This article provides a brief review of sport participation for
children and adolescents from psychological, physical, and social
developmental perspectives. The following areas are reviewed:
the relationship between normal developmental readiness and
sporting participation; the potential positive and negative aspects
of athletic participation for the child and adolescent; the effects
of sporting participation on self concept; potential adverse physi¬
cal and psychological effects; recent research regarding motiva¬
tion for youth sports participation; proposed recommendations
for guidelines in youth sport programs; a social perspective on
sports in the United States; the current and future role of child
and adolescent psychiatrists; and future challenges for sport psy¬
chiatry.

Societal and familial forces of enormous emotional intensity have
contributed to the growth of youth sports during the twentieth
century, and continue to affect adult participation in children s

athletics. This article addresses the historical evolution of youth
sport programs, then identifies opportunities for adults to facili¬
tate developmental progress and surmount emotional obstacles
as they interact in sport activities with children ranging from
preschool ages through adolescence. Youth sports are viewed as
offering tremendous opportunities to teach not only physical
skills and playing strategies, but also psychological skills and
coping strategies that have applicability throughout life. A devel¬
opmental rationale is provided to support recommendations for
parents and coaches. Child and adolescent psychiatrists are faced
with the challenge of collaborating with other professionals and
making their knowledge base available to the volunteers who
coach in and support youth sport programs.
Young Females in the Athletic Arena 745

There are factors that uniquely affect young females in sports.
The sociologic aspects of female youth sports can be understood
from a historical perspective. Athletics have an influence on the
development of young girls, from gender identity and sexual
orientation to the foundations of self esteem and moral develop¬
ment. There is a relationship between certain psychopathologic
conditions common in girls and athletics, including eating disor¬
ders, depression, and anxiety disorders. Although involvement in
sports may engender some of these problems, others may be
successfully treated through physical exercise.
Exercise and Sports in Children and Adolescents with
Developmental Disabilities: Positive Physical and
Psychosocial Effects 757

This article reviews findings to date on the effects of exercise and
sports in children and adolescents with developmental disabili¬
ties. Although much work remains, exercise and sports are associ¬
ated with reduced maladaptive behavior in children with disabili¬
ties, as well as with improved physical fitness, self esteem, and
social competence. Improved physical and psychosocial function¬
ing are found in studies of both children and adults with mental
retardation, as well as in research on athletes enrolled in Special
Olympics International, the largest recreational sport program in
the world for persons with developmental disabilities. The review
ends with recommendations for promoting recreation and sports
in children and adolescents with disabilities.
The Effects of Aerobic Training on Children s Creativity,
Self Perception, and Aerobic Power 773

The article examines whether participation in an aerobic exercise
program (AE), as compared with a traditional physical education

class (PE), significantly increased children s perceived athletic
competence, physical appearance, social acceptance, behavioral
conduct, and global self worth; increased their figural creativity;
and improved aerobic power as measured by an 800 meter run
around a track. Further research on the effects of different types
of AE is discussed, as well as the need for aerobic conditioning
in the elementary school.
The Psychiatric Consultant in Professional Team Sports 791

The role of the sport psychiatrist consulting to a professional
sports team is complex, challenging, and varied. Sport psychia¬
trists serve to enhance the psychosocial development of the ath¬
lete by removing obstacles to growth and by facilitating lines of
communication that support the exchange of information, sharing
of feelings, and the development of solid winning relationships
on the team and throughout the organization. The consultant
psychiatrist often provides input to players and to management.
These dual responsibilities raise ethical concerns and often require
the services of additional treating psychiatrists and other mental
health professionals.
The Achievement by Proxy Spectrum in Youth Sports:
Historical Perspective and Clinical Approach to Pressured
and High achieving Children and Adolescents 803

This article, with examples of the descriptively defined three
stages of achievement by proxy distorted (ABPD) behavior,
should increase awareness of this field by facilitating communica¬
tion, enabling child psychiatrists and other professionals to iden¬
tify the red flags that can lead to abuse and exploitation of
children and adolescents, and further contributing toward psy
choeducation of self help skills in parents and other adults with
the goal of understanding motivations for ABPD behaviors and
risky rationalizations, in addition to creating prevention strate¬
gies.
Anabolic Androgenic Steroid Abuse and Performance
enhancing Drugs Among Adolescents 821
Michael S. Bahrke, Charles E. Yesalis, and Kirk J. Brower
Adolescents use a wide variety of drugs and supplements, includ¬
ing anabolic steroids, to improve their sports performance and
physical appearance. Prevalence rates for steroid use generally
range between 4% and 12% for male adolescents and between
0.5% and 2% for female adolescents. Although the short term
health effects of anabolic steroids, such as effects on the liver,
serum lipid levels, the reproductive and cardiovascular systems,

and moods and behavior, have been increasingly studied, the
long term health effects are not well known. Steroid users are
more likely to be male, participate in strength related sports, and
use other drugs. The effects of many other potential risk factors
have not been fully elucidated, however. Assessment of anabolic
steroid abuse includes physical, mental status, and laboratory
examinations. Steroid cessation, supportive therapy, and ad
junctive pharmacotherapies are all employed in treating steroid
abuse and dependence.
Eating Disorders Among Athletes 839
David M. Garner, Lionel W. Rosen, and Declan Barry
Research indicates that certain athletes, particularly those in
sports that emphasize leanness to enhance performance or ap¬
pearance (e.g., gymnastics, wrestling, figure skating, diving, and
ballet), are at risk for eating disorders. Little is known about the
risk for eating disorder symptoms in weight and strength train¬
ing. It is concluded that, even when mild variants of eating
disorders are observed in athletes, they should be given immedi¬
ate attention because they may severely compromise health and
performance. Practical recommendations are made for coaches
and trainers to identify the physical, psychological, and behav¬
ioral symptoms of eating disorders. Finally, suggestions are made
for addressing eating disorders in athletes.
Using Sports to Strengthen Resiliency in Children:
Training Heart 859
Carl C. Bell and Hay ward Suggs
The authors describe how a particular aspect of sports can nur¬
ture resiliency. They delineate principles for encouraging the de¬
velopment of heart in youth. Building heart in youth is a preven¬
tive public health strategy designed to inoculate against the
potentially negative effects of stress and trauma. Accordingly, it
is an essential tool in the primary prevention of stress related
disorders in youth.
The Psychotherapist and the Adolescent Athlete 867

This article describes some factors to be considered in the psycho¬
therapy of adolescent athletes. It points out that an adolescent s
experience in sports will be influenced by developmental issues
and family dynamics. Sports may become a source of problems
for the adolescent, as well as an arena for the expression of
problems that originate elsewhere. The therapy will be influenced
by the relative weight assigned to personal, family, and perfor¬
mance issues in the overall clinical picture.

Relationship violations have been documented in all service pro¬
fessions, particularly those involving a power differential in the
relationship. Although the coach athlete relationship is not a
typical therapeutic interaction, there should be professional and
ethical boundaries. Boundary violations, including sexual abuse
of a minor, do occur. Five clinical cases of relationship distur¬
bances are presented. The authors use the paradigms of transfer¬
ence, countertransference, and achievement by proxy distortion
to discuss these worrisome vignettes. Colleagues are encouraged
to collaborate in an effort to develop awareness and prevention
programs through research and education. Developing an en¬
larged workbook of similar case studies for use as a stimulus for
seminars and discussions in training environments is an immedi¬
ate goal.
A Developmental and Psychoeducational Approach to
Reducing Conflict and Abuse in Little League and Youth
Sports: The Sport Psychiatrist s Role 891

Many adults involved in organized youth sports would benefit
from a better understanding of the developmental and psycholog¬
ical needs of latency aged children and a better awareness of their
own motivations for entering a child in youth sports or for
becoming coaches themselves. Most children aged 6 to 10 years
play sports not to win, but for fun, to learn new skills, make new
friends, or be a part of a team. Before a child participates in a
sport, parents and children should fill out and compare a short
checklist to ensure that everyone s expectations for what the child
will gain from the experience are the same. It is advocated that
coaches sign a Codes of Conduct to which they would adhere,
and agree to year end evaluations by the children they coach and
their parents. It is suggested that psychiatrists offer their services
to youth sports leagues, and give coaching and parent education
courses that can, hopefully, reduce some of the (subtle and not
so subtle) child abuse that occurs on the playing field and in
the home.
