The endoscopy era made it possible to see many of the diseases
that were being treated by clinicians. The use of endoscopic
biopsy further enhanced that ability. This article illustrates how
gastrointestinal biopsy and other practices can be improved so
that patients benefit more than they might otherwise. This article
focuses on pinch biopsy forceps technique and on dialogue with
the pathologist.
Molecular and Immunologic Pathology for the
Endoscopist: Special Techniques 573

The surgical pathologist can add diagnostic power to the endo¬
scopic biopsy beyond that obtained from the hematoxylin and
eosin stained slide. Molecular and immunologic techniques use
nucleic acid hybridization fidelity and antigen antibody interac¬
tion to bring increased specificity to diagnoses suspected on clini¬
cal and pathologic grounds. This article describes techniques that
are practical and applicable to the endoscopic biopsy. Most of the
techniques described require no additional endoscopic effort. As
a result, more specific distinction among malignancies, benign

conditions, and infectious diseases can be made from standard
endoscopy.
Endoscopic Mucosal Resection: Current Concepts 595

Recent advances in endoscopic mucosal resection of superficial
early digestive tract cancers are truly remarkable. The extraordi¬
nary long term outcomes of patients who have been treated with
endoscopic mucosal resection have encouraged the widespread
practice of endoscopic mucosal resection in Japan. These mini¬
mally invasive techniques allow safe and effective treatment of
diseases that would otherwise require major surgery. This article
provides an overview of endoscopic mucosal resection tech¬
niques, their associated outcomes, and other potential applica¬
tions of endoscopic mucosal resection.
Endoscopic and Endosonography Guided Fine Needle

Endoscopic ultrasound guided fine needle aspiration is emerging
as the preferred technique for the cytologic diagnosis of various
gastrointestinal lesions. This technique may not be routinely
available, but there is still a role for endoscopic or endoscopic
ultrasound assisted fine needle aspiration. This article provides
an overview of the evolution of these various techniques and
discusses the advantages, disadvantages, indications, and contra¬
indications of each.
Gastrointestinal Mucosal Biopsy in HIV Disease and

The role of the gastroenterologist as consultant for patients with
HIV infection is reviewed, with a particular focus on when endos¬
copy with biopsy may be helpful in the diagnostic evaluation.
Suggestions on where to biopsy, how to collect samples, and what
pathologies might be anticipated are included. In the clinical
setting of new antiviral therapies, there has been a dramatic
change in the etiologic factors for common presentations such
as diarrhea. A review of suspect infections and malignancies is
included.

children and adults, the mucosal manifestations are the same.

nal procedures and mucosal biopsy that are of a particularly or
peculiarly pediatric nature. This article discusses, in detail, the
issues pertaining to endoscopy and other techniques of mucosal
biopsy in children. Some approaches and techniques are consider¬
ably different than those in adults.

the diagnosis of Barrett s esophagus. Special considerations for
screening and biopsy technique are discussed. The value of
esophageal biopsy and cardia biopsy in patients with reflux is

of the evaluation of infective esophagitis and esophageal cancer.

logic zones of the stomach, when to biopsy to rule out neoplasia,
and biopsy in benign gastric mucosal disease.
Small Intestinal Mucosal Biopsy for Investigation of
Diarrhea and Malabsorption in Adults 739

The use of small intestinal biopsy for diagnosis in diarrhea and
suspected malabsorption depends on an optimal interaction be¬
tween the clinician endoscopist and the pathologist. This necessi¬
tates open and interactive communication between involved phy¬
sicians and an appreciation for correct tissue handling and biopsy
orientation in the endoscopy unit and the pathology laboratory.
Classification of biopsy changes on the basis of architectural ab¬
normalities in the small intestinal biopsy may be helpful in defin¬
ing the diagnosis and include severe (flat) or variably severe
(mild or moderate) abnormalities. For some small intestinal disor¬
ders that are characterized by diarrhea or malabsorption, the
biopsy findings may be distinctive and lead to a specific diagno¬
sis. For others, like celiac disease, the changes are less specific,
and it has become better recognized that an increasing number
of conditions can produce similar histopathologic changes. Defi¬
nition of typical gluten sensitive biopsy changes in this disorder
is critical.

Colonoscopy Plus Biopsy in the Inflammatory Bowel

Biopsy of the colon is an important diagnostic tool in the investi¬
gation of the inflammatory bowel diseases. There are a number
of scenarios where colon biopsies are of particular importance,
such as biopsies in the patient with undiagnosed diarrhea, distin¬
guishing different forms of inflammatory bowel disease, assessing
disease extent and activity, differential diagnosis of and diagnos¬
ing other disorders superimposed on inflammatory bowel dis¬
ease, neoplasia in patients with inflammatory bowel disease, and
colonic biopsy as a mirror of generalized gastrointestinal or sys¬
temic disease. This article discusses the use of colonoscopy with
colon biopsies for each of these scenarios.
