The past century has witnessed a change in the causes of eye injury. The
workplace has been replaced by the home and street as the preponderant
location for eye injury. The injuries are every bit as destructive, and, fortu¬
nately, the ophthalmologists ability to handle them has approved dramati¬
cally. The small group of ophthalmologists dedicated to injury prevention
have produced an important legacy over the past 100 years. This article
examines the history of eye injury prevention and provides recommenda¬
tions for the future of eye protection. The next 100 years hopefully will see
the elimination of unnecessary vision loss.

Safety eyewear should protect the user. The process of assuring protection
involves standards to specify performance, testing by reliable laboratories,
and final certification of the protector by a certification council. Essentially,
all sports eye injuries are preventable.
The Hockey Eye Safety Program 359

In Canada, very accurate statistics have been kept regarding the number
and severity of eye injuries in ice hockey since 1972. Compulsory full face
masks were introduced into organized minor hockey in Canada in the
late 1970s. Since that time, there has been a dramatic, sudden, and persis¬
tent decline in the number of eye injuries. No eye injuries have ever been
reported by players wearing full face protection. Much education of the
public is still required to ensure that all players, in all age groups, wear
adequate, approved eye protection. Eye injuries in hockey will then be
eradicated.

This article reviews reports and mechanisms of injury in racquet sports and
suggests that almost all eye injuries in racquet sports are preventable. Safety
equipment, materials, and standards are discussed.

This article provides an overview of impact protective eyewear products
available in the safety and sports market. The focus is on ophthalmic lenses,
frames, materials, treatments, and processes currently used for safety and
sports for adults and children. Explanation of compliance to current safety
and sports standards is provided. A product selection protocol is described
to prescribe and select protective eyewear for various conditions, and current
products are explained and evaluated.

Sports and recreational related eye injuries are a major cause of serious eye
injuries in the United States. To prevent these potential disasters, athletes
should wear appropriate, well fitted eye or face protectors. Many athletes,
parents, and coaches are reluctant to follow recommendations to wear such
protectors. The American Society for Testing and Materials (ASTM) has
developed standards for many sports that carry a relatively high eye injury
risk. No blinding injury has ever occurred when the protector has been
certified to the ASTM standard.

A certain number of severe eye injuries in children always will occur on a
random basis that are both unpredictable and unpreventable. The majority
of eye injuries in children and adolescents, however, are both predictable
and preventable. Sports are the major cause of eye injuries in the school
age population. Ophthalmology has played an important role in the develop¬
ment of regulations mandating the use of protective face and eye equipment.
This article looks at the special considerations for eye injuries in children
and discusses the importance of eye protection in children with good vision
in only one eye.
The Development of Eye Armor for the American Infantryman 421

The infantry are the soldiers who suffer by far the preponderance of injuries
in war. This article is an account of the development of eye protection for
the American infantryman, a 20 year effort that culminated in the production
and distribution of eye armor for the emmetrope and ametrope, protective
against small missiles, blunt force threat, and some of the eye threatening
laser wavelengths.
Management of Orbital and Ocular Adnexal Trauma 435

Ocular adnexal trauma may involve the eyelids, orbit, or lacrimal system.
Management of complex orbital and ocular adnexal trauma should involve
a multidisciplinary approach. Life and sight threatening injuries must be
recognized and treated appropriately. Adherence to the fundamentals of
trauma management, along with a stepwise, systematic approach to ocular
and adnexal examination, reduces the likelihood that serious injuries will
be missed. After all injuries have been identified, they should be repaired
appropriately to optimize both functional and cosmetic outcomes.

There have been a variety of new developments in the medical and surgical
management of the injured eye that have greatly improved the success of
visual rehabilitation of patients with ocular trauma. Ocular injury, however,
remains a major cause of visual morbidity, especially in the young adult.
Regardless of the progress in treatment of trauma, the most effective inter¬
vention remains prevention. It is imperative, therefore, that each practitioner
educate his or her patients in appropriate protective eyewear and actively
participate in his or her community to reduce the incidence of these poten¬
tially devastating injuries.
Current Concepts in the Treatment of Traumatic Injury to the
Posterior Segment 465

Posterior segment traumatic injuries, although quantifiable and predictable,
can manifest in a variety of ways. The involvement of some or all of the
intraocular tissues makes these trauma cases diagnostically and surgically
challenging. An understanding of the mechanism causing the injury and

the basic principles involved in diagnosis and therapy improves the manage¬
ment of these cases. This article discusses an overview of the mechanisms
of posterior injury and describes an approach to the management of posterior
segment injuries caused by closed and open globe trauma.
