A carefully executed, thorough physical examination of the elbow will per¬
mit reproducible results to be recorded over time. It also provides a working
diagnosis or contributes to the determination of a correct diagnosis in a
majority of presenting patients.
Imaging of Elbow Disorders 21

Numerous modalities are available for imaging the elbow. Radiographs
should be the first imaging procedure performed for evaluation of an elbow
abnormality. The use of advanced imaging modalities such as MR imaging,
CT, and sonography also is discussed in this article.
Fractures of the Elbow 37

Elbow fractures encompass a spectrum of severity from low energy non
displaced fractures to high energy fractures with associated severe soft tis¬
sue injury. Treatment is based on fracture pattern, patient age, bone quality,
associated soft tissue injuries, and associated fractures. The basis for treat¬
ment relies on the knowledge of the complex osseous and soft tissue anat¬
omy of the elbow. The goals of operative and nonoperative treatment are
to achieve a stable, anatomic reduction that allows early motion to maximize
function.

Acute Elbow Dislocations: Simple and Complex 63

The elbow is the second most common joint dislocated in adults. Up to 20%
of dislocations are associated with fractures. Treatment principles are re¬
duction of the joint, stabilization of associated fractures, and early motion.
Ligament repairs or reconstruction and hinged external fixators are neces¬
sary in some cases to restore stability for early motion. In general, simple
dislocations have a better prognosis than complex dislocations (fracture
dislocations).
Chronic Elbow Instability 81

Chronic elbow instability without fractures in adults can be broken down
into three types: (1) valgus instability, (2) posterolateral rotator instability,
and (3) isolated radial head instability. By far the most common is valgus
instability, which usually occurs in throwing athletes as a result of repetitive
microtrauma to the anterior band of the medial collateral ligament. Surgical
treatment is aimed at reconstruction of the anterior band of the medial col¬
lateral ligament. Posterolateral rotatory instability is much less common and
is diagnosed by the lateral pivot shift test of the elbow. It is usually the result
of an undiagnosed acute injury to the lateral ulnar collateral ligament. Sur¬
gical treatment is aimed at its reconstruction. Isolated radial head subluxa
tion in adults is extremely rare. The essential lesion is a disruption of the
annular ligament. Surgical treatment is either radial head resection or re¬
construction of the annular ligament.
Traumatic Nerve Injuries About the Elbow 91

Although not common, nerve injuries about the elbow occur because of the
proximity of the three upper extremity nerves or because of the relationship
of the median, ulnar, and radial nerves to the bony and soft tissue structures
about the elbow joint. Nerve injuries at and about the elbow joint occur
more frequently with fractures than with any other kind of trauma. Nerve
injuries may be found with periarticular fractures, dislocations, gunshot
wounds, lacerations, and other iatrogenic causes.

reviewed. The relevant anatomy, presenting signs, symptoms, and treat¬
ment of these injuries are discussed.

This article provides a summary of the acute management of pediatric elbow
trauma. Treatment options are reviewed for all pediatric fractures, specific
recommendations are outlined, and complications are addressed. Particular
emphasis is placed on the common, but often complicated supracondylar
fracture of the distal humerus.

This article describes the technique of elbow release and debridement using
a lateral approach designed to spare the lateral collateral ligament complex
and extensor tendon origins of the elbow. This exposure allows for complete
exposure of the anterior and posterior ulnohumeral and radiocapitellar
joints through a single incision. Advantages include simplified surgical dis¬
section, minimal operative morbidity, and less potential for postoperative
instability after release.
Posttraumatic Elbow Arthritis and Arthroplasty 141

Elbow contractures without significant articular injury can be treated with
soft tissue procedures, such as joint capsular releases, resection of hetero
tropic ossification, and joint debridement. Elbows with significant intra ar
ticular injuries or posttraumatic arthritis require some form of joint recon¬
struction, such as a distraction arthroplasty, interposition arthroplasty, or
implant replacement arthroplasty. With a better understanding of the sur¬
gical indications for elbow arthroplasty, improvements in surgical tech¬
nique, improvements in elbow implants, and increased surgical experience,
the results of implant arthroplasty for the posttraumatic elbow continue to
improve.
Elbow Arthroscopy in Trauma and Reconstruction 163

Arthroscopy of the elbow has become a valuable adjunct in the evaluation
and treatment of elbow trauma and reconstruction. Arthroscopic visuali¬
zation aids diagnosis and treatment while minimizing surgical trauma. The
indications and techniques of elbow arthroscopy continue to expand and
evolve. This article demonstrates the use of elbow arthroscopy as it relates
to some of the topics in this volume. Arthroscopic setup, portal anatomy,
technique, as well as an overview of trauma, loose bodies, bursitis, insta¬
bility, arthrofibrosis, arthritis, and complications are discussed.
