A thorough and accurate understanding of the gross and arthroscopic
anatomy of the wrist and distal radioulnar joints is a prerequisite for
effective arthroscopy. This article describes the surface anatomy of the
radiocarpal, midcarpal, and distal radioulnar joints, and provides a de¬
tailed review of the gross and arthroscopic features of the ligaments of the
wrist and distal radioulnar joints. The article is written in a manner that
allows the arthroscopist to perform a comprehensive survey of the entire
wrist and distal radioulnar joint in a systematic fashion, with correlation
to the extra articular gross anatomy of those structures observed within
the joint.

Chronic wrist pain may be evaluated by physical examination, imaging,
and arthroscopy. Arthroscopy provides an attractive diagnostic and thera¬
peutic modality in selected patients. Currently the correlation between
arthroscopic findings and symptomatic pathology is not clear, because
arthroscopy often identifies asymptomatic pathology. Nevertheless, the
ability to diagnose and treat intra articular pathology simultaneously pro¬
vides a substantial advantage over radiographic analysis.

Cartilaginous Lesions of the Wrist 423

Articular cartilage lesions in the wrist are common. Diagnosis of these
lesions is difficult, and the decision to operate may not be clear. A meticu¬
lous history and physical examination allows an accurate diagnosis. Ap¬
propriate surgical intervention can be useful in alleviating symptoms and
returning the patient to normal activities. The origin, diagnosis, and treat¬
ment of cartilaginous lesions of the wrist are discussed in this article.

Arthroscopic Resection of the Dorsal Ganglia of the Wrist 429

Ganglia are the most common soft tissue tumors seen and treated in the
hand. Arthroscopic ganglion resection provides a means by which dorsal
wrist ganglia may be safely resected while avoiding the requisite scar
accompanying open resection. Use of the arthroscope provides a much
more complete examination of the wrist, allowing assessment of the cause
of the ganglion and associated intra articular problems. This article dis¬
cusses the symptoms and signs, origin, and operative techniques for deal¬
ing with dorsal wrist ganglia.

Wrist arthroscopy is an available tool for the diagnosis and treatment of
scapholunate instability. Arthroscopy allows a detailed examination of the
scapholunate interosseous and the extrinsic radiocarpal ligaments. The
extent of ligamentous damage can be quantified and a treatment algorithm
can be developed based on the arthroscopic findings. Debridement of
partial scapholunate interosseous tears and arthroscopic reduction and
internal fixation are discussed and illustrated.

The treatment of lunotriquetral interosseous (LTIO) ligament injuries is
controversial. It is part of a spectrum of injuries involving the ulnar side
of the wrist. Associated injuries may play a greater role in the outcome of
treatment than does the LTIO injury. Arthroscopy is the most accurate
diagnostic tool available for the evaluation of LTIO ligament injuries; it
allows diagnosis and often treatment of associated injuries. This article
describes the current approach to the diagnosis and treatment of LTIO
ligament injuries and related use of arthroscopy.
Arthroscopic Management of Intra Articular Distal Radius

Wrist arthroscopy is increasingly becoming recognized as an important
adjunct in the management of displaced intra articular fractures of the
distal radius. Anatomic restoration of the articular surface is possible
under bright, magnified conditions. Osteochondral loose bodies and asso¬
ciated intracarpal soft tissue injuries that may not be apparent on plain
radiographs are detected and simultaneously managed. A method of man¬
agement for displaced distal radius fractures using wrist arthroscopy and
a combination of percutaneous and limited open reduction techniques are
described.

Nondisplaced fractures of the scaphoid are traditionally treated with cast
immobilization, but athletes and patients with high demands on their
dexterity and time find this an unsuitable treatment option. Percutaneous

and arthroscopic screw fixation of the scaphoid provides an alternative
treatment method that minimizes soft tissue trauma, provides stable fixa¬
tion, and reduces rehabilitation time. This percutaneous technique also
offers the advantage of not requiring placement of a jig. This article
discusses treatment options for acute nondisplaced scaphoid fractures
focusing on percutaneous screw fixation.
Arthroscopy of the Distal Radioulnar Joint 475

Arthroscopy of the distal radioulnar joint is a natural extension of radiocar
pal arthroscopy and can be a useful tool for certain conditions that affect
this small joint. Arthroscopy provides a minimally invasive means of
evaluating the distal radioulnar joint and enables visualization of the
articular surfaces, synovium, and undersurface of the triangular fibrocarti
lage. In this article, indications, surgical techniques, and potential pitfalls
of this technically demanding procedure are presented.

The triangular fibrocartilage complex is an intricate anatomic structure
located at the ulnar aspect of the wrist. The triangular fibrocartilage is
important to the stability and biomedical function of the ulnar carpus and
distal radioulnar joint. This article reviews the anatomy and biomedical
function of the triangular fibrocartilage. Diagnosis and treatment of trau¬
matic injuries to the triangular fibrocartilage are also discussed.

The use of arthroscopy to treat wrist arthritis is a relatively new approach
to a common problem. Recent advances in arthroscopic surgery techniques
and instrumentation have enabled the surgeon to improve the treatment
of wrist pathology. Patients undergoing arthroscopic procedures poten¬
tially have better motion postoperatively because the dorsal carpal liga¬
ments are preserved. This article reviews the techniques of arthroscopic
proximal row carpectomy, radial styloidectomy, and wrist joint synovec
tomy.

Lasers are used by many specialties, such as ophthalmology, urology,
orthopedics, hand surgery, and otolaryngology, and the cost, therefore, is
spread over many cost centers. Their delivery systems (laser probes) are
very small and perfectly suited for use in small joints. The combination of
small size and efficient tissue ablation has been sufficient to move the
laser into the mainstream of small joint arthroscopy. This article provides
an overview of the use of lasers in wrist arthroscopy.

Arthroscopic percutaneous treatment of disorders of the metacarpopha¬
langeal (MCP) joints of the hand permits direct joint inspection and a
better understanding of joint pathology than open techniques. Understand¬
ing the pathology and knowing its exact location permits percutaneous

treatment of joint disorders. This article reviews the anatomy, instruments,
techniques, and indications for arthroscopy of the MCP joints of the hand.
Specific arthroscopic techniques are illustrated. Case examples include
repair of ulnar collateral ligament tears of the thumb, percutaneous frac¬
ture reduction and fixation, synovectomy, correction of malunions, de
bridement of osteochondral lesions, and removal of free bodies.

Wrist arthroscopy has become an essential tool in the evaluation and
treatment of a variety of wrist injuries and disease processes. As such, an
understanding of complications related to wrist arthroscopy is critical to
overall outcome. Complications are based on the unique anatomy of the
wrist, portal locations, surgical technique, and type of procedure per¬
formed. This article describes techniques that may decrease the likelihood
of each of these complications.
