Facial growth is a complex, multifactorial phenomenon with both genetic
and environmental influences. Since the nose is the last facial feature to
fully develop and the anterior septum is an area of great activity after
puberty, surgery on the pediatric or adolescent nose traditionally had been
avoided. However, recent clinical experience has suggested there are expec¬
tations to this doctrine and, in fact, surgical intervention for pediatric nasal
obstruction may be indicated in many cases. Depending on the severity of
the obstruction and age of the patient, nasal obstruction can present as a
life threatening emergency, interfere with normal facial growth, or manifest
as quality of life issues. Several clinical situations are discussed.
Work Up and Evaluation of Patients with Nasal Obstruction 263

Nasal obstruction is a common symptom with a multifactorial origin. Di¬
rected, complete work up results in appropriate diagnosis and formulation
of a treatment plan. Most patients require a complete history, rhinologic
examination, and edoscopic examination. Radiologic work up, cultures, bi¬
opsy, rhinomanometry, and acoustic rhinometry are useful in some cases.
Nasal tissue stiffness measurements may play a role in the future.

Perforations of the nasal septum are an uncommon, yet challenging clinical
entity for the facial plastic and reconstructive surgeon. This article describes
the essential anatomy, possible causes, and reasons for repair of septal
perforations. In addition, both the nonsurgical and the surgical treatment
options are discussed as well as the senior author s preferred surgical tech¬
nique. Finally, a number of new biotechnologies that may have future appli¬
cation in septal perforation repair are reviewed.
The Saddle Nose Deformity 303

The saddle nose deformity, or pug nose, refers to the concavity or depression
of the nasal dorsum, which results in a saddle like appearance when viewed
from the profile. It also describes a wide spectrum of dorsal depressions,
the treatment of which relies upon the accurate preoperative explanation
and assessment. It is important for the facial plastic surgeon to address
both the functional aspects of nasal valve collapse or stenosis, which often
accompany these deformities, as well as the aesthetic appearance of the
external nose. A thorough assessment of the severity of deformity guides
the surgeon to reconstruct the saddle nose with an aesthetically pleasing
and functional postoperative result.
Controversies in Turbinate Surgery 311

The treatment of nasal obstruction involves careful evaluation followed by
judicious medical and surgical management. Surgery has centered around
septal and occasional rhinoplastic reconfiguration with the rurbinates histor¬
ically relegated to a minor role. Experience with over 3000 turbinectomies
over 13 years redefines surgical nasal obstruction management indicating
that conservative volume reduction of the inferior turbinates should indeed
become a regular component of difficult nasal airway treatment. Meticulous
intraoperative attention to technique and hemostasis minimizes periopera
tive morbidity and justifies the turbinate volume reduction as a safe, surgical
adjunct to achieving an enhanced bisymmetrical nasal airway.
Surgical Correction of the Deviated Septum 319

The nasal septum forms the foundation upon which the external nasal
osseocartilagenous framework is built. Adequate surgical correction of a
significant septal deviation is required to optimize both the functional nasal
airway as well as the final aesthetic result that may be achieved in rhino
plasty. This article reviews the relevant surgical anatomy and origin of
septal deviation as well as the various techniques that are available to the
facial plastic surgeon to safely correct any deviation that may be present.

Nasal obstruction is a common problem encountered by the otolaryngolo
gist, and there are many different causes associated with it. It remains
important to consider the physiologic and reversible effects, such as the
nasal cycle and vasomotor rhinitis as well as less common problems includ¬
ing foreign bodies and primary nasal tumors. By focusing on the morphol¬
ogy of the septum or searching for intranasal polyposis, the contributions of
the nasal sidewall are easily overlooked. This article discusses preoperative
evaluations and several surgical techniques.

There can be many different components contributing to the symptoms of
nasal obstruction. While the internal nasal valve has been the topic of exten¬
sive discussion in the literature, the external nasal valve has received little
attention. This article describes anatomy, aesthetics, physiology, and patho
physiology of the external nasal valve. In addition, a variety of treatment
options that may be used to address external nasal valve dysfunction are re¬
viewed.

The practice of contemporary rhinology requires mastery of ethmoidectomy,
septoplasty, and rhinoplasty. Although rhinoplasty specifically implies aes¬
thetic nasal surgery, the term septorhinoplasty connotes a more functional
basis for nasal surgery based on septal reconstruction resulting in airway
as well as esthetic improvement. This article familiarizes the otolaryngolo
gist and facial plastic surgeon with concepts for patient selection; techniques
for safe, sequential effective endorhinoplasty; potential pitfalls, complica¬
tions, and anticipated results of the combination of rhinoplasty, endoscopic
ethmoidectomy, and septal surgery.
Snoring, Sleep Apnea, and the Facial Plastic Surgery Patient 373

Patients with snoring and sleep apnea may potentially present to the facial
plastic surgeon with obstructive complaints related to septo nasal deformi¬
ties. It is important to undersatnd the diagnosis, work up, and treatment
of obstructive sleep apnea for patients who present with these symptoms.
The correction of a functional nasal obstruction may relieve complaints of
snoring, however, additional evaluation may be required for those patients
who truly have sleep apne.
Nasal Airway Devices From Ancient History to the Present 379

This article discusses the history of nasal devices, from devices of ancients
and biblical times to devices of the twentieth century. Aspects of the physics
and physiology are discussed as well as clinical studies evaluating their
usefulness in exercise, infants, snoring, and apnea. Drawings of ancient
nasal devices and photographs of modern nasal devices are included for
comparison. From the nasal bougies, nasal vibrators, and nasal insufflators
of the past to modern internal and external nasal dilators, mechanical devices
have been devised to improve the nasal airway. These offer an alternative
for patients who are not interested in nor are they candidates for surgery.
Nasal Airway Problems in the Cleft Lip Population 391

The unilateral cleft nasal deformity is a complex malformation affecting
both the soft tissue and skeletal components of the nose. A good functional
outcome requires accurate diagnosis and repair of each of the obstructing
components. This article discusses the origin of nasal obstruction in the cleft
lip population and describes specific surgical techniques used to correct
these abnormalities.
M Plasty Correction of Nasal Valve Obstruction 405

Nasal obstruction resulting from an abnormality of the nasal valve can be
a complaint both before and after rhinoplasty. This article describes the
anatomy of the nasal valve and emphasizes its importance in the preopera
tive evaluation of all rhinoplasty patients. The M plasty surgical approach
and correction of the compromised nasal valve is described. The indications
and contraindications for M plasty are discussed.
The Percutaneous Columellar Strut 411

The percutaneous columellar strut is a technique to provide additional
support to the tip after septoplasty. It may be useful in preventing long
term tip ptosis when septal injury has reduced the contribution of the septum
to tip support. It has the advantage of ease and simplicity of insertion. The
columellar strut is another technique in the armentarium of the nasal
surgeon.
