Now at the end of its second decade, the AIDS epidemic is a
major world crisis affecting the health and psychologic well being
of tens of millions of individuals. In the developed world, treat¬
ment of pregnant women with antiretroviral therapy has lead to
dramatic decreases in children being infected, but in the devel¬
oping world, such treatment is yet to become a reality. Although
the number of children orphaned by this disease is already very
high in some areas, as the epidemic ages and more people die,
this will become a problem of unprecedented magnitude. Of
particular concern is the increasing rate of infection among young
people below the age of 25 that now account for one half of all
new infections.
HIV Infection in Children: A New Hope 279

Further understanding of the pathophysiology of HIV infection
and recent advances in the development of antiretroviral medica¬
tions have led to dramatic changes in the ways that children and
adolescents are affected by the disease and have provided a hope
that was non existent in the early years of the epidemic. These

advances have posed new challenges, however, for ensuring that
each infected child and adolescent receives the maximum avail¬
able benefits. While it is likely that there will continue to be
pauses in the pace of progress, the future holds real promise.
The third decade of the AIDS epidemic should see additional
developments that will continue to improve the lives of those
infected with the virus.
Children, Adolescents, and Families Infected and
Affected by HIV and AIDS 295

An estimated 80,000 to 125,000 children in the United States will
be orphaned by the year 2000 as a result of an HIV related illness.
Studies have found that the support of a surviving parent is
positively associated with the grieving child s ability to cope with
the loss of the other parent. HIV/AIDS affected children often do
not have another parent available to them. This article will review
some of the issues, struggles, and reactions of children and ado¬
lescents who have been affected and infected by HIV. The mate¬
rial is illustrated by two cases, one of a child and an adolescent
affected by HIV and the second a child and an adolescent infected
and affected by HIV/AIDS.
Home based Treatment for Children and Families
Affected by HIV/AIDS: Dealing with Stigma, Secrecy,
Disclosure, and Loss 313

Children affected by HIV/AIDS face multiple challenges beyond
the disease itself. This article discusses the mental health and
treatment needs of HIV affected children and families, with par¬
ticular attention to the important issues of stigma, secrecy, disclo¬
sure, and loss. A home and community based intervention that
takes a developmentally informed, psychoanalytic perspective is
described. The goal of this program, which is supervised by the
authors, makes use of a multi systemic approach to assist HIV
affected children and their families to work through the psycho¬
logical, health, economic, and other burdens placed upon them
by the disease.
Groups: An Effective Strategy for Intervention 331

Therapeutic efforts in a group format have been practiced suc¬
cessfully since the late nineteenth century. Children and adoles¬
cents affected by HIV/AIDS are among the populations that have
been found to benefit from participation in groups. This article
outlines the theoretical approach taken by one such group pro

gram and describes the ways in which clinically defined support
groups can provide an appropriate and effective intervention for
children deeply troubled by shame, secrecy, and loss associated
with HIV disease. Case studies are used as illustration.
Providing Clinical Opportunities for Youths
Affected by HIV 347

This article describes a youth centered program that offers an
opportunity for HIV affected youth to overcome their resistance
to treatment and become engaged in clinical experiences by focus¬
ing on their strengths and interests, and helping them to develop
friendships and trust within the agency. Through active outreach,
community involvement, and special projects the author uses
case material to demonstrate the ways in which the adolescent s
sense of isolation is gradually decreased, and individual treat¬
ment is made possible.
Relative Caregiving: An Option for Permanency 359

Children affected by HIV/AIDS are at high risk for multiple
placements as the adults in their lives are often too incapacitated
to provide stable, consistent care for them. This article provides a
historical overview of child caregiving, describes the complex
needs of HIV/AIDS affected children and their relative caregiv
ers, usually grandparents, and suggests that with necessary sup¬
ports, placement with relatives can be a thoughtful, realistic op¬
tion for permanency.
Teaching Young Children about HIV and AIDS 375

Given the younger age of initiation of sexual behavior, the real
risk of HIV infection for youth, and the greater efficacy of inter¬
vention that begins prior to active sexual participation, effective
AIDS prevention programs need to begin during the elementary
grades. This article outlines ways that mental health clinicians
can promote and support elementary school AIDS education pro¬
grams and addresses potential barriers to implementing such
programs. The article concludes by offering recommendations for
future directions for AIDS prevention efforts at the elementary
school level.
School Based HIV Prevention: A Promising Model 389

This article explores connections between the mental health and
educational systems by profiling the Redefining Actions and Deci
CONTENTS v
sions (RAD) curriculum, a theory based, developmentally in¬
formed HIV education program for middle and high schools.
RAD was developed by Deborah R. Schoeberlein and evaluated
by Dr. John Brett. RAD s public health approach, which has
gained considerable attention at the national level, argues that
HIV prevention, either through risk abstinence or risk reduction
techniques, is an appropriate strategy for safeguarding commu¬
nity health.
Guardianship as a Therapeutic Option 407

Recognizing the importance of consistency on the part of adult
caregivers to the children they care for, a number of states have
amended the guardianship law to provide new options for per¬
manency. These options include standby guardianships, co guard¬
ianships, and subsidized guardianships. This article summarizes
the evolution of guardianship law in response to the HIV/AIDS
epidemic and highlights a number of the challenges that remain.
National and State Policies Influencing the Care of
Children Affected by AIDS 425

This article addresses the difficulty that secrecy and stigma im¬
pose on the ability of HIV affected children and youth to benefit
from national and state policies and programs that provide gen¬
eral types of care and assistance. The author calls for the develop¬
ment of new policies, programs, and interagency planning and
collaboration designed to specifically meet the variety of needs
of affected children and families.

This article describes the plight of HIV affected children, and the
work of mental health professionals with HIV affected children
and their families, and presents pertinent research findings on
the topics. The concepts of prevention, resilience, and family and
child development are applied to the organizing of mental health
care for HIV affected children.
