Induction of labor is an everyday obstetric practice. Frequently
the cervix is unfavorable and preinduction cervical ripening is
necessary. This article reviews the current knowledge about the
physiology of cervical ripening and its assessment. Various meth¬
ods available for cervical ripening including their risks, benefits,
and limitations are reviewed.
Management of Fetal Distress 259
Stephanie Penning and Thomas J. Garite
This article presents a discussion of fetal heart rate monitoring
and its role in the laboring patient. Several origins of abnormal
fetal heart rate patterns are reviewed and examples are given.
Management alternatives are also presented and discussed. Fetal
pulse oximetry is also briefly reviewed.
Cesarean Delivery Rates in the United States: The 1990s 275
M. Kathryn Menard
One out of five births in the United States in 1996 was by cesarean
delivery. The cause for wide regional variation in cesarean deliv¬
ery rates is not well understood. The variation cannot be ex¬
plained by simple demographic risk factors and is likely due to
differences in practice style. Adjustment of rates for case mix
will lead to more valid comparisons of rates, which may unveil

opportunities for safe reduction of cesarean rates. Strategies for
reducing the number of cesarean births are described.
Active Management of Labor 287

The cesarean section rate in the United States peaked at 24.7% in
1988. The leading indications for cesarean section are now dys
tocia and prior cesarean birth. Strategies to decrease operative
delivery for dystocia range from patience and patient support in
labor to the active management of labor protocol. This review
describes the active management of labor protocol, examine its
efficacy and safety, and discuss proposed rationales as to why it
may be an effective strategy to lower rate of cesarean section
for dystocia.
Vaginal Birth After Cesarean Delivery 295

Cesarean delivery sparks much controversy among care provid¬
ers, patients, and insurance carriers. This review highlights the
history, candidate, expanded indicators, patient preferences, suc¬
cess rates, factors influencing or predicting success rates, risks,
and maternal and neonatal morbidity and mortality of vaginal
birth after previous cesarean delivery.
Routine Use of Episiotomy in Modern Obstetrics:
Should It Be Performed? 305

This article examines the history of the episiotomy and the evolu¬
tion of its acceptance. The purported benefits of episiotomy are
examined using the scientific literature in an attempt either to
support or reject these claims. The impact of episiotomy on other
issues, such as sexual function, postpartum pain, dyspareunia,
and the complications of the procedure are examined. It is hoped
that this process will allow the reader to come to a conclusion
supported by scientific literature that exists, regarding the role of
episiotomy in modern obstetrics.

Because of the increasing use of reproductive endocrine tech¬
niques, the incidence of multiple gestations has increased dramat¬
ically. This article reviews the literature on many of these contro¬
versial topics related to the intrapartum management of twins

In the United States, almost one third of all deliveries require
some form of intervention. The ability to safely and effectively
apply obstetric forceps remains an important skill and one that
sets obstetrician apart from their surgical colleagues. This capabil¬
ity, one that continues to be performed in a similar fashion as
when it was first introduced into obstetric practice nearly 400
years ago, continues to have an important place in modern obstet¬
rics.
Shoulder Dystocia 371

Shoulder dystocia is an infrequent and unexpected emergency
requiring prompt and appropriate action by the delivering physi¬
cian. This article discusses the recognition of risk factors and the
usefulness of methods to predict and prevent this disorder. The
epidemiology and incidence of should dystocia and neonatal
injury is reviewed; techniques for clinical management are de¬
scribed. A discussion of the medical legal implications of this
event and guidance for optimal documentation are provided.
Peripartum Hemorrhage 385

Peripartum hemorrhage is a relatively common obstetric problem
with multiple causes. It is a major cause of maternal morbidity
and is responsible for 11% to 13% of maternal mortality in the
United States annually. This article reviews the most common
causes of this condition, including immediate postpartum hemor¬
rhage, hematologic disorders, and acquired disorders of preg¬
nancy. Blood product replacement and operative techniques are
also discussed.
Considerations for Delivery on Infants with Congenital
Abnormalities 399

The antepartum evaluation of fetuses with congenital anomalies
includes consideration of peripartum variables, such as route of
delivery, gestational age, and location of each, which may impact
eventual outcomes. In this article, the controversies, risks, and
benefits surrounding peripartum interventions for various fetal
congenital anomalies are discussed.
