Radiologic evaluation of the foot and ankle affected by rheumatic
disease involves objective assessment of the radiologic changes
that may be associated with a specific arthritis. Assessment of the
severity of involvement and evaluation of progression or regres¬
sion of the disease are important in the treatment regimen and
ultimate clinical outcome of every patient.

Most patients with arthritic diseases present with joint disease,
inflammation of various organs, and, in some rheumatic diseases,
various degrees of pedal deformity. Unlike the upper extremities,
the lower extremities in afflicted individuals commonly develop
deformity secondary to superimposed body weight on inflamed
joints and soft tissues. At the foot and ankle, small deviations
from normal motion cause altered foot mechanics that can com¬
pound and accelerate destructive forces. Most rheumatic diseases
do not cause death; therefore, the later stage manifestations usu¬
ally diminish an individual s ability to ambulate and perform
activities of daily living, decreasing his or her quality of life. With
a proper understanding of these diseases, their natural history,
and progression, the clinician can be prepared to assist the patient

in understanding and coping. By understanding the mechanics
of the lower extremity, the clinician better understands what is
necessary to restore some function and add to the quality of life

of second line drugs, and discusses the doses, side effects, and

Reconstruction of the painful rheumatoid forefoot is based on
panmetatarsal head resection. The authors consider indications

Rheumatoid arthritis is a common disorder that has significant
destructive effects on the midfoot. When these degenerative joint
changes occur, any motion at the involved joints causes severe
pain and disability, and limits the patient in his or her attempts
to perform daily activities. This disorder has a significant social
and economic impact, and causes unwanted lifestyle changes.
Arthrodesis, when used judiciously, can offer pain relief and
a restoration of normal activity for the patient suffering from
rheumatoid arthritis.

This article presents an overview of rearfoot surgical correction in
the patient with rheumatoid arthritis. Pathophysiology, diagnosis,
imaging, surgical planning, and technique are discussed. The

Tibiotalar arthrodesis is the definitive treatment for advanced
changes in the rheumatoid ankle. Various fixation techniques are
explored in this article through a literature review. The authors
preferred procedure of screw fixation is described. Achievement
of a solid arthrodesis and pain relief can be attained in this
patient population; however, marked improvement in function is
limited by adjacent joint involvement.

Rheumatoid Arthritis of the Forefoot 363

Severe and often debilitating involvement of the forefoot is seen
frequently in patients with rheumatoid arthritis. The combination
of destructive synovitis and weightbearing leads to a spectrum
of disorders that cause pain, deformity, and eventual loss of
function. In addition, advances in total knee and total hip
arthroplasty have placed added importance on preserving the
ability of the foot and ankle to bear weight and allow functional
ambulation. This article reviews the epidemiology, pathophysiol
ogy, clinical presentation, physical examination, and treatment of
rheumatoid arthritis of the forefoot.
Rheumatoid Arthritis of the Ankle and Hindfoot 373

Rheumatoid arthritis frequently affects the hindfoot and ankle,
and may present a considerable source of dysfunction. Awareness
of characteristic clinical and radiographic findings and other diag¬
nostic modalities help the clinician to evaluate the progression of
the disease and determine the best methods of treatment. A
variety of nonoperative treatments may slow progression of de¬
formities, improve function, and provide symptomatic relief. If
these measures fail, surgical intervention, including soft tissue
procedures and a variety of arthrodesis techniques, can return a
patient to a more active lifestyle.
