Intrapartum Fetal Assessment: A Review 549

Some of the difficulty in assessing the clinical value of intrapar¬
tum fetal heart rate (FHR) monitoring stems from lack of unifor¬
mity in pattern interpretation and, therefore, in response to per¬
ceived FHR abnormalities. This article reviews the physiology
and pathophysiology of FHR changes, stresses the new NICHD
guidelines for unambiguously defining FHR patterns, emphasizes
the avoidance of circumstances stressful to the fetus, and suggests
appropriate treatment for the distressed fetus. The strong likeli¬
hood that such treatment will be successful in reversing abnormal
FHR patterns, as well as the importance of preparing for expedi¬
ent delivery should such treatment fail, are noted. Finally, some
of the newer tools for judging the timing of perinatal brain injury
are described.
Intrauterine Resuscitation of the Compromised Fetus 569

The gold standard for assessment of fetal well being in labor is
the adequacy of fetal oxygenation using continuous electronic
fetal monitoring. Additional biophysical properties of the fetus
are increasingly being used to ascertain the health of the intrauter¬
ine environment. This article focuses on the detection and man¬
agement of the compromised fetus. The article describes the
history, physiologic basis, and current techniques for fetal moni¬
toring. In addition, it presents the common diagnostic tools used
to identify the compromised fetus and available options for the
management of fetal intolerance of labor.

Alveolar Fluid Clearance in Developing Lungs and its
Role in Neonatal Transition 585

This article reviews the process of alveolar fluid clearance at
birth and its relevance to successful transition from placental to
pulmonary gas exchange. Although mechanical factors contribute
to the fluid shift from developing air spaces, the bulk of fluid
movement is secondary to transepithelial sodium reabsorption
through amiloride sensitive sodium channels. Disruptions of this
process can lead to retention of fluid in the air spaces resulting
in wet lungs and may play a role in the pathogenesis of dis¬
eases like hyaline membrane disease and transient tachypnea of
the newborn. The key factors that stimulate lung fluid clearance
in the perinatal period are also discussed.
Pulmonary Vascular Biology During Neonatal Transition 601

Although the normal pulmonary vascular transition at birth takes
place quickly in the delivery room, it has its basis in the complex
structural and biochemical development of the lung. It is not
surprising that this process can be easily disrupted by factors
such as prematurity, intrauterine hypoxia, and parenchymal lung
disease. This article reviews the structural and functional devel¬
opment of the pulmonary vasculature during fetal life. It also
describes the multiple vasoactive substances that modulate pul¬
monary vascular tone in the fetus and initiate and maintain the
pulmonary vascular transition at birth.
Physiologic Reflexes and Their Impact on Resuscitation
of the Newborn 621

Most depressed newborn infants respond promptly to minimal
intervention. When that does not happen, a thorough understand¬
ing of the physiologic reflexes that promote and prevent, prompt
restoration of heart rate, blood pressure, and respiration is crucial.
This article will review data demonstrating that lung inflation, in
the purely mechanical sense, is a key element in a successful
resuscitation. A complicated resuscitation is most often because
of the caretaker s lack of awareness of basic cardiopulmonary
physiology than to an unusually depressed infant.
History of Neonatal Resuscitation: Tales of Heroism and
Desperation 629

Although the history of neonatal resuscitation is as old as medi¬
cine itself, today s standards of practice evolved over the past 40

years. Most ancient physicians and midwives did know that
stimulation and expansion of lungs was needed to revive the
apparently dead newborn, but the means of providing these
therapies varied from brutal shaking, hitting, swinging, electro¬
cuting, hanging upside down to applying gentle pressures or
squeezing of the chest. It would take centuries of development in
physiological concepts and technology for the evolution of a
rational approach in resuscitating the newborn infant. Even after
great advances in medical science in the 19th century, cardiopul
monary resuscitation techniques remained primitive until the
mid 1950s. In this article the author has traced some elements
of cardiopulmonary resuscitation techniques and developed an
outline of the history of neonatal resuscitation.
Delivery Room Resuscitation of the Newborn 641

This article reviews the complex cardiopulmonary changes that
normally occur at birth and what elements can go wrong. The
steps that must be taken by delivery room personnel to resuscitate
both preterm and term neonates is also discussed, including the
use of positive pressure ventilation, chest compressions, and med¬
ications. Also, new research questioning several of our routine
resuscitative practices will be examined in detail, including the
use of 100% oxygen, the deleterious effects of over ventilation,
the optimal timing for surfactant administration, and the need for
intubation and suctioning of all infants when delivered through
meconium stained amniotic fluid.

Throughout most of medical history, meconium stained amniotic
fluid has been known to be associated with adverse neonatal
outcomes. Many therapies have been suggested as having benefit
in preventing the meconium aspiration syndrome. The history of
meconium stained amniotic fluid management and the current
status of knowledge concerning the effect of various therapies on
the prevention of meconium aspiration syndrome are reviewed.

With the availability of more sophisticated techniques for evaluat¬
ing the premature neonate comes a parallel responsibility to care¬
fully asses the process of resuscitation and assisted ventilation.
The most fragile patients whom delivery room personnel are
called to assist are the very low birthweight preterm neonates.

Their lung volumes are small, their chest walls are compliant, and
their cerebral blood flow is low compared with term neonates. In
this setting, everything that is done to the neonate must be
performed with extreme caution and great care. This article dis¬
cusses the causes of iatrogenic injury and suggests how such
injury might be avoided.

This article reviews epinephrine and sodium bicarbonate, the two
medications most often recommended in neonatal resuscitation,
the controversies concerning their use, and the rationale for the
recommendations in the context of neonatal resuscitation. In addi¬
tion, it reviews atropine, a medication still commonly used in the
resuscitation of adults and older children, in the context of its
value in neonatal resuscitation.

Respiratory problems in the newborn period are a diagnostic and
therapeutic challenge. The goal of expeditious airway establish¬
ment may not be straightforward. Familiarity with features of the
normal and abnormal infant airway anatomy, physiology, as well
as intubation equipment and alternatives to intubation is essen¬
tial. A systemic approach provides clues that are helpful in the
successful management of the neonate in respiratory distress.
Preserving Brain Function During Neonatal Asphyxia 733

Presently, there is no intervention that can be used that preserves
brain function in hypoxic ischemic encephalopathy. This review
covers the interventional strategies that are available to the health
care worker at present and suggests a different approach to this
frustrating problem. The proposal is to use clinically available
interventions and combine them into an effective intervention
strategy. The merits of each intervention is discussed.

Postasphyxial Management of the Newborn 749

In this article, postasphyxial management of the newborn is re¬
viewed. Emphasis is placed on the multisystem approach to com¬
plications that can occur in an infant who has sustained signifi¬
cant hypoxia and ischemia. The frequently involved organs are
reviewed individually with respect to specific complications that
arise secondarily to the initial injury. Depending on the severity
of damage to the organ, therapeutic interventions are frequently
required; however, medical management is often limited to sup¬
portive measures and serial evaluations.
Long term Neurodevelopmental Outcome of
Asphyxiated Newborns 767

Perinatal asphyxia continues to be a major cause of long term
morbidity in both premature and term infants. This article re¬
views some of the latest information on long term outcome in
asphyxiated infants and relates neurologic sequelae to the specific
type of hypoxic ischemic brain injury. Sequelae related more spe¬
cifically to asphyxiated premature and term infants are compared.
A practical approach to follow up is offered with a brief review
of current methods being suggested and used to predict long
term outcome.

Clinicians and parents often find themselves with difficult deci¬
sion making problems related to extremely premature birth or
fetal malformations. This article examines these issues, the role of
parents and clinicians in the decision, and a process from decision
making, with medical ethical, social, and legal perspectives. Case
examples are used to highlight certain issues. The authors also
present practical advice and conclusions for the clinician.
