One of the most widely used and effective treatments for breast
cancer is the anti estrogen tamoxifen. The efficacy of this hor¬
monal therapy can be explained by the dependence of many
breast tumors on estrogen for growth and survival. Like estrogen,
many other hormones and growth factors are known to play a
role in normal breast development, and also have been proposed
as factors in the origin of breast cancer. A better understanding
of the contribution of these factors to breast neoplasia may pro¬
vide the impetus for developing new therapies to combat inherent
or acquired resistance to anti estrogens, which is common.

Many exciting developments are occurring in breast imaging.
Digital mammography holds the promise of telemammography
and computer aided diagnosis. Mammoscintigraphy may be help¬
ful in identifying drug resistant rumors before therapy. There is
renewed interest in evaluating ultrasound as a potential ad
junctive screening tool in women with radiographically dense
breasts. Finally, contrast enhanced magnetic resonance imaging
may be used more extensively in the monitoring of tumor re¬
sponse to primary chemotherapy and in the preoperative work
up of patients being considered for breast conservation therapy.

The most accurate predictor of survival in breast cancer is the
presence or absence of lymph node metastases. Lymphatic map¬
ping with sentinel node biopsy is a new technique that provides
more accurate nodal staging compared with routine histology for
women with breast cancer, but without the morbidity of a com¬
plete lymph node dissection. Sentinel lymph node (SLN) biopsy
is a more conservative approach to the axilla that requires close
collaboration from the surgical team, nuclear medicine, and pa¬
thology. National trials are investigating the clinical relevance of
the upstaging that occurs with a more intense examination of the
SLN. As is the case with breast preservation as a viable alternative
to mastectomy for the definitive treatment of the primary node,
selective lymphadenectomy has the ability to decrease morbidity
without compromising patient care.
An Update on Radiation Therapy in Breast Cancer 373

Radiation therapy plays an important role in the management of
both invasive and noninvasive breast cancer. During the last 20
years, the availability of radiation therapy has made it possible
to test the feasibility and safety of breast preservation after the
diagnosis of early stage breast cancer. This article summarizes
some of the ongoing controversies concerning the use of radiation
therapy in the multidisciplinary management of breast cancer.

The breast cancer mortality rate is falling, most likely because of
a combination of early detection, refined surgical and radiation
therapy techniques, and improved systemic therapy efficacy. The
proper integration and application of these treatment modalities
present evolving challenges for clinicians. Systemic therapy, in
particular, is changing rapidly with the advent of new chemother¬
apy drugs, new classes of agents, and new therapeutic regimens.
The most recent studies suggest that optimal outcomes are possi¬
ble through the broad but appropriate use of hormone therapy
and chemotherapy to prevent relapse and possibly prevent second
primary tumors. The choice of therapy for patients remains a matter
for careful consideration and discussion in each individual case.
The Role of Chemotherapy for Metastatic Breast Cancer 415

Metastatic breast cancer remains a cause of considerable morbid¬
ity and mortality and probably accounted for 43,900 deaths in the
vi CONTENTS
United States in 1998. The last decade has seen an increase of
new chemotherapeutic agents with significant activity. The tax
anes have become the salvage therapy of choice, particularly
in patients who received anthracycline based adjuvant therapy.
Questions of the optimal duration of therapy and the importance
of dose intensity continue to be debated, but have been clarified
by recent trials. Continued research to develop novel drugs and
biologic therapies offers hope for future patients.
Hormonal Therapy for Breast Cancer: An Update 435

The use of endocrine manipulation for the treatment of breast
cancer has been available for 100 years, but in recent years the
number of therapeutic options available to patients has increased
dramatically. This article considers new developments in the use
of hormonal agents for the treatment and prevention of breast
cancer.
Locally Advanced Breast Cancer 457

The multidisciplinary management of patients with locally ad¬
vanced breast cancer (LABC) has evolved over the last three
decades. The introduction of chemotherapy (CT) and endocrine
therapy in conjunction with surgery and radiation therapy (XRT)
have changed the natural history of this disease. The authors
discuss the role of the taxanes, novel endocrine therapy, and high
dose CT programs in the management of LABC. Indications for
breast conserving surgery are presented. The role of prognostic
and predictive tumor markers in LABC is discussed.
