Laparoscopically directed salpingoscopy, hysteroscopically di¬
rected falloposcopy, and transvaginal hydrolaparoscopy are tech¬
nical advances used to evaluate tubal condition in infertile women.
These endoscopic techniques help to identify appropriate patients
for tubal surgery, in vitro fertilization, or controlled ovarian stimu¬
lation. The improved instrumentation allows the use of minimally
invasive techniques for the diagnosis and treatment of infertile
women. Early diagnosis and intervention are helpful to gynecolo¬
gists in deciding on the best course of action for patients.
Laparoscopic Tubal Anastomosis 17

Reversal of tubal sterilization is conventionally performed by mi¬
crosurgery via laparotomy but in the past several years has also
been done by laparoscopy. This article outlines five technical steps
of laparoscopic tubal anastomosis that have been developed at the
authors center. Recent series of laparoscopic tubal anastomosis

show that the pregnancy rate following this procedure is substan¬
tially higher than previously reported. Accordingly, this operation
can be considered an alternative choice for women seeking reversal
of tubal sterilization.
Microlaparoscopy and Conscious Pain Mapping for the
Diagnosis and Management of Pelvic Pain 31

Chronic pelvic pain is a complex disorder with multiple etiologies.
Recently, the techniques of microlaparoscopy under local anesthe¬
sia and conscious pain mapping have been used to aid in the
diagnosis and treatment of these patients. Although exciting new
techniques hold great potential to improve the treatment of pa¬
tients, they are still being refined, and practitioners must under¬
stand the appropriate use and limitations of these procedures. This
article reviews the history and use of microlaparoscopy for patients
with acute or chronic pelvic pain and the controversies surrounding
this procedure.
Laparoscopic Treatment of Polycystic Ovary

Laparoscopic ovarian surgery is an effective therapy for polycystic
ovarian syndrome in women with anovulatory infertility. It is an
alternative treatment to gonadotropin or luteinizing hormone
releasing hormone in clomiphene resistant anovulatory women. It
can also be considered in women with polycystic ovarian syndrome
in whom in vitro fertilization treatments are cancelled owing to
ovarian hyperstirnulation and in women who have sustained ovar¬
ian hyperstimulation syndrome.

Laparoscopic treatment of ovarian endometrioma by stripping the
cyst wall from the remaining ovarian tissue appears to be the
most feasible surgical treatment. This procedure is technically more
demanding than fenestration but is associated with prolonged
symptomatic improvement, a lower recurrence of pelvic pain, and
a lower reoperation rate. The cumulative pregnancy rate is also
higher than after fenestration only. Concerns that excision of the
endometrioma will result in more adhesion formation and the loss
of ovarian follicles are not supported by the results of a prospective
randomized study.

Uterine Artery Embolization as Therapy for Myomata 77

Limitations of surgical and drug therapies for myomata, the most
common benign tumors, have promoted the search for an alterna¬
tive treatment. The authors objective is to evaluate uterine artery
embolization as therapy for uterine myomata. Ultrasound data and
interviews of patients who underwent embolization are analyzed.
Relief of symptoms, analysis of ultrasound shrinkage 6 months
after embolization, and complications are also discussed. Results
confirm earlier studies in which myomas offered an alternative to
surgery with similar results and low morbidity for treating uterine
artery embolization.
Endometrial Ablation by Microwave 95

The search for alternative endometrial ablative techniques contin¬
ues in an attempt to simplify their use and shorten operation time.
This article describes microwave endometrial ablation, a second
generation method, and outlines its development, equipment, and
technique required. It appears to be quick and easy to use. Random¬
ized controlled trials have been undertaken that indicate its efficacy
compared with endometrial resection. Although it is associated
with fewer intraoperative complications than transcervical endo¬
metrial resection (TCRE), the effects on menstrual symptoms are
not equivalent to the effects of TCRE until 12 months. Long term
results are still awaited.
Endoscopic Complications in Reproductive Medicine 107

The concept of laparoscopic complications requires knowledge of
laparoscopic anatomy: the expertise and innovative thinking of a
skilled surgeon and the ability to know when to convert an endo¬
scopic procedure to a laparotomy. A basic understanding of the
physics of energy sources and the radius of extension of thermal
tissue effect is another prerequisite for a skilled surgeon. Vascular
injury can occur with virtually any laparoscopic instrumentation
and the importance of summoning a consultant as well as knowing
what to do in the interim are of paramount importance. Knowing
one s limitations, having proper informed consent, and keeping
safety first will serve the gynecologic surgeon well.

Standard of care requires that a person, or physician, act as a
reasonably prudent person would under the particular circum

stances. To prove negligence, and hence malpractice, evidence must
support failure to conduct adequate evaluation, careless execution
of the medical or surgical procedure, inadequate monitoring of the
patient or unethical conduct. Before embarking on a procedure,
informed consent must be obtained and properly documented. The
basic tenets for providing quality care, meeting the appropriate
standard, and avoidance of negligence are the objectives of this ar¬
ticle.
