Disease state management programs were launched in recent years in an attempt to better manage
the health of target patient populations in a cost effective manner. In heart failure, outpatient disease
state management programs have demonstrated improved patient outcomes and robust savings
without compromise in quality patient care. The disease state management concept could be incorpo¬
rated along the continuum of care to promote optimization of heart failure medical therapies that
increase survival and quality of life, decrease hospitalized patient length of stay, prevent hospital
admissions, and significantly reduce health care expenditures fc r this costly medical ci nditii n. Aggres¬
sive, multidisciplinary programs that use pharmacologic and nonpharmacologic therapeutic guidelines,
and expert nurse clinicians to care for clients, may provide the best hope of manipulating current
morbidity and mortality statistics.

Interventional cardiology procedures have skyrocketed in number since the first percutaneous translu
minal coronary angioplasty (PTCA) was performed in Zurich in 1977. The growth seen in this population
makes it an ideal model for incorporating principles of outcome management into current nursing prac¬
tice. This article discusses how the clinical care of the interventional cardiology patient in a large New
York State hospital has changed using evidence based outcome management. Selected topics discussed

In today s health care environment, the goals of providing high quality care and achieving positive
patient care outcomes are determined with an eye on the financial picture. The process of creating
a clinical pathway for carotid endarterectomy allowed us to examine our care and determine an
evidence based practice for this population. Significant changes in our practice resulted in improved
patient satisfaction, quality patient care outcomes, decreased length of stay, and decreased costs.

Severe head injury is a major cause of disability, death, and economic cost. Significant reductions in
mortality and morbidity can be achieved through the use of evidence based protocols and guidelines.
Improved outcomes following severe head injuries are seen with complete, rapid physiologic resuscita¬
tion, and specific strategies that decrease intracranial pressure.

This article discusses the immunological role of the gastrointestinal tract and explains how critical
illness or injury can adversely impact this system. Through a review of the research literature, early
enteral nutrition is identified as a key factor in maintaining gastrointestinal integrity, thus preventing
adverse effects from gastrointestinal dysfunction.

Acute respiratory distress syndrome (ARDS) is a complex clinical syndrome characterized by noncardio
genic pulmonary edema and damage to the alveolar capillary membrane. The complexity of the
mediator cascades, fluid shifts, and inflammatory process associated with ARDS requires an in depth
understanding of the pathophysiology to determine management strategies. This article addresses
the pathophysiology, integration of assessment findings, and management strategies for the patient
with ARDS.
Successful Outcomes for the Ventilator Dependent Patient 249

In a health care environment where care is increasingly based on outcomes and focused on resource
utilization, the ongoing ventilator dependent population creates a challenge to the system. This article
explores the characteristics of patients who require mechanical ventilation beyond the critical care
unit and presents strategies to manage their plan of care for successful outcomes. Since the causes
of ventilator dependence are so varied, a customized plan of care is necessary for each individual,
with close attention paid to the setting of realistic goals in collaboration with the multidisciplinary
patient care team. This article also reviews a case study and includes sample teaching documents.
Developing an Emergency Department Team for Treatment of
Stroke with Recombinant Tissue Plasminogen Activator 261

A sample patient suffers an ischemic stroke and receives rt PA treatment at a medical center equipped
with a stroke team. With the advent of approved thrombolytic therapy in the form of rt PA, many of
the over 500,000 people who suffer a stroke annually in the United States will be treated with this
agent in the emergency department. In this article, the author discusses the National Institute of
Neurological Disorders and Stroke (NIDS) rt PA Stroke Trial, which provides a blueprint for the
formulation of an in house organization to cope with the critical three hour window for successful
treatment. An account is also given of the cooperation required between the stroke team and the
EMS, ED, Neurology, Radiology, Nursing, Laboratory, and Pharmacy departments.

The Effect of the Clinical Nurse Specialist on
Patient Outcomes 269

There is increasing pressure for clinical nurse specialists (CNSs) to justify their role in terms of
improving quality of care as evidenced by positive patient outcomes. The literature review shows a
beginning body of evidence to support the positive effects of CNS on patient outcomes. Researchers
have shown that CNSs decrease patients length of stay (LOS), complications, health care use, health
care cost, and mortality rates. This article also reports the result of the author s research comparing
LOS and number of complications in patients cared for on units with clinical nurse specialists and
on units without them.
Implementation of a Nurse Practitioner Role in an
Acute Care Setting 277

Advanced registered nurse practitioners (ARNP) are becoming a mainstay in primary and acute care
settings because of the changes in the health care environment. Acute care hospitals are recognizing
the need to provide expert clinical care in a holistic manner, which requires communication and
recognition of the patient and patient s needs as central to the care delivered. Patients with neurological
acute care needs or injuries are often at high risk for fragmented care due to their complexity and
rehabilitation needs. The goal of this demonstration project was to develop a more proactive, patient
focused care delivery model. Outcome data assessed included: influence of the ARNP on the discharge
process, patient satisfaction, management structure, and enhancement of critical thinking by staff.

Powerful economic forces are driving systems of higher education to implement change. Bducational
institutions need to borrow from health care and develop an oulcomes based educational delivery
system. Many states have already developed performance indicators for education and are tying them
into funding. The most important performance indicators answer the question of whether or not
students learned. This critical care nursing facility describes the evidence they use to answer yes.
