Pediatric head and neck trauma can be the result of child abuse, negligence,
product liability, or sporting injuries. The common thread that should be
remembered is the preventability of most of these cases. The treating surgeon
must be aware of the effects of further growth on any repair or reconstruc¬
tion. Conversely, the impact of surgical intervention on future growth must
also be kept in mind when managing these patients.

Laryngotracheal complex injury in the pediatric population is rare and often
unrecognized. A high index of suspicion is required in the evaluation and
diagnosis to avoid missing potentially disastrous injury. Stabilization of the
airway and cervical spine, as well as appropriate management of the laryngo¬
tracheal injury can prevent a potentially catastrophic outcome. Management
is dependent on the degree of symptoms and extent of injury. The repair of
massive injuries provides the chance for functional laryngeal outcome.

Head and neck injuries account for high mortality and morbidity in pediatric
patients annually in the United States. The most severe damage is usually
inflicted on the actively developing nervous system. Although the overall
prevalence of trauma compared with adult patients is lower, it has

been shown that inherent physiological differences account for dramatic

Penetrating trauma to the face, neck, and oral cavity in the pediatric popula¬
tion is more often of an accidental nature than of felonious intent. The face
and neck have been divided into anatomic areas to assist with diagnosis
and treatment of these injuries. Oral cavity penetrating wounds can appear
to be quite minor, but can be associated with serious sequelae and should
not be minimized. Assessment and treatment of penetrating injuries to the
face, neck, and oral cavity in children are presented.

Pediatric soft tissue injuries are common, however the majority are minor
and require only basic wound care techniques. The initial management of
such injuries to the head and neck region is discussed. Despite the develop¬
ment of a variety of local anesthetic agents and alternative methods for
wound closure, operative management is often required for younger pa¬
tients. General anesthesia should not be unnecessarily avoided if it will
allow for the optimal treatment of the injury.
Midfacial Fractures in Children: A Classification System Based
on CT Findings to Aid in Surgical Planning 169

There is controversy over which types of facial fractures in the pediatric
population are appropriate for rigid reconstruction. For adult trauma pa¬
tients, CT scans accurately define the pattern of fracture for specific anatomi¬
cal areas suggesting the appropriate approach to reconstruction. This is
based on the concept of functionally classifying fractures as low, middle,
or high energy, depending on the segmentation and displacement seen on
CT. An analysis of facial fractures seen in 53 children confirmed that axial
and coronal CT scans are predictive of the need for open three dimensional
reconstruction, as in adults.

Trauma to the nose and its underlying structures is commonplace in the
pediatric population. Accurate diagnosis is often difficult to achieve despite
clinical and radiographic evaluation. We outline the diagnostic steps and
therapeutic modalities for the treatment of a spectrum of injuries ranging
from nasoseptal trauma to complex nasoethmoidal fractures. Special consid¬
eration is given to nasal development and growth.

Accidental burn injuries are common and most of these are preventable.
Child abuse can account for 25% of burn injuries in children. Pediatric burns
specific to the head and neck region can be isolated injuries or part of a larger
surface area burn. In nonfatal injuries, facial burn wounds command an im¬
portance much greater than any other site. The etiology of the burns, their
prevention, their psychological impact and their rehabilitation are discussed.

Mandibular fractures in the pediatric population are less common than in
adults. The mechanism of injury and stage of dental development will
often determine the fracture pattern and treatment of modality. This article
discusses the epidemiology, anatomy, and evaluation of pediatric mandibu¬
lar fractures. A site specific treatment protocol is presented.

Trauma to the dentoalveolar complex is a fairly common phenomenon in
the pediatric age group. Management of the resultant injuries may present
unique challenges to the clinician. A knowledge of the anatomy, and physiol¬
ogy of this complex, along with an appreciation of the epidemiology and
etiology of these injuries will ensure a course of treatment appropriate to
each individual case.

In pediatric patients suffering trauma involving the eye and periocular
areas, it is essential to ascertain visual function. There can be associated
injuries of the ocular structures with apparently superficial eyelid trauma.
Full thickness lid lacerations in the medial area can cause damage to the
tear drainage system. It is essential that the patient be thoroughly assessed
for injuries to this delicate and vital region.

Facial nerve injury in the pediatric population is an uncommon problem,
usually resulting from craniofacial trauma and occasionally from iatrogenic
injury during otologic or parotid surgery. We review the diagnostic ap¬
proach, use of electrophysiologic tests, and treatment planning in specific
clinical situations. In addition, we discuss the current perspectives on surgi¬
cal management of traumatic facial nerve paralysis in children.

Rigid fixation has revolutionized treatment in pediatric reconstructive sur¬
gery, allowing three dimensional control and excellent stability at the fracture
site. This has resulted in rapid primary healing of bone, better revasculariza
tion, and ultimately, improved functional and aesthetic outcomes. Problems
of rigid fixation in the pediatric patient include exposure of plates beneath
the child s thin skin and translocation to the inner cortex of the skull. There is
also concern about the effect of fixation on the developing craniomaxillofacial
skeleton. The clinical significance of this is discussed, as well as new initiatives
employing resorbable hardware and tissue adhesives such as fibrin glue.
