for an adult or pediatric patient, one must fully understand the
etiology and biomechanic, extrinsic, and intrinsic factors that
produce the deformity. This article discusses symptomatic pes
planus valgus; the subtalar and midtarsal joints; the hips, tibia,
and posterior tibial tendon; subtalar clinical and surgical con¬
cerns; posterior tibial tendon repair or advancement and augmen¬
tation; calcaneal slide osteotomies; and triceps equinus.

Chronic rupture of the posterior tibial tendon is a common disor¬
der causing a progressive flatfoot deformity. MR imaging is an

excellent modality for assessing dysfunction of the tendon. It can
easily and accurately detect and grade the tears. A variety of
secondary soft tissue and bony abnormalities are associated with
the tear and can be useful in confirming the diagnosis.

A well respected orthopedic surgeon once offered an impressive
definition of a normal foot. Although it is not comprehensive, it
does establish a basic criterion, a benchmark from which one can
measure, observe, and compare a normal foot with one that
deviates from the norm in progressive stages: A normal foot is
a balanced foot that is asymptomatic. In consideration of this
definition, this article presents the imbalanced stages of PTTD,
resulting deformity, and conservative therapeutic management.
Treatment of Posterior Tibial Tendon Dysfunction with
Tendon Procedures from the Posterior Muscle Group 453

The treatment of patients with PTTD remains driven by the
preference of the surgeon with little science to guide him or her.
Tendon procedures, such as tendon decompression, tenodesis,
and tendon tranfers of the posterior muscle group, either alone
or in combination with other procedures, are popular treatment
options. Controversy remains regarding procedure selection and
the specific techniques of each procedure. Much study is needed
to identify the best technique for each stage of the deformity. The
authors review the tendinous procedures currently used for the
treatment of PTTD.

joint disease, has proved to be a challenging condition for surgical

Subtalar joint arthroereisis procedures have been used success¬
fully in the treatment of the flexible pediatric flatfoot; however,

the literature is lacking regarding their use in the surgical correc¬
tion of the flexible adult flatfoot. This article presents a case
involving the Maxwell Brancheau Arthoereisis Implant, among
other procedures, performed in the surgical reconstruction of a
severe adult flatfoot deformity with PTTD after an unsuccessful
attempt with the Kidner procedure.
Calcaneal Osteotomy for the Treatment of Posterior

PTTD is a multiplanar deformity with significant impact on me¬
chanical function. This article discusses the use of the calcaneal
osteotomy as an important, relatively easy adjunct in the surgical
correction of the deformity. Historical reviews of calcaneal osteot¬
omies and proper procedural techniques are presented.
Talonavicular Arthrodesis for Correction of Posterior
Tibial Tendon Dysfunction 501

The main contributing factor to PTTD is the presence of prona
tion, which places excessive strain on the medial soft tissues of
the foot and ankle. In this article, the authors review the diagnosis
and treatment considerations for PTTD, and discuss the proce¬
dure, postoperative care, and complications for the talonavicular
arthrodesis. The talonavicular arthrodesis was performed in 29
cases with few complications. In this article, the authors present
two illustrative case studies of this procedure. The procedure is
indicated in the symptomatic flexible flatfoot deformity that does
not have degenerative changes of the subtalar joint and has not
been responsive to conservative therapy. The talonavicular
arthrodesis is effective for correcting the deformity because it
reduces forefoot abduction, increases the height of the arch, stabi¬
lizes the medial column, and prevents excessive subtalar joint
pronation.

Triple arthrodeses and isolated subtalar joint arthrodeses have
been advocated for treatment of end stage arthritic deformities.
Recently, the literature has reviewed the efficacy of these proce¬
dures for the treatment of end stage complications of PTTD. This
article reviews the indications for such procedures, surgical tech

mity secondary to PTTD.

Posterior Tibial Tendon Problems in Athletes 557

Posterior tibial tendon problems are uncommon sources of dis¬
ability in athletes. Awareness of the condition and early diagnosis
are important to prevent disability and prolonged absence from
sports activities. Appropriate imaging studies aid diagnosis. Ten¬
don tears in younger patients are different from the typical rup¬
tures seen in middle aged and older adults. Treatment must be
individualized based on the type of tear and degree of deformity
present. Other conditions, such as dislocation of the posterior
tibial tendon and accessory navicular and exertional compartment
syndrome, can involve the tibialis posterior muscle and present
as diagnostic dilemmas.
