CONTENTS
Foreword xiii
George H. Kraft
Preface xv
Patricia W. Nance
Antidepressants in Rehabilitation 237
Robert W. Teasell, Harold Merskey,
and Santosh Deshpande
Antidepressant medications have a variety of uses in addition to
the treatment of depression. This article focuses on the two most
widely used categories of antidepressants, tricyclic antidepres¬
sants (TCAs) and the newer selective serotonin reuptake inhibi¬
tors (SSRIs), and highlights their use in post stroke depression
and chronic pain.
Analgesics: Opiate, Adjuvants and Others 255
Matthew T. Matthew and Patricia W. Nance
The purpose of this article is to summarize the main categories
of pain relieving medications. The authors review a number of
analgesic preparations and treatments, with special emphasis on
advantages, precautions, limitations, and various routes of admin¬
istration.
Antiepileptic Drugs 275
Jay M. Meythaler and Stuart A. Yablon
Antiepileptics are a very important class of medications, and the
number of these drugs available for clinical use has increased

dramatically in the last decade. The pharmacology and indica¬
tions for use in a variety of physiatric patient groups are compre¬
hensively and systematically reviewed.
Anti inflammatory Therapy 301
Janice M.G. Canvin and Hani S. El Gabalawy
This article reviews the pathways involved in inflammation as an
introduction to understanding ways to impede the process. This
article focuses on rheumatic disorders in which pain or inflam¬
mation require medical management. The two main groups of
drugs discussed are nonsteroidal anti inflammatory drugs, both
traditional and selective, and corticosteroids.
Antihypertensives 319
Valsa Daniels and Alan Casey
Management of hypertension requires a thorough knowledge of
currently available antihypertensive drugs. This article reviews
the mode of action of the major classes of antihypertensives,
indications for their use, and adverse effects. The main emphasis
is placed on their use, in physiatric practice.
Antispasticity Medications 337
Patricia W. Nance and Robert R. Young
The article illustrates a practical approach to the challenging
management of problematic, generalized spasticity. Use of dose
titration to achieve symptomatic relief is described. Currently
approved pharmaceuticals used as antispasticity agents and mus¬
cle relaxants and other medications with antispasticity effects are
reviewed.
Neuromuscular Blockers 357
Jean Michel Gracies and David Simpson
The use of corrosive, injectable neuromuscular blockers has been
a treatment option for many years; however, the more recent
advent of botulinum toxin (BTX) treatment has revived interest
in localized treatments. This article reviews the use of local anes¬
thetics, alcohol, phenol, and BTX treatment for localized muscular
overactivity syndromes.
Intrathecal Drug Therapy 385
Patricia W. Nance and Jay M. Meythaler
The direct application of drugs for non hospitalized patients be¬
came a practical therapeutic modality with the advent of im

plantable drug delivery devices, or pumps . This article de¬
scribes the use of pumps for the intrathecal infusion of baclofen,
morphine and clonidine.
Antimicrobial Agents 403
John M. Embil and Lindsay E. Nicolle
This article employs a comprehensive approach to review antimi¬
crobial drugs, emphasizing the strengths and limitations of drugs
including the traditional, those newly introduced and those cur¬
rently under development. The significant issue of antimicrobial
resistant organisms is addressed specifically.
Medications Used in the Treatment of Multiple Sclerosis 437
Randall T. Schapiro
Recently, there have been numerous advances in the treatment of
multiple sclerosis. This article focuses on reviewing the various
forms of MS, managing its various symptoms, and possibly alter¬
ing the course of the disease in an overall attempt to improve the
quality of life of the patient.
Neuroprotective Agents 447
J. Steven Richardson
Cells die all the time, some are pre programmed to do so and
others die as a result of excessive trauma from the external envi¬
ronment. Scientists are searching for ways to delay or even arrest
pre programmed cell death or apoptosis. This article examines
the biochemical processes by which cells die and possible mecha¬
nisms which may allow them to live longer.
Tranquilizers, Stimulants, and Enhancers of Cognition 463
Stephen S. Flitman
For patients with cognitive dysfunction, the appropriate use of
neurotransmitter specific medication may permit independent liv¬
ing. This article examines a variety of tranquilizers, stimulants,
and enhancers of cognition and evaluates which may be most
effective given their side effect profiles.
Urinary and Gastrointestinal Systems Medications 473
Orpha Schryvers and Patricia W. Nance
This article reviews the medical management of the neurogenic
bladder and bowel. The drugs discussed specifically affect detru
sor instability, detrusor weakness, high urethral pressure, low

urethral closure pressure, inflammatory cystitis, and chronic con¬
stipation.
Special Considerations for Medication Use in Children
with Developmental Disabilities 493
Gina Rempel and Jan Coates
Disabilities in children are, by their nature, developmental. Thus,
the rehabilitation approach for children is different from that used
for adults. The purpose of this article is to heighten the reader s
awareness of some of the medical conditions commonly seen in
children with developmental disabilities and the pharmacologic
management that is unique to childhood.
