This article serves as a general review of laparoscopy and hyster¬
oscopy. Subjects covered include equipment and instrumentation,
use of laser and electrosurgery, surgical technique, and complica¬
tions.

With a detailed knowledge of pelvic anatomy, familiarity with
the many laparoscopic instruments, and attention to the details
of good technique, many of the complications of laparoscopy
described in this article can be avoided. The information pre¬
sented complements that in the other articles in this issue and
briefly summarizes many salient lessons already published in
other excellent texts. The focus is on the most common laparos¬
copic complications, namely, those associated with insufflation
and trocar insertion, such as vascular injury and bowel perfora¬
tion. Other complications that may arise during adhesiolysis,
removal of and bleeding from an ectopic pregnancy, and speci¬
men removal from the abdomen are also discussed.

The cases in this article were chosen to demonstrate vitally im¬
portant clues that should alert the surgeon of an impending

complication. The discussion following each case includes a re¬
view of the data on the specific complication as well as the
author's opinion on what went wrong and how the complication
could have been avoided. Recognition of these situations will
lead to prevention, which is essential in hysteroscopy.

The role of the human fallopian tube in reproduction is more
than a passive conduit. This organ is involved, in oocyte pick up,
fertilization, and embryo transport. Metabolic substrates, cyto
kines, and immunoglobulins have been identified within the tubal
lumen. Complex and coordinated endocrine secretions, neuro
muscular activity, and cilial action are required for successful
tubal function.

The dual endoscopic modalities of laparoscopy and hysteroscopy
have progressed from strictly diagnostic studies designed to eval¬
uate the causes of gynecologic complaints, such as pelvic pain
and infertility, to operative procedures encompassing a multitude
of complex surgeries. The evolution of endoscopic instrumenta¬
tion has enabled procedures formerly performed only by laparot
omy and microsurgery to be completed by endoscopy as mini¬
mally invasive procedures. Near simultaneous evolution of
ultrasound transducers from relatively primitive and cumber¬
some A mode equipment to real time, sophisticated, miniaturized
transducers encased in catheters and endoscopic sheaths has per¬
mitted the introduction of intraoperative sonographic monitoring.
This article outlines the evolution of endoscopic sonography and
reviews its applications to gynecologic laparoscopic and hysteros
copic operative procedures.

Worldwide, tubal sterilization is the most commonly chosen form
of contraception by women who have completed their desired
childbearing. In the United States, it is chosen by more married
or formerly married women than any other method. Sterilization
has enjoyed increasing popularity since the 1950s, primarily ow¬
ing to the technologic advances and increasing social acceptance.
Improvements in technique and technology have rendered it a
safe, effective, and cost efficient outpatient procedure. This makes
tubal sterilization a particularly attractive choice in the managed

care setting and in developing countries, where access to medical
care occurs most frequently in rural clinic settings.

A trend is emerging in the United States whereby surgical proce¬
dures are gradually migrating to less complex environments. The
demands of cost containment, pressures to limit unnecessary time
delays, and desires for increased control have all conspired to
promote ambulatory surgicenters, minor procedure center, and
office surgical suites. Concomitant with this shift is a differing
attitude toward anesthesia, with an increasing number of proce¬
dures using alternatives to general anesthesia, such as regional
blocks and conscious sedation.
Office Microlaparoscopy Under Local Anesthesia 109

Office laparoscopy under local anesthesia is especially suited to
meet the current pressures of quality versus cost in an era of
managed care. It is likely that this technique will soon become a
major part of the practicing gynecologist's diagnostic operative
armamentarium. Advantages of office microlaparoscopy under
local anesthesia are realized by the practitioner, the patient, and
the managed care provider. Office microlaparoscopy under local
anesthesia is a safe, effective, and less costly tool for the evalua¬
tion of patients with many different indications. To date, the
procedure has been primarily used for patients with infertility,
chronic pelvic pain, and tubal ligation. The ease of scheduling,
reduced costs, and rapid recovery suggest that it may be the
preferred initial procedure for these patients.

Surgical therapy for the treatment of patients with symptomatic
endometriosis has always been a mainstay of therapy; over the
past decade, endoscopy has largely replaced laparotomy. With
the availability of improved laparoscopic instrumentation, most
women can have the diagnostic and therapeutic intervention per¬
formed at one setting. This article reviews surgical approaches to
peritoneal disease, both superficial and deeply infiltrating, as well
as ovarian, urinary, and gastrointestinal endometriotic disease.
Surgical approaches to pelvic pain relief, such as presacral neurec
tomy and uterosacral nerve resection are also discussed. Because
no definitive cure is available for endometriosis except castration,
the formation of a rational treatment plan should lead to the

most efficient number of repetitive surgical interventions over the
patient's reproductive lifetime.

There is a place for both laparoscopy and laparotomy in the
surgical management of patients with uterine myoma. The sur¬
geon's expertise in laparoscopic suturing is a crucial requirement
for laparoscopic myomectomy. Hysteroscopic myomectomy is the
best treatment of women with submucous myoma.
Laparoscopic Treatment of Stress Urinary Incontinence 149

Urinary incontinence is the demonstrable involuntary loss of
urine that is socially or hygienically unacceptable or detrimental
to the patient. This article reviews the basic principles of surgical
treatment of genuine stress incontinence, describing laparoscopic
surgical technique and the results of the author's experience in
treating patients with this condition.
Laparoscopically Assisted Vaginal Hysterectomy 169

Laparoscopically assisted vaginal hysterectomy has emerged as a
viable surgical option to abdominal hysterectomy. This article
addresses the indications for laparoscopically assisted vaginal
hysterectomy optional routes for hysterectomy, and alternative
surgical approaches for performing laparoscopic hysterectomy,
with varying degrees of assistance. Also discussed is the role of
surgical experience in minimizing complications and enhancing
outcomes and the use of ancillary medications in the preoperative
treatment of patients. Further reviewed are the comparative clini¬
cal and economics outcomes of the primary approaches to hyster¬
ectomy.
Laparoscopic Microsurgical Tubal Anastomosis 189

The authors explored the feasibility of performing true microsur¬
gery through the laparoscope in 1990. The first laparoscopic mi¬
crosurgical tubal anastomosis was performed in February 1992.
Operative laparoscopy will continue to expand as technical feasi¬
bility continues to improve, driven by both hardware advances
and increased surgical dexterity. Laparoscopic microsurgery will
introduce a new dimension to reproductive surgery and over
time, will replace laparotomy for microsurgery.

Recent technologic advances in laparoscopic equipment have en¬
abled surgeons to apply operative endoscopic techniques to pa¬
tients with gynecologic malignancies. Operative laparoscopy has
been described in the surgical staging and treatment of patients
with ovarian, cervical, and endometrial cancers. It has the poten¬
tial to revolutionize many aspects of gynecologic cancer manage¬
ment; however, the use of operative laparoscopy for the treatment
of patients with gynecologic malignancies is still in its infancy,
and until more data accumulate, it must be considered a promis¬
ing but unproven tool in this setting.

Abnormal uterine bleeding is a common clinical problem. Hyster¬
oscopy is unparalleled in its sensitivity and specificity in identi¬
fying both benign and malignant intrauterine disease. This article
covers hysteroscopic instrumentation, diagnostic and operative
hysteroscopy, and hysteroscopic complications as they apply to
the management of patients with abnormal uterine bleeding.
