New Insights into the Molecular Basis of Glucocorticoid

Understanding the molecular mechanisms of glucocorticoid ac¬
tion is a primary step in the development of more effective
therapeutic agents. An important part of the anti inflammatory
effects of glucocorticoids in asthma involves repression of the
expression of inflammatory mediators, particularly cytokines. The
NFkB and AP 1 transcription factors seem to be involved in the
expression of a number of crucial target genes in asthma, and
there are several possible mechanisms through which glucocorti¬
coids may repress genes regulated by NFkB and AP 1. Recent
studies also have demonstrated a significant role for steroid
nuclear receptor coactivators, histone acetylation, and chromatin
structure in gene regulation. These newly described mechanisms
of gene regulation are discussed in relation to the molecular basis
of glucocorticoid action.

Glucocorticoids are powerful anti inflammatory agents used for
the treatment of many diseases, including allergic inflammation
and asthma. This end effect is a result of the summation of a
diverse range of actions of glucocorticoids not only on inflamma¬
tory cells but also on those cells that make up the stroma of
inflamed tissues. This article comprises an account of the effects

of glucocorticoids on individual leukocytes involved in the in¬
flammatory response, followed by an overview of how these

Corticosteroids are the most effective drugs used in the treatment
of asthma. The purpose of this article is to examine the effects
of corticosteroids on the histopathology of asthma, such as the
structural changes of the airways and their cellular elements (e.g.,
inflammatory cells and mediators).

Glucocorticoids, administered topically or systemically, form the
cornerstone of asthma therapy. Delivered systemically, they serve
as a major component of rescue therapy for acute asthma exacer¬
bation. They also serve as chronic supplemental therapy in those
patients with severe persistent asthma. Inhaled glucocorticoids
are now the preferred controller medication for all but the pa¬
tients with the mildest asthma. This article provides a broad
overview of the structure, pharmacokinetics, and pharmacody
namics of systemically administered and inhaled glucocorticoids.

This article reviews the pertinent pharmacology of the inhaled
glucocorticoids that is needed to compare their efficacy and safety.
Significant differences in potency and pharmacokinetics exist be¬
tween the inhaled glucocorticoids that could produce clinically
significant differences in safety and efficacy. Until now, most
comparative clinical trials have established potency differences,
but have been inadequate to establish comparative therapeutic
indexes between products.
Role of Systemic and Inhaled Glucocorticoids in
Asthma 739

Corticosteroids are the preferred anti inflammatory drugs for
long term control of persistent asthma. Alone or in combination
with long acting bronchodilators, they are indicated for primary
treatment of mild, moderate, and severe persistent asthma. They
hasten the resolutions of asthma exacerbation, reducing morbid

ity, hospitalization, and death. They also reduce the long term
accelerated loss of pulmonary function and development of fixed
airflow obstruction caused by airway remodeling. Their effective¬
ness depends on their early introduction into the treatment of
asthma.

This article summarizes important clinical studies relevant for
assessment of the efficacy and safety of inhaled steroid therapy
in children. In addition, advice on how to optimize therapy with
various inhaled corticosteroids is provided.
Corticosteroids in the Treatment of Allergic Rhinitis 783

The manipulation of the chemical structure of corticosteroids to
enable the development of potent topically active therapeutic
agents while minimizing systemic bioavailability has revolution¬
ized the pharmacotherapy of allergic rhinitis. The multiplicity of
anti inflammatory effects on the nasal mucosa when used topi¬
cally account for their high therapeutic benefit and risk profile
and frequent success when used without need of other therapy.
Questions concerning the safety, especially in children, of long
term use of intranasal corticosteroids, however, underscore the
importance of assessing benefit and risk for the use of intranasal
steroids along with other therapeutic options in each patient.
Corticosteroids in the Treatment of Sinusitis and Nasal
Polyps 799

This article reviews the underlying pafhogenesis of acute and
chronic sinusitis and the rationale for use of systemic and topical
corticosteroids for treatment of sinusitis exacerbations. The immu
nopathology of chronic hyperplastic sinusitis with nasal polyposis
and its response to topical corticosteroids are discussed at the
level of inflammatory cytokines and adhesion molecules. A sum¬
mary of published studies regarding the use of topical corticoste¬
roids for treatment of nasal polyp diseases and prevention of
nasal polyp recurrence after sinus surgery also are reviewed.

the potential of controlling asthma without the side effects consis

efficacious topical dose with minimal systemic absorption. Topics
discussed include hypothalamic pituitary adrenal axis suppres¬
sion, growth, osteoporosis, ocular complications, and dermal thin¬

Corticosteroid insensitive Asthma 837

Corticosteroids are highly effective anti inflammatory drugs in
the treatment of asthma; however, a subset of patients with
asthma fails to experience clinical improvement when treated
with corticosteroids. This phenomenon has been referred to as
corticosteroid insensitive asthma. The purpose of this article is to
review our current understanding of the molecular and cellular
mechanisms underlying corticosteroid insensitive asthma. Under¬
standing the mechanisms that give rise to corticosteroid insensi
tivity has important clinical implications for the management of
severe asthma and other allergic disorders characterized by tissue
inflammation.
Alternative Anti inflammatory and Immunomodulator
Medications for Asthma 855

There have been many proposed alternative therapies for persis¬
tent mild to severe asthma. The standard of care as outlined in the
1997 Expert Panel Report 2 is the use of inhaled corticosteroids in
doses adjusted to disease severity. Although many of the alterna¬
tives studied have yielded useful information in our understand¬
ing of asthma disease mechanism, none have demonstrated evi¬
dence that would alter the Expert Panel s conclusions supporting
inhaled corticosteroids in first line position.
