Critical care nurses are in excellent positions to transition their practice into the growing high tech
home health industry. They need to be aware of the prerequisites for success, many of which they
already possess. They need to know more about insurance systems, diagnosis aiding, and outcomes
tools, and to prepare themselves for the rapid growth of computer assisted technologies that are
being introduced into the practice of homecare. They also need to take the whole continuum of care
into consideration when planning care for patients in the home, and to be aware of the community
and family supports that can enhance the patient s health related quality of life.

Over the last two decades, complex financial issues and trends have dramatically reconfigured the
health care landscape. When governmental regulation forced a change in third party reimbursement
mechanisms from a retrospective to a prospective payment system, the effect on hospitals was
profound. To meet the demand for cost containment, acute care institutions decreased the length of
stay, and returned the client to the home setting still in need of highly complex care. As a result, the
nation witnessed a tremendous growth in homecare services. In search of less expensive delivery
options, the present technologically driven healthcare industry has focused its attention on high tech
homecare. As the scope of practice for the newly emerging high tech homecare nurse continues to
evolve, the boundaries separating the traditional ICU from the homecare setting will continue to
dissolve as the way is paved for a more cost effective, seamless healthcare system.
Telecommunication Technologies in High Tech Homecare 279

Telecommunication technologies have virtually exploded the homecare industry. With the advent of
telenursing systems, electronic dala can now be transmitted instantaneously from the home to ihe
physician s office and beyond. Computerized F.CGs and portable radiographs, obtained in Ihe home,
can be imported, via digital imaging, into the client s electronic medical record. Interactive video
devices and cable technology permit the transmission of visual images and sound lo remote facilities
where health professionals can monitor client progress during the electronic home visit. Tith the
assistance of specialized devices, vital signs, heart and lung sounds, pacemaker function, wound
assessment and intravenous site assessment can be obtained by computer and stored as digital images
for later retrieval. Transtelephonic defibrillation will become a routinely used intervention to

arrest life threatening chaotic heart activity in the home, discharging an electronic signal over ordinary
telephone lines through the phone receiver placed over the client s chest.

Dramatic changes in health care delivery have created an increasing reliance on ambulatory care.
Adults and children are increasingly receiving infusions of intravascular medication or fluid in the
home for a wide array of conditions. Several therapies have been adapted to the home. High
technology therapy in the home mandates safe and reliable vascular access to maximize positive
outcomes, independence, and self care; minimize costs and disruption of daily activities and work
schedules: and enhance client and family adjustments and satisfaction.

acute care settings and into the community for subacute treatment and management of their illness.
Nurses with critical care knowledge and experience increasingly provide technologically advanced
care to patients at home, who often manifest co morbid psychiatric symptoms. This article discusses
common, acute psychiatric symptoms in medically ill patients, and provides the knowledge necessary
to manage and stabilize the psychiatric symptoms in collaboration with advanced practice psychiat¬
ric nurses.
High Tech Wound and Ostomy Care in the Home Setting 327

Changes in the health care delivery system present a multitude of challenges to the home care
clinician. The level of acuity is much higher and the use of technology is on an upward trend. Wound
and ostomy care, with thousands of products, multiple wound etiologies, and limited reimbursement
present frustration for client and caregiver. In this environment, planning for effective care is essential.
This article reviews the physiology of wound healing and identifies goals for the promotion of wound
healing. Product selection is correlated with wound assessment and the goals of care. Wound care is
discussed in light of available technology, along with the principles for ostomy and fistula management.

Pediatric high tech homecare is in a state of evolution. Nurses are faced with challenges that require
more than technical expertise. The purpose of this article is to assist nurses in their transition from
high lech hospital based care to pediatric home care. Family centered care, theoretic frameworks,
the nature of pediatric home care agencies, and nursing skills required for pediatric homecare are
the primary issues addressed in this article.

High risk pregnancy and neonatal homecare management is on the rise. More conditions and problems
are being treated in the homecare environment than ever before. The most frequently encountered
high risk maternal conditions are reviewed and discussed. Apnea monitoring and the management
of hyperbilirubinemia in the neonate are also discussed. Management in the home has proven to be
a familiar, comfortable, and safe interventional strategy for health care delivery. Education of the
patient and family is essential to ensure optimal outcomes.

In the culturally rich environment of the homecare setting, nurse have been and continue to lie
concerned with providing culturally specific and sensitive care to racially and ethnically diverse groups
of clients. In the Southeastern and Southwestern regions of the nation, two out of every five children
are classified as minorities. By the year 2000, two out of every three children will be classified as
ethnic children of color; and by the year 2080, majority status will not be held by any single ethnic
population. The purpose of this article is to provide guidelines for the rendering of culturally sensitive
nursing care in the high tech homecare environment. As our nation becomes more culturally diverse,
it is incumbent upon the high tech homecare nurse clinician to become familiar with and be able to
incorporate relevant aspects of culture into the plan of care.

approach to achieving that goal for both national and international health care systems. It is clear
that managed care organizations seek innovative approaches to care deliver) , and that they recognize

sophisticated models of interdisciplinary practice, high tech homecare nurses are positioned at ihe
