ice dancers. A well designed off ice training program
decreases injury incidence and enhances performance. Assisting
these dedicated athletes is truly rewarding as they train and
compete at their highest potential, with passion for their sport.
The Young Gymnast 757

Intensive gymnastics training places the skeletally immature ath¬
lete at risk for certain acute and overuse injuries. An understand¬
ing of the sport of gymnastics is essential in dealing with the
young gymnast. This article summarizes the changes that have
occurred in the sport and how these changes may affect the risk
for injuries. Diagnosis and management of common acute and
overuse injuries of the extremities and spine are reviewed. The
female athlete triad (disordered eating, amenorrhea, and osteopo¬
rosis) is discussed in the context of gymnastics.
Musculoskeletal Injuries in the Young Tennis Player 781

The varied and frequent demands imposed on the young athlete
by tennis are associated with a high incidence of injuries. Most
of these are overload in nature and are in the lower and upper
extremities. The injuries are more sport specific than gender spe¬
cific, and they frequently are associated with maladaptations in
flexibility or muscle strength. Preventative conditioning may be a
method to decrease the maladaptations and injury risk.

This article is intentionally broad in scope, as a result of a collabo¬
ration from the fields of primary care sports medicine, orthopedic
surgery, and kinesiology. What has been borne out in the process
is a true appreciation for the benefits of a multidisciplinary ap¬
proach toward providing care for the young athlete with a physi¬
cal disability. Joint involvement of parents, coaches, trainers,
physical therapists, orthotists, prosthetists, wheel chair engineers,
neurologists, physiatrists, nutritionists, and, most importantly, the
athletes themselves should be further encouraged, because each
discipline provides a unique perspective in the identification and
management of health related issues. It is the intent of this article
to provide readers with at least some new insight that they can
carry into their future practice.

With the explosion of the number of youth participating in sports
activities has come an epidemic of injuries. Myriad factors make
their prevention a great challenge for society. This article presents
an overview of statistics, factors contributing to sports injuries, a
review of significant prevention efforts, barriers to prevention,
and recommendations for the future.
