ity control and reduction of exposure to infection are discussed.

Although the double blind, placebo controlled oral challenge re¬
mains the gold standard, under appropriate circumstances, the
clinician can effectively, practically, and safely use open and sin¬
gle blind challenges with benefits to patients and staff of de¬
creased time and expense and increased convenience and flexibil¬
ity; however, unequivocal confirmation of a positive response in
these tests calls for a double blind, placebo controlled challenge.

Allergic diseases are common in the United States, affecting as
many as 50 million Americans. Exposure and sensitization can
occur with common indoor allergens, such as house dust mites,
pets, cockroaches, and fungi, and outdoor pollens and fungi. Air

irritants can contribute to symptoms. Assessment of environmen

Asthma and allergies are treated with some unique devices that
allow the medications to be placed at the site of the problem, the
nasal passages, and airways. These devices allow people with
asthma to use small doses of medication to attain appropriate
responses. The number of medications used to treat asthma and
allergies has grown in the last few years. As allied health profes¬
sionals, we are responsible for helping our patients understand
their treatment and the methods used to deliver it. Our goal is to
help patients improve their quality of life by optimizing their
therapy.
Allergen Immunotherapy 129

Allergen immunotherapy is an effective modality for patients
with allergic rhinitis, allergic asthma, and stinging and biting
insect hypersensitivity. An effective dosage of allergen is neces¬
sary to achieve the desired reduction of symptoms, but it is
essential to balance the risk of anaphylaxis with the need to
relieve symptoms. The allied health caregiver plays an important
role in administration of immunotherapy, prevention, and recog¬
nition of symptoms of adverse reactions, as well as knowledge of
how to initiate expedient treatment of adverse reactions.

Genetic predisposition and environmental exposures are im¬
portant factors in the development of allergic disease. Although
genetic engineering holds promise for absolute prevention in the
future, our current approach relies on preventive strategies that
may help to reduce or delay the occurrence of atopic disorders.
Identifying infants at risk for atopic diseases is the first step, with
a family history of allergy being the most important clue. Risk
factors and their role in allergy prevention are discussed, includ¬
ing those that are not easily altered, as well as those that can be
manipulated to prevent atopic sensitization.

Education of the Patient with Asthma 161

Education of the patient with asthma is an ongoing process. This
article discusses how to create a partnership with the patient;
how to teach effectively and design goals with patient participa¬
tion; and how to integrate patient education into the medical
practice, using clinical staff and physicians. It also emphasizes
how to encourage the patient into self management of asthma by
seeking family involvement in all aspects of the learning process.
This article also discusses which skills and information are most
important for the patient to learn. Asthma is a chronic disease
that causes many hospitalizations and low productivity because
of missed school or work. With proper patient education, unnec¬
essary hospitalizations and decreased productivity can be de¬
creased or eliminated.

This article describes the guidelines for telephone advice and
triage in an asthma/allergy setting. Medical problems discussed
include rhinosinusitis, conjunctivitis, asthma, anaphylaxis, urti¬
caria and angioedema, and eczema or dermatitis.

Allergic emergencies, such as acute asthma or anaphylaxis, are
serious situations that may occur in the school setting. Under¬
standing what triggers an allergic emergency and recognition of
early warning signs are important first steps. Appreciating the
limitations on learning or participation in sports, identifying
health risk factors that may be present in the school setting, and
having a well informed staff with a clear action plan are also
crucial in the management of asthma, allergies, or anaphylaxis.
Pregnancy and Atopic Diseases 191

Atopic diseases affect between 15% and 20% of the normal popu¬
lation, and this proportion holds true for women of child bearing
age as well. Asthma, one of the most common of the respiratory
diseases encountered during pregnancy, affects about 4% of preg¬
nant women. This article details the normal physiologic changes
that occur with pregnancy, and discusses the concerns regarding
the use of medications in pregnancy. It reviews the effects of
pregnancy on asthma and the effects of asthma on pregnancy.
Guidelines for the treatment of asthma and other allergic diseases
are presented.
