There is a tremendous variability in the perception of pain that is
modulated by cognitive, emotional, and environmental factors.
The authors review discrete neuronal connections and neuro
chemicals that contribute to the variability of the pain experience
by way of descending inhibitory and facilitory pathways. The
evidence for integration of stress systems with descending pain
modulatory pathways is discussed. Appreciation of the influence
of higher centers should lead to improvements in the clinical
management of pain.
Pain in the Rheumatic Diseases: Practical Aspects of
Diagnosis and Treatment 15

Pain is a pervasive complaint of patients with musculoskeletal
disease. This symptom may result from both central and periph¬
eral factors and, in the individual patient, may have individual
causes. Because of the interplay of these factors, treatment can
entail a variety of pharmacological agents, with drugs modulating
central pain as a common component even in patients with struc¬
tural and inflammatory disease. In older patients and in patients
with chronic inflammatory disease, frequent reevaluation may be
needed to stage disease and detect intercurrent problems leading
to pain.

Causes of Pain in Children with Arthritis 31

Pain in children with juvenile rheumatoid arthritis (JRA) is often
not fully recognized and is therefore incompletely treated. The
use of pain assessment instruments developed specifically for
children may enhance recognition by health care providers. Re¬
cent studies suggest that coping variables and disease severity
are significant predictors of pain in children with JRA. Pain in
children with JRA is a complex phenomenon, best managed using
a multidisciplinary approach that includes aggressive traditional
medical management and addresses psychosocial variables such
as coping strategies and perceptions about disease.
Pain in Fibromyalgia 55

Fibromyalgia is not a disease. It is a clinical construct that de¬
scribes a chronic pain syndrome with many associated somatic
symptoms, particularly fatigue. Fibromyalgia may be part of a
spectrum of related disorders with similar antecedents and patho¬
genic mechanisms. Pain in fibromyalgia is caused by a combina¬
tion of psychologic, neuroendocrine, and central pain regulatory
processes that lower thresholds of pain perception and pain toler¬
ance. Female sex, chronic stress, psychologic distress, and associ¬
ated blunting of the stress response are central elements in this
regard. Overall morbidity and disability are potentiated by envi¬
ronmental and sociocultural variables.
Psychosocial Assessment of Pain in Patients Having
Rheumatic Diseases 81

This article provides an overview of psychosocial approaches to
assessing factors relevant to the understanding and management
of arthritis pain. The article is divided into two major sections.
Section one provides a conceptual background that highlights the
role that psychosocial factors play in the pain experience. Section
two briefly describes and evaluates methods used to assess psy¬
chological factors (e.g., pain behavior, pain coping, and self effi¬
cacy) and social factors (e.g., social support, marital adjustment)
that can affect adjustment to persistent pain.
Negative Affect and Pain in Arthritis 105

The experience of pain in arthritis conditions has important af¬
fective dimensions. This article reviews evidence for a relatively
strong association between negative affect (i.e., depression, anxi¬
ety, and anger) and arthritis related pain. Possible physiologic

and psychologic mechanisms of the relationship between negative
affect and pain are examined, and issues relevant to future re¬
search, particularly the need for biopsychosocial theoretical mod¬
els are discussed. Finally, the article highlights the importance
of biopsychosocial treatment approaches in managing arthritis
related pain and negative affect.
The Contribution of Functional Imaging Techniques to
Our Understanding of Rheumatic Pain 123
Anthony K. P. Jones
Some of the recent advances in functional brain imaging that
allow us to view molecular events within the brain are reviewed
in this article. The focus is on positron emission tomography, but
some of the important results using other techniques such as
single photon emission tomography, functional magnetic reso¬
nance imaging, surface electrophysiology (EEG and pain evoked
potentials), and magnetoencephalography are mentioned where
appropriate. The contributions these techniques have made to
providing a much more detailed understanding of how the brain
processes information about actual and possible noxious stimuli
is described. All of the imaging techniques are not reviewed, but
a brief outline of what they measure is included.
The Use of Analgesics in the Management of Pain in
Rheumatic Diseases 153

Pain is the most common complaint of patients who see rheuma
tologists. In this article, the current treatment options for pain
are reviewed; these include acetaminophen, nonsteroidal anti
inflammatory drugs, new specific cyclooxygenase 2 inhibitors,
opioid analgesics, centrally acting muscle relaxants, antidepres
sants, and topical analgesics and counterirritants. The doses of
medication and known adverse effects of these medications are
highlighted.
Aggressive Pharmacologic Treatment of Pain 193
Marco Pappagallo
In this article, the author outlines the pharmacology of nocicep
tion and discusses the two major classes of drugs used for pain
control: opioids and nonopioid analgesics. In order to provide
satisfactory pain relief and prevent the possible sequelae of un¬
treated pain, physicians must possess both knowledge and exper¬
tise in the use of opioid and nonopioid analgesics. Opioid analge¬
sics have been underused in the management of pain. Opioids
have a higher analgesic potency and wider range of indications
than any of the other currently available medications for pain
control. The second class of drugs, the nonopioid analgesics and

adjuvants, has recently expanded to include new and potentially
beneficial medications. This article furthers the understanding on
how to use analgesics for a prompt, safe, and effective pharmaco
logic treatment of acute and chronic pain.
Psychological and Behavioral Approaches to Pain
Management for Patients with Rheumatic Disease 215

This article reviews the efficacy of the psychological and behav¬
ioral pain management interventions that have been evaluated
among adult patients with rheumatoid arthritis (RA), osteoarthri
tis (OA), and fibromyalgia (FM). Using published criteria for
empirically validated interventions, it is concluded that cognitive
behavioral therapies and the Arthritis Self Management Program
represent well established treatments for pain among patients
with RA and OA. These interventions involve education, training
in relaxation and other coping skills, and rehearsal of these skills
in patients home and work environments. There currently are no
psychological or behavioral interventions for pain among FM
patients that can be considered as well established treatments.
Future intervention research should use clinically meaningful
change measures in addition to conventional tests of statistical
significance, attend to the pain management needs of children,
and assess whether outcomes produced in university based treat¬
ment centers generalize to those in local treatment settings.
The Role of Physical Therapy and Physical Modalities
in Pain Management 233

This article provides an overview to arthritis care of the common
physical modalities (heat, cold, transcutaneous electrical nerve
stimulation, low energy laser, topical applications, and external
devices). The rationale for use and effectiveness of the various
physical modalities are discussed. Exercise is presented in terms
of mode and effect of range of motion, muscle conditioning, and
aerobic exercise.
