Infertility: Contemporary Office Based Evaluation and

Infertility is a common but treatable condition. Diagnostic tests,
including tests that couples can perform at home, tests that all
couples need (semen analysis and hysterosalpingogram), and
other office based tests, help the clinician determine the best
treatment option. Currently, intrauterine insemination and ovula
tion induction are the more popular treatment options.
Contemporary Approach to the Male Infertility

Evaluation of infertile couples has revealed that male factor infer¬
tility contributes to the problem in up to 50% of cases. Evaluation
of the male infertility patient may include endocrine studies,
sophisticated semen testing, and select radiographic studies. Re¬
versible and life threatening causes of male infertility should be
identified before proceeding directly to assisted reproductive
technology. For cases with irreversible causes, a proper evaluation
can identify patients who may be treated with the breakthrough
method of intracytoplasmic sperm injection. Many men who were
previously thought to be infertile may now initiate their own
biologic pregnancies. This article emphasizes the essentials of
the contemporary approach to evaluating the male factor in an
infertile couple.

This article clarifies the use of statistics to describe the success of
infertility treatments, with specific emphasis on assisted repro¬
ductive technology. A discussion of the January 2000 Society for
Assisted Reproductive Technology/Centers for Disease Control
report for procedures performed in 1997 is included. Limitations
on the use of statistical analysis and the problem of multiple
gestation are addressed. A glossary of terms is included for refer¬
ence.

In the past, clinicians have counted the number of spontaneous
abortions among couples in an attempt to determine who should
be evaluated for recurrent abortion. A better approach is to exam¬
ine the environment in which pregnancy has occurred or been
planned. This article discusses the previously held etiologies re¬
garding spontaneous abortion. Evidence for and against these
causes is presented, and the causes are included in contemporary
evaluation and treatment paradigms.

some form of hirsutism, the presence of prominent facial and

body hair caused by excessive androgen effect. Treatment meth¬
ods are classified into three categories: (1) hormonal suppression
consisting of medical therapy to decrease androgen production,
(2) the administration of antiandrogens to block the androgen
effect at the hair follicle, and (3) physical methods of hair removal.

Ovary Syndrome 583

Polycystic ovary syndrome (PCOS), one of the most common
clinical abnormalities, is associated with infertility, obesity, and
metabolic dysfunction. A significant proportion of obese and lean
women with PCOS have greater insulin resistance than is ex¬
pected. Resistance is accompanied by a defect in insulin secretion,
and recent studies have documented a dramatically high rate of
diabetes mellitus and impaired glucose tolerance in this popula¬
tion of women. Medications that reduce serum insulin levels have
been administered to women with PCOS to study the role of
hyperinsulinemia in the pathophysiology and treatment of PCOS.
This article presents the evidence for hyperinsulinemia and insu¬
lin resistance in women and reviews the therapeutic reponse to
insulin lowering agents. Recommendations are made for further
research and the use of new agents in the treatment of PCOS.

the market. This article presents the histories, descriptions, and
future trends for oral contraceptive pills, hormonal implants and
injections, and intrauterine devices. Other methods discussed in¬
clude barrier methods, spermicides, and emergency contracep¬
tion, and permanent sterilization.

Premenstrual Syndrome: Advances in Diagnosis and

The diagnosis and treatment of premenstrual syndrome (PMS)
has advanced significantly in the past decade, in particular, for

premenstrual dysphoric disorder (PDD), a severe form of PMS
with strict diagnostic criteria. Although the underlying patho
physiology remains unclear, increasing evidence supports an al¬
teration in sensitivity of central nervous system neurotransmit
ters, particularly serotonergic transmitters, to fluctuating levels of
ovarian sex steroids. Successful therapy for PDD is achieved
with serotonergically active drugs, such as the selective serotonin
reuptake inhibitors. Increased attention is being placed on inter¬
mittent luteal phase administration of these drugs.

Endometriosis is a complex disorder affecting the quality of life
of many women in the reproductive age group. Treatment modal¬
ities focus on relief of symptoms. Selecting the optimal therapy
for patients requires assessing each patient's symptoms and re¬
productive needs.
Update on the Medical Treatment of Ectopic Pregnancy 653

Medical treatment of ectopic pregnancy with methotrexate has
been shown to be effective and safe in appropriate patients. When
considering medical management, the definitive diagnosis of an
ectopic pregnancy and the assurance that the patient is a candi¬
date are of paramount concern. An understanding of the mechan
sim of action and pharmacokinetics of methotrexate allows the
clinician to better inform patients, recognize and treat side effects,
and be cognizant when treatment is unseccessful. Two common
protocols, the "multi dose" and the "single dose" protocols, have
excellent success rates; however, neither one of these is com¬
pletely effective. The "multi dose" protocol appears to have a
higher success rate than the "single dose" protocol.
New Approaches to Management of Fibroids 669

A number of new treatments are available for management of
leiomyomas, including supracervical hysterectomy, laparoscopic
myomectomy, myolysis, and uterine artery embolization. All of
these techniques can be performed as minimally invasive proce¬
dures. Little is known about long term outcome for these treat¬
ment modalities.
